Prospektiv basalledsartrit (BJAM)
Multicenter prospektiv studie på basal ledartrit i tummen
Studien antar att det finns effektiva icke-operativa och operativa behandlingar för vissa patientpopulationer med basal ledartrit i tummen. Det finns också en funktionellt överlägsen, kostnadseffektiv och lågrisk icke-operativ eller minimalt invasiv operativ behandlingsregim för att lindra smärta och bromsa utvecklingen av sjukdom hos de med mindre avancerad sjukdom. Likaså finns det en betydande funktionell, hälsonytta och ekonomisk fördel med att kirurgiskt behandla avancerad basal ledartrit med en av de populära procedurerna som för närvarande är i praktiken.
Artrit vid basen av tummen är ett vanligt tillstånd förknippat med betydande obehag för patienten. Många icke-operativa och operativa behandlingar har beskrivits, men få prospektiva studier som involverar flera centra som jämför dessa behandlingar har gjorts. Genom att följa patienter från början av deras artritbehandling och bedöma deras framsteg vid vissa tidpunkter med hjälp av frågeformulär och röntgenbilder tror forskarna att patientresultat och tillfredsställelse kan förbättras. Denna studie kommer att inkludera patienter som behandlas icke-operativt och operativt och kommer inte att förändra patienternas vårdstandard. Genom att etablera standardiserade resultatmått och samla in data prospektivt från tidpunkten för den första presentationen genom alla behandlingsinsatser kommer det att vara möjligt att direkt jämföra behandlingar under konservativ och operativ vård av basal ledartrit i tummen. Denna multicenterstudie kommer att identifiera de specifika procedurer eller konservativa behandlingar som har störst potential att studeras i efterföljande fokuserade och väldesignade randomiserade kontrollstudier.

Basalledsartrit i tummen är ett vanligt tillstånd förknippat med betydande sjuklighet. Många icke-operativa och operativa behandlingar har beskrivits men få multicenter prospektiva evidensbaserade studier som jämför standardbehandlingar har gjorts. Detta fortsatta sökande efter konsensus om bästa kliniska praxis har granskats i en grundlig metaanalys av operativa behandlingar för basal ledartrit. Genom systematisk insamling av data angående patientspecifika egenskaper, behandlingsinterventioner och longitudinella funktionella resultatmätningar tror forskarna att patientresultat och tillfredsställelse kan förbättras genom att belysa riskfaktorer för sjukdomsprogression och tajming och val av behandlingsmodaliteter, antingen konservativa eller kirurgiskt, för en viss patient. Inrättandet av ett multicenter kliniskt register kommer i hög grad att underlätta dessa mål. Genom att etablera standardiserade resultatmått och samla in data prospektivt från tidpunkten för den första presentationen genom alla behandlingsinsatser kommer det att vara möjligt att direkt jämföra behandlingar under konservativ och operativ vård av basal ledartrit i tummen. Detta föreslagna multicenterregister kommer att identifiera de specifika procedurer eller konservativa behandlingar som har störst potential att studeras i efterföljande fokuserade och väldesignade randomiserade kontrollstudier.
Artros är den vanligaste formen av artrit och den näst vanligaste orsaken till långvariga funktionshinder i Nordamerika. Artros i basalleden i tummen är en försvagande sjukdom som drabbar en stor del av den vuxna befolkningen, även om inga stora epidemiologiska studier på förekomsten av basalledsartrit har utförts i USA.
Patogenesen av artros är fortfarande multifaktoriell men de flesta forskare är överens om att det är en degenerativ process som är ett resultat av mekaniska, biokemiska och genetiska faktorer som destabiliserar den normala balansen mellan nedbrytning och syntes av ledbrosk och subkondralt ben.
Även om det finns några utmärkta studier med randomiserad, prospektiv design som visar goda resultat med flera kirurgiska tekniker, är det fortfarande okänt vilka procedurer som är mest lämpliga för en viss patient för att uppnå det optimala funktionella resultatet och långsiktig framgång med minimalt kirurgiskt trauma. Optimal immobiliseringstyp och varaktighet är inte heller känd. Effekten av hyperextension av metakarpalfalangealleden på långtidsresultat och den optimala behandlingsmetoden för denna intilliggande led är också okänd. Denna föreslagna studie kommer att möjliggöra prospektiv datainsamling från flera institutioner över hela landet och engagera erkända experter, som alla behandlar basal ledartrit med olika kliniska protokoll.
Miljontals människor i USA har eller kommer att utveckla basalledsartros, som är den vanligast drabbade leden i den övre extremiteten som kräver kirurgisk ingrepp. Efterfrågan på en mer effektiv och kostnadseffektiv form av operativ och icke-operativ behandling av denna handikappande sjukdom är betydande. Att identifiera säkra och effektiva behandlingar bland de många tillgängliga behandlingssätten skulle sannolikt leda till betydande förbättringar av livskvaliteten för en stor del av denna befolkning. Om framtida studier visar att mindre inblandade rekonstruktioner, som enbart trapesiektomi, är lika effektiva som mer komplicerade ligamentrekonstruktioner med senor, kan många patienter dra nytta av denna mindre komplexa operation. Denna studie skulle öka läkarens och patientens medvetenhet och förtroende för effektiva behandlingsregimer.

Inklusionskriterier:
- Patienter som har symtomatisk basal ledartrit.
- Patienter som är kapabla att ge informerat samtycke
Exklusions kriterier:
- Patienter yngre än 18 år vid tidpunkten för inskrivningen
- Patienter med neuromuskulär sjukdom som påverkar den opererade handen, inte orsakad av CMC-operationen
- Patienter med kända inflammatoriska artritiska tillstånd, såsom reumatoid eller psoriasisartrit
- Patienter med en historia av eller aktuell infektion i basalleden i den drabbade handen.
- Patienter som är dementa eller inte kan ge informerat samtycke.
- Patienter som inte kan följa studiens riktlinjer.
- Patienter som har metakarpofalangeal ledhyperextension är INTE uteslutna. Dessa patienter kommer att följas och om de får en kapsulodes eller annan procedur vid MCPJ samtidigt med sin basalledsprotes kommer detta att noteras i deras dataregister.
