Basale gewrichtsartritis prospectief (BJAM)
Multicenter prospectieve studie over basale gewrichtsartritis van de duim
De studie veronderstelt dat er effectieve niet-operatieve en operatieve behandelingen bestaan voor bepaalde patiëntenpopulaties met basale gewrichtsartritis van de duim. Er bestaat ook een functioneel superieur, kosteneffectief en laag risico niet-operatief of minimaal invasief operatief behandelingsregime om pijn te verlichten en de progressie van de ziekte te vertragen bij mensen met minder gevorderde ziekte. Evenzo is er een aanzienlijk functioneel, gezondheidsnut en economisch voordeel aan het chirurgisch behandelen van geavanceerde basale gewrichtsartritis met een van de populaire procedures die momenteel in de praktijk zijn.
Artritis aan de basis van de duim is een veel voorkomende aandoening die gepaard gaat met aanzienlijk ongemak voor de patiënt. Er zijn veel niet-operatieve en operatieve behandelingen beschreven, maar er zijn weinig prospectieve onderzoeken gedaan waarbij meerdere centra deze behandelingen vergeleken. Door patiënten vanaf het begin van hun artritisbehandeling te volgen en hun voortgang op bepaalde tijdstippen te beoordelen met behulp van vragenlijsten en röntgenfoto's, denken de onderzoekers dat de resultaten en tevredenheid van de patiënt kunnen verbeteren. Deze studie omvat patiënten die niet-operatief en operatief worden behandeld en zal de zorgstandaard van patiënten niet veranderen. Door gestandaardiseerde uitkomstmaten vast te stellen en prospectief gegevens te verzamelen vanaf het moment van eerste presentatie tot en met alle behandelingsinterventies, zal het mogelijk zijn om behandelingen tijdens de conservatieve en operatieve zorg van basale gewrichtsartritis van de duim direct te vergelijken. Dit onderzoek in meerdere centra zal die specifieke procedures of conservatieve behandelingen identificeren die het grootste potentieel hebben om te worden bestudeerd in daaropvolgende gerichte en goed opgezette gerandomiseerde controleonderzoeken.

Basale gewrichtsartritis van de duim is een veel voorkomende aandoening die gepaard gaat met aanzienlijke morbiditeit. Er zijn veel niet-operatieve en operatieve behandelingen beschreven, maar er zijn weinig multicenter prospectieve, evidence-based onderzoeken gedaan waarin standaardbehandelingen werden vergeleken. Deze voortdurende zoektocht naar consensus over de beste klinische praktijken is beoordeeld in een grondige meta-analyse van operatieve behandelingen voor basale gewrichtsartritis. Door de systematische verzameling van gegevens met betrekking tot patiëntspecifieke kenmerken, behandelingsinterventies en longitudinale functionele uitkomstmetingen, geloven de onderzoekers dat de resultaten en tevredenheid van de patiënt kunnen verbeteren door de opheldering van risicofactoren voor ziekteprogressie en de timing en selectie van behandelingsmodaliteiten, hetzij conservatief of chirurgisch, voor een bepaalde patiënt. De oprichting van een klinisch register met meerdere centra zal deze doelen aanzienlijk vergemakkelijken. Door gestandaardiseerde uitkomstmaten vast te stellen en prospectief gegevens te verzamelen vanaf het moment van eerste presentatie tot en met alle behandelingsinterventies, zal het mogelijk zijn om behandelingen tijdens de conservatieve en operatieve zorg van basale gewrichtsartritis van de duim direct te vergelijken. Dit voorgestelde multi-center register zal die specifieke procedures of conservatieve behandelingen identificeren die het meeste potentieel hebben om te worden bestudeerd in daaropvolgende gerichte en goed opgezette gerandomiseerde controleonderzoeken.
Artrose is de meest voorkomende vorm van artritis en de tweede meest voorkomende oorzaak van langdurige invaliditeit in Noord-Amerika. Artrose van het basale gewricht van de duim is een slopende ziekte die een groot deel van de volwassen bevolking treft, hoewel er in de Verenigde Staten geen grote epidemiologische studies zijn uitgevoerd naar de prevalentie van basale gewrichtsartritis.
De pathogenese van artrose blijft multifactorieel, maar de meeste onderzoekers zijn het erover eens dat het een degeneratief proces is dat het gevolg is van mechanische, biochemische en genetische factoren die de normale balans van afbraak en synthese van gewrichtskraakbeen en subchondraal bot destabiliseren.
Hoewel er enkele uitstekende onderzoeken zijn met een gerandomiseerde, prospectieve opzet die goede resultaten laten zien met verschillende chirurgische technieken, is het nog onbekend welke procedures het meest geschikt zijn voor een bepaalde patiënt om het optimale functionele resultaat en succes op de lange termijn te bereiken met minimaal chirurgisch trauma. Het optimale immobilisatietype en de optimale duur is ook niet bekend. Het effect van hyperextensie van het metacarpofalangeale gewricht op de langetermijnresultaten en de optimale behandelingsmethode voor dit aangrenzende gewricht is ook onbekend. Deze voorgestelde studie zal prospectieve gegevensverzameling van meerdere instellingen in het hele land mogelijk maken en erkende experts inschakelen, die allemaal basale gewrichtsartritis behandelen met verschillende klinische protocollen.
Miljoenen mensen in de VS hebben of zullen basale gewrichtsartrose ontwikkelen, het meest aangetaste gewricht in de bovenste extremiteit dat chirurgische ingreep vereist. De vraag naar een meer effectieve en kosteneffectieve vorm van operatieve en niet-operatieve behandeling van deze invaliderende ziekte is groot. Het identificeren van veilige en effectieve behandelingen uit de vele beschikbare behandelingsmodaliteiten zou waarschijnlijk leiden tot aanzienlijke verbeteringen in de kwaliteit van leven voor een groot deel van deze populatie. Als uit toekomstige studies blijkt dat minder ingrijpende reconstructies, zoals trapeziectomie alleen, net zo effectief zijn als meer gecompliceerde ligamentreconstructies met peesinterpositie, kunnen veel patiënten baat hebben bij deze minder complexe operatie. Deze proef zou het bewustzijn van clinici en patiënten en het vertrouwen in effectieve behandelingsregimes vergroten.
Deze studie veronderstelt dat er effectieve niet-operatieve en operatieve behandelingen bestaan voor bepaalde patiëntenpopulaties met basale gewrichtsartritis van de duim. Beschrijving
Inclusiecriteria:
- Patiënten met symptomatische basale gewrichtsartritis.
- Patiënten die in staat zijn geïnformeerde toestemming te geven
Uitsluitingscriteria:
- Patiënten jonger dan 18 jaar op het moment van inschrijving
- Patiënten met een neuromusculaire aandoening die de geopereerde hand aantast, niet veroorzaakt door de CMC-operatie
- Patiënten met bekende inflammatoire artritische aandoeningen, zoals reumatoïde artritis of artritis psoriatica
- Patiënten met een voorgeschiedenis van of huidige infectie van het basale gewricht van de aangedane hand.
- Patiënten die dement zijn of geen geïnformeerde toestemming kunnen geven.
- Patiënten die zich niet aan de studierichtlijnen kunnen houden.
- Patiënten met hyperextensie van het metacarpofalangeale gewricht zijn NIET uitgesloten. Deze patiënten worden gevolgd en als ze een capsulodese of een andere procedure ondergaan in het MCPJ tegelijk met hun basale gewrichtsartroplastiek, wordt dit genoteerd in hun gegevensrecords.
