L'essai RESOLUTE-III Allcomers est une étude prospective, multicentrique, randomisée, à deux bras, internationale, de non-infériorité, en ouvert, avec 2300 patients dans 15 à 20 centres. L'étude est une étude "monde réel, tous venants".
Objectif principal : comparer le système Medtronic Endeavour-Resolute (stent à élution de Zotarolimus) avec le système Abbott XIENCE V (stent à élution d'évérolimus) en ce qui concerne la mort cardiaque, l'infarctus du myocarde (pas clairement attribuable à un vaisseau non cible), la cible Revascularisation lésionnelle à 1 an dans une population de patients "monde réel".

L'essai RESOLUTE-III Allcomers est une étude prospective, multicentrique, randomisée, à deux bras, internationale, de non-infériorité, en ouvert, avec 2300 patients dans 15 à 20 centres. L'étude est une étude "monde réel, tous venants".
Objectif principal : comparer le système Medtronic Endeavour-Resolute (stent à élution de Zotarolimus) avec le système Abbott XIENCE V (stent à élution d'évérolimus) en ce qui concerne la mort cardiaque, l'infarctus du myocarde (pas clairement attribuable à un vaisseau non cible), la cible Revascularisation lésionnelle à 1 an dans une population de patients "monde réel".
Hypothèse de l'étude : Déterminer si le stent à élution de Zotarolimus (Medtronic Endeavour-Resolute) est non inférieur au stent à élution d'évérolimus (Abbott XIENCE V) en termes de critère principal à 12 mois après l'implantation du stent.

La description
Critère d'intégration:
- Âge minimum 18 ans
- Maladie coronarienne symptomatique
- Candidat acceptable par le patient pour un traitement avec un stent à élution médicamenteuse conformément aux directives applicables
- Présence d'une ou plusieurs sténoses coronariennes > 50 % avec un diamètre de référence de 2,25 à 4,0 mm qui peut être couvert par un ou plusieurs stents
- Indication du patient, longueur de la lésion et diamètre du vaisseau conformément aux « instructions d'utilisation » des stents à l'étude
- Le patient est disposé et capable de coopérer avec les procédures de l'étude et les visites de suivi requises et le patient ou son représentant légal a été informé et accepte en signant le consentement éclairé écrit approuvé par CE.
Critère d'exclusion:
- Femmes en âge de procréer qui n'ont pas de test de grossesse négatif dans les 7 jours précédant l'intervention et femmes qui allaitent
- Intolérance connue à l'aspirine, au clopidogrel ou à la ticlopidine, à l'héparine, au cobalt, au nickel, au chrome, au molybdène, aux revêtements polymères, au zotarolimus, à l'évérolimus ou au produit de contraste
- Participer à un autre essai avant d'atteindre le critère d'évaluation principal
- Chirurgie prévue dans les 6 mois suivant l'ICP à moins que la bithérapie antiplaquettaire ne soit maintenue tout au long de la période péri-chirurgicale.

|
Pourcentage de participants ayant subi soit un décès cardiaque, soit un infarctus du myocarde (non clairement attribuable à un vaisseau non cible) ou une revascularisation de la lésion cible (TLR, cliniquement indiquée) après un an.
IM : IM Q si nouvelles ondes Q pathologiques et douleur thoracique, IM non Q si CK élevée plus de deux fois la normale, troponine élevée plus que la normale, selon les définitions de l'ARC.
TLR, cliniquement indiqué s'il est associé à des symptômes ischémiques et à un diamètre de lumière minimal angiographique supérieur à cinquante pour cent par QCA ou sans symptômes et à un diamètre de lumière minimal supérieur à soixante-dix pour cent.

Dans Stent Pourcentage de diamètre de sténose à treize mois.
Dans Stent Percent Diameter Diameter Stenosis : pourcentage mesuré de sténose de diamètre au niveau de la région du stent (calculé comme 100x(RVD-MLD)/RVD en utilisant les valeurs moyennes de 2 vues orthogonales par QCA.
RVD (diamètre du vaisseau de référence) : moyenne des segments normaux dans un rayon de 10 mm proximal et distal par rapport à la lésion cible à partir de 2 vues orthogonales à l'aide de QCA.
MLD (Minimal Lumen Diameter) : moyenne de 2 vues orthogonales du point le plus étroit dans la zone d'évaluation.
MLD mesurée pendant QCA par le laboratoire principal d'angiographie.
