Description détaillée

Il s'agit d'un essai clinique de phase II ouvert, multicentrique et à un seul bras (avec rodage de l'innocuité) évaluant l'association d'ALX148, de cetuximab et de pembrolizumab chez des patients atteints d'un cancer colorectal métastatique microsatellite stable qui a progressé sur au moins 2 lignes de thérapie systémique. Un sous-ensemble de patients subira des biopsies liées à l'étude. Il y aura une phase de rodage de sécurité suivie d'une phase d'expansion de dose. Les patients aux deux stades continueront de recevoir le traitement à l'étude jusqu'à la progression de la maladie selon RECIST v1.1.

Critère d'intégration:

Pour être admissible à participer à cette étude, une personne doit répondre à tous les critères suivants :

  • Avoir un diagnostic de cancer colorectal métastatique précédemment traité avec au moins deux lignes de traitement pour une maladie non résécable/métastatique
  • Avoir une maladie microsatellite stable
  • Fonction hématologique et des organes cibles adéquate

Critère d'exclusion:

Une personne qui répond à l'un des critères suivants sera exclue de la participation à cette étude :

  • Patients avec un statut MSI élevé connu ou un déficit connu de réparation des mésappariements (dMMR)
  • Patients chez qui la réparation des mésappariements et l'état de stabilité des microsatellites sont inconnus
  • Antécédents de réactions allergiques, anaphylactiques ou autres réactions d'hypersensibilité graves à l'un des médicaments à l'étude ou à leurs classes
  • Cancer colorectal métastatique RAS/BRAF de type sauvage du côté gauche (au niveau ou en aval de l'angle splénique) qui n'ont jamais reçu d'inhibiteur de l'EGFR.
  • Traitement antérieur avec un agent anti-PD-1, anti-PD-L1, anti-PD L2, anti-CD47 ou anti-SIRPα ou avec un agent dirigé vers un autre récepteur des lymphocytes T stimulateur ou co-inhibiteur (p. ex., CTLA- 4, OX 40, CD137)
