  2. Évaluer l'innocuité de la réparixine par rapport au placebo chez les patients inclus dans l'étude.

Aperçu de l'étude

Description détaillée

Étude multicentrique de phase 2, randomisée, en double aveugle, contrôlée par placebo. Tous les patients recevront un traitement conforme aux normes de soins actuelles en ce qui concerne la gestion du SDRA (la gestion protocolaire des ventilateurs sera mise à la disposition de tous les sites conformément aux normes de soins actuellement acceptées). Les patients seront randomisés (1:1) pour recevoir soit de la réparixine, soit un placebo. La durée du traitement sera de 14 jours.

L'étude comprendra 4 périodes d'études:

Dépistage, randomisation et évaluations de base, traitement (14 jours avec prolongation discrétionnaire jusqu'à 21 jours), suivi (jusqu'à 28 jours ou sortie de l'hôpital, selon la première éventualité, puis jusqu'au 60e jour).

Critère d'intégration:

  1. Consentement éclairé signé, conformément aux directives et réglementations locales.
  2. Adultes masculins et féminins (>18 ans).
  3. Patients ventilés mécaniquement (invasifs) avec un rapport PaO2/FIO2 ≤200 en présence d'une PEP ≥5 cmH20.
  4. Insuffisance respiratoire non entièrement expliquée par une insuffisance cardiaque ou une surcharge hydrique (si une exacerbation d'insuffisance cardiaque congestive aiguë est identifiée dans le cadre du tableau clinique, cela doit être traité efficacement et dès que possible avant que le patient puisse être inscrit).
  5. Opacités radiologiques bilatérales compatibles avec un œdème pulmonaire sur la radiographie thoracique frontale (CXR), ou opacités bilatérales en verre dépoli sur une tomodensitométrie (TDM) thoracique.
  6. ≤ 48 heures après avoir rempli les critères ARDS ci-dessus.
  7. ≤ 7 jours après l'admission à l'hôpital.

    3. Abstinence.

Les participantes en âge de procréer ou en situation de post-ménopause depuis au moins 1 an seront admises. Pour toutes les femmes en âge de procréer, le résultat du test de grossesse doit être négatif avant la première prise de médicament.

Critère d'exclusion:

  1. Maladie hépatique chronique modérée à sévère (telle que vérifiée par les antécédents pertinents, l'imagerie, si préexistante, et le score de Child-Pugh B-C).
  2. Insuffisance rénale chronique sévère : DFGe (MDRD) < 30 mL/min/1,73 m2 ou Insuffisance rénale en phase terminale sous thérapie de remplacement rénal.
  3. Participation à un autre essai clinique interventionnel.
  4. Patients dont il est cliniquement déterminé qu'ils présentent une forte probabilité de décès dans les prochaines 24 heures selon l'estimation de l'IP.
  5. Preuve de lésion cérébrale anoxique
  6. Reçoit actuellement ECMO ou ventilation oscillatoire à haute fréquence.
  7. Extubation anticipée dans les 24 heures suivant l'inscription.
  8. Malignité active (à l'exception des cancers de la peau non mélaniques).
  9. Instabilité hémodynamique (> 30 % d'augmentation du vasopresseur au cours des 6 dernières heures ou noradrénaline > 0,5 mcg/Kg/min).
  10. Preuve de dysmotilité gastro-intestinale (GI), par exemple, due à une pancréatite aiguë ou à un état post-opératoire immédiat, comme démontré par une distension gastrique persistante, une intolérance à l'alimentation entérale et/ou des résidus gastriques persistants > 500 ml).
  11. Sortie anticipée de l'hôpital ou transfert vers un autre hôpital dans les 72 heures suivant le dépistage.

  13. L'histoire de:

    1. Allergie / hypersensibilité documentée à plus d'un médicament appartenant à la classe des sulfamides, tels que la sulfaméthazine, le sulfaméthoxazole, la sulfasalazine, le nimésulide ou le célécoxib (l'hypersensibilité aux antibiotiques sulfanilamides seuls, par exemple, le sulfaméthoxazole n'est pas éligible à l'exclusion), et au produit à l'étude et /ou ses excipients.
    2. Déficit en lactase, galactosémie ou malabsorption du glucose-galactose.
    3. Antécédents de saignement ou de perforation gastro-intestinale en raison d'un traitement antérieur par des anti-inflammatoires non stéroïdiens (AINS) ou d'un ulcère gastro-duodénal / d'une hémorragie récurrente.

Expérimental: Réparixine + Norme de soins
Comprimés de réparixine 1200 mg TID (2 comprimés x 600 mg TID) en complément de la norme de soins (SoC).
Comprimés de Reparixine à 600 mg, administrés écrasés par sonde nasogastrique à la dose de 1 200 mg trois fois par jour (2 comprimés trois fois par jour administrés environ toutes les 8 heures environ) en complément de la norme de soins. Après extubation et si le patient peut avaler, la réparixine peut être administrée par voie orale. Durée totale du traitement : 14 jours

Radiographic Assessment of Lung Edema (RALE) score is a validated, semi-quantitative tool for quantifying pulmonary edema and ARDS severity using chest X-rays (CXR). It is calculated by dividing the lung fields on the chest radiograph into four quadrants. Each quadrant was assigned a number, and the extent of alveolar opacities (the consolidation score, from 0 to 4) and density of alveolar opacities (the density score, from 1 to 3) was determined. If the consolidation score was 0, the density score was 0. The final RALE score was the sum of the product of the consolidation and density score for each quadrant. Thus, the final RALE score ranged from minimum 0 to maximum 48. Higher RALE scores indicate more severe edema and poorer outcomes. Plasma biomarker levels for Interleukin -6 (IL-6), IL-8 and plasma Tumor Necrosis Factor Receptor 1 (TNFr-1) were measured using validated laboratory methods. Baseline was defined as the last measurement collected prior to IMP intake, regardless of the visit at which it was obtained. Change from baseline was calculated as the post-baseline plasma concentration minus the baseline concentration.

Plasma biomarker levels of plasminogen activator inhibitor-1 (PAI-1), intercellular adhesion molecule-1 (ICAM-1), and receptor for advanced glycation end products (RAGE) were measured using validated laboratory methods. Number of Participants Reporting Treatment Emergent Adverse Events (TEAEs) and Serious Treatment Emergent Adverse Events (Serious TEAEs)

An Adverse Event (AE) is any untoward medical occurrence in a participant, which does not necessarily have a causal relationship with the trial intervention. A TEAE is defined as any untoward medical occurrence that begins or worsens in intensity or frequency after the initiation of a treatment (e.g., drug, device, or procedure) in a clinical study. A Serious TEAE is defined as any untoward medical occurrence that at any dose: results in death, is life-threatening, requires inpatient hospitalization or prolongation of existing hospitalization.
