Gli obiettivi della ricerca attuale sono adattare un efficace intervento clinico di psicoterapia walk-in al contesto delle cure primarie a bassa barriera; e condurre uno studio di implementazione pilota in due cliniche di assistenza primaria a bassa barriera che hanno sede in un'organizzazione comunitaria che è un centro di accoglienza per persone senzatetto nel nord di Seattle.

Le interazioni tra le molteplici minacce per la salute rappresentate dalle malattie mentali, dall’uso di sostanze e dalle vulnerabilità sociali (senzatetto, povertà) rappresentano una sindrome che perpetua l’epidemia di HIV.
È stato dimostrato che i disturbi mentali e l’uso di sostanze aumentano i comportamenti a rischio di HIV e hanno un impatto negativo sull’aderenza alla PrEP.
Gli individui senza alloggio devono affrontare ostacoli all’assistenza sanitaria sia a livello individuale (stigma, supporto sociale limitato e sintomi psichiatrici) che a livello strutturale (povertà, alloggi instabili, trasporti limitati, costo delle cure mediche).
Le cliniche walk-in sono progettate per affrontare questi ostacoli all’assistenza primaria tradizionale e fornire accesso immediato ai servizi quando sono necessari.
King County WA ha finanziato quattro cliniche di assistenza primaria a bassa barriera dal 2018 come componente chiave del piano EHE di King County.
I dati di queste cliniche suggeriscono che i disturbi mentali non trattati rappresentano un ostacolo fondamentale all’impegno nelle cure primarie a bassa barriera.
I modelli di assistenza integrati in cui specialisti qualificati della salute mentale forniscono assistenza sanitaria mentale basata sull’evidenza in contesti di assistenza primaria aumentano l’accesso alle cure fornendo cure di salute mentale laddove le persone stanno già cercando assistenza.
L’assistenza integrata migliora i risultati in termini di salute mentale e i modelli possono essere implementati anche in contesti con risorse molto scarse.
Gli ambulatori di consulenza in tutta la provincia dell'Ontario, in Canada, hanno implementato un intervento di terapia narrativa a sessione singola per aumentare l'accesso alle cure di salute mentale.
In questo modello, i terapisti utilizzano un approccio a sessione singola per aiutare i clienti a identificare i problemi, riconoscere e sviluppare i propri punti di forza e sviluppare un piano d’azione.
Gli approcci narrativi creano un modo non patologizzante, collaborativo e orientato alle competenze per affrontare i bisogni di salute mentale.
Data l’imprevedibilità del follow-up nell’assistenza primaria ambulatoriale, un approccio narrativo a sessione singola può sfruttare al meglio ogni singola sessione, fornendo conversazioni terapeutiche pragmatiche alle persone quando ne hanno bisogno e supportando il coping, riducendo i sentimenti di isolamento e aumentando il senso di validazione.
Gli obiettivi del presente studio sono 1) sviluppare un intervento di terapia narrativa a sessione singola per aumentare l'accesso alle cure di salute mentale tra le persone senza dimora; e 2) condurre uno studio pilota di implementazione di 4 mesi dell'intervento adattato in due cliniche di assistenza primaria a bassa barriera nel nord di Seattle.

Sperimentale: Intervento di terapia narrativa a sessione singola
Ai pazienti delle cliniche a bassa barriera [in cieco] verrà offerta una singola sessione di terapia narrativa con un operatore sanitario comportamentale autorizzato e qualificato.

L'intervento di terapia narrativa proposto a sessione singola è stato ampiamente implementato nelle cliniche di terapia walk-in nella provincia dell'Ontario, in Canada.
In queste cliniche sono disponibili sessioni di psicoterapia di 60 minuti, basate sulla terapia narrativa.
Il terapeuta collabora con un paziente per co-sviluppare un focus utile e significativo per la sessione e quindi coinvolge il paziente in una conversazione rispettosa ma che cambia il suo modo di pensare al problema presentato.
Il terapista evidenzia e documenta i valori e le capacità del paziente e lavora con lui per creare un piano d'azione.
Se il terapista identifica ulteriori necessità di trattamento, al paziente viene offerto un rinvio esterno a servizi specialistici di salute mentale.

Staff, therapist and pilot participant in-depth interviews were conducted post-implementation by research team members.
They explored stakeholder perspectives on who was not reached, why they were not reached and opportunities for improvement (Reach); SSNT impact on clients (Effectiveness); acceptability and appropriateness of the intervention, and determinants of implementation (Implementation).
After interviews were collected, we used rapid deductive qualitative analysis to assess these outcomes, and generated a table with key findings.

The Single Session Impression and Feedback Tool (SSIFT) is considered as an indicator of acceptability.
Developed for walk-in clinics implementing SSNT, it includes eight items on a 7-point scale (range 1-7) that invite feedback from participants on their perception of the process and outcome of the SSNT intervention.
Six items focus on how collaborative the service was, one on the degree of hope the conversation engendered, and one on the conversation's usefulness.
We report the mean across the eight items, and higher scores indicate more favorable impressions of the SSNT intervention.
As the SSIFT was designed as a therapist feedback tool, its psychometric properties have not been reported.
