小児の一般入院および死亡率に対するワクチンキャンペーンの効果 (RE-CAMP)
はしかワクチンと経口ポリオワクチンが一般病院への入院と子供の死亡率に及ぼすキャンペーン効果に関するクラスターランダム化比較試験
世界は、今後 10 年間で麻しんとポリオの感染を根絶することを目指しています。 両方の感染が抑制されれば、はしかと経口ポリオワクチンのキャンペーンは段階的に廃止されます。 これは、低所得国の子どもの生存にとって、良いことよりも害を及ぼす可能性があります。 ギニアビサウなどのBandim Health Projectの研究により、はしかと経口ポリオの生ワクチンには有益な非特異的効果があることが明らかになりました。

仮説は次のとおりです。
RECAMP-MV: ギニアビサウでのはしか予防接種キャンペーンは、はしか感染が限られている状況で、生後 9 か月から 59 か月の子供の罹患率と死亡率をその後の 18 か月で 80% 減少させました。
RECAMP-OPV: ギニアビサウでの経口ポリオワクチン接種キャンペーンにより、ポリオに感染していない場合、その後の 12 か月間に生後 0 か月から 8 か月の子供の罹患率と死亡率が 25% 減少します。
当初、試験は 182 のクラスターで実施され、21,000 人の子供が登録される予定でした。 改訂されたサンプル サイズの計算とデータの安全性および監視委員会との話し合いに続いて、クラスターの数は 222 に増加し、計画された登録数は 21,000 から 28,000 に増加しました (RECAMP-MV: 18000、RECAMP-OPV: 10000)。
OPV の有益な非特異的効果の少なくとも一部が腸および/または呼吸器マイクロバイオームの変化によって引き起こされているという仮説を調査するために、サブグループのマイクロバイオーム サンプルを収集します。
鼻スワブと直腸スワブは、介入群に割り当てられた 50 人の乳児、および対照群に割り当てられた 50 人の乳児から収集されます。 1 つは RECAMP-OPV に募集されたとき、もう 1 つは 2 か月後に乳児ごとに 2 つのサンプルが収集されます。

包含基準: 親/保護者の同意があれば、田舎の Bandim Health Project Health and Demographic Surveillance Site に登録されている家族と同居する 0 ~ 59 か月の子供が含まれます。
除外基準:
- 子供の体温が 39.0°C を超えるか、検査を担当する看護師によって定義された重度の急性疾患にかかっている
また
- 子供は上腕の周囲が110mm未満で、生後6か月以上です(エイズおよび慢性免疫抑制疾患の最も現実的な局所指標)
また
- 子供は、以前のワクチン接種、薬物または食品の後に重度のアレルギー反応を経験しました。
また
- -子供はバチルスカルメットゲランワクチンの進行中の研究に登録されており、生後2か月未満です

介入村では、生後 0 ~ 8 か月の子供の体重を測定し、標準的な経口ポリオ ワクチンを 1 回または 2 回接種します。

世界保健機関によって事前に認定された 2 価経口ポリオ ワクチンは、1 回または 2 回の接種で、地元の看護師が 1 回の接種につき 2 滴ずつ、ワクチン接種者の口に直接投与されます。
