200 人のリスクの高い高齢者 (70 ~ 89 歳で、認知テストで通常のパフォーマンスが低く、介入の対象となる 2 つ以上の修正可能な危険因子) を 2 年間の体系的マルチドメイン アルツハイマー病リスク軽減試験にランダム化することを提案します。 (SMARRT) 介入または健康教育 (HE) コントロール。
SMARRT チームは、介入群に無作為に割り付けられた参加者と協力して、リスク軽減に対処するための調整された行動計画を作成します。 対象となる分野には次のものが含まれます。身体的、精神的、社会的活動の増加。心血管危険因子(糖尿病、高血圧)の制御;禁煙;抑うつ症状の軽減;睡眠の改善;神経保護食;潜在的に有害な薬の使用を減らす。 HE 参加者は、これらのトピックに関する定期的な資料を郵送で受け取ります。

- Cognitive Abilities Screening Instruments (CASI) を使用して測定された、簡単な電話認知スクリーンでの通常のパフォーマンスの低下。 低正常スコアは 26 ~ 29 と定義されます。
- 介入の対象となる追加の危険因子が少なくとも 2 つある。
除外基準:
- 熟練した看護施設またはリハビリテーション施設に居住している
- 緩和ケアやホスピスサービスを受けている
- -5を超えるチャールソン併存疾患指数スコア
- 双極性障害または統合失調症
- 現在のアルコールまたは薬物使用障害
- 慢性オピオイド療法を受けている

- 重度の視覚障害または聴覚障害

アクティブコンパレータ:スマート介入
SMARRT 介入チームは、標準化された手順を使用して、SMARRT 介入群に無作為に割り付けられた各参加者の個別化されたアルツハイマー病のリスク プロファイルを作成します。
その後、参加者はインターベンショニストと直接会ってリスクプロファイルを確認し、最初の個別のリスク軽減アクションプランを作成します。
COVID中に登録された少数の参加者については、最初の介入主義者の訪問が電話で行われました。
対象となる分野には次のものが含まれます。身体的、精神的、社会的活動の増加。禁煙;健康的なダイエット;糖尿病患者の低血糖の回避を含む、心血管危険因子(糖尿病、高血圧)の制御;抑うつ症状の軽減;睡眠の改善;潜在的に有害な薬の使用を減らす。

介入担当者は、特定のリスク軽減行動計画の個別化を可能にする SMARRT 介入を提供するための標準プロトコルに従います。これらの計画は、参加者の進捗状況、動機、好み、または新たに特定されたリスク要因に応じて、時間の経過とともに進化します。
スタッフは追跡データベースを使用して、セッションの日付、特定されたリスク要因、動機付けの障壁と重要な価値、および開発目標に関する議論の結果など、各参加者の情報を記録します。
参加者ごとに連絡先の正確な数と方法 (電話または対面) は異なりますが、各参加者と少なくとも月に 1 回は連絡を取ることを目指します。
ベスト プラクティスには、2 年間の介入期間中の年 2 回の対面ミーティングが含まれます。
