경련성 편마비 환자의 과도한 족저굴곡에 대한 동적 부목: 보행 분석에 대한 무작위 통제 연구
이 연구의 목적은 뇌졸중이나 외상성 뇌손상으로 인한 경직성 고지혈증 환자의 보행 패턴 개선을 위한 onabotulinum toxin A(BTX) 주사(긴장 관리)와 동적 부목(구축 감소) 및 그 결과 과도한 족저 굴곡의 효과를 조사하는 것입니다.

포함 기준:
등록된 환자는 남성 또는 여성이고 18세 이상이며 다음을 포함하는 경련성 편마비 및 과도한 저굴곡으로 진단되었습니다.
- 본 연구에 등록하기 전 6개월 이상 뇌졸중 또는 외상성 뇌 손상 진단
- 발목 배측굴곡 시 AROM 및 PROM 감소
- 발목 보조기(AFO) 없이 20피트를 안전하게 걸을 수 있는 능력
- 초기 발뒤꿈치 접촉으로 걸을 수 없음
- 6개월 이내에 사전 BTX 치료 없음
- -10° 이상의 R1(Tardeiu 평가 방법 사용)
- 족저굴곡근에 대한 평균 Ashworth Scale Test ≥3
제외 기준:
- 벨 마비
- 바이러스성 뇌염
- 근이영양증
- 다발성 경화증
- 발/발목의 이전 발뒤꿈치 길이 연장 또는 힘줄 이동 절차
- 발목의 고정 구축이 있는 피험자
- 임신 중이거나(양성 소변 임신 검사), 모유 수유 중이거나 가임기이며 신뢰할 수 있는 산아제한 방법을 시행하지 않는 영아가 있는 여성 피험자
- 출혈 장애
- 신경근 기능을 방해할 수 있는 질병의 진단(예: 중증 근무력증, Lambert-Eaton 근무력증 증후군, 근위축성 측삭 경화증)
- 현재 아미노글리코시드계 항생제, 큐라레 유사 제제 또는 신경근 기능을 방해할 수 있는 기타 제제를 사용 중인 피험자
- 주사 대상 부위의 근력이 심하게 약하거나 위축된 피험자
- 활동성 전신 감염 또는 주사 부위 감염
- 보툴리눔 독소 A에 대한 알레르기 또는 민감성

실험적: 다이나스플린트
실험적 Dynasplint 그룹의 환자는 onabotulinum 독소를 포함한 현재 표준 치료로 치료되며 발목 배측굴곡 Dynasplint를 사용합니다.

동적 부목은 구축을 줄이기 위해 TERT(Total End Range Time)를 늘리기 위해 조정 가능한 장력을 보정한 LLPS(Low-Load Prolonged Stretch) 프로토콜을 활용합니다.
Dynasplint 또는 "Experimental" 그룹은 이 요법을 표준 치료 요법에 추가합니다.
