描述
纳入标准:
入组患者将是男性或女性,至少 18 岁,并被诊断患有痉挛性偏瘫和过度跖屈,其中包括以下内容:
- 在参加本研究前 6 个月以上诊断为中风或创伤性脑损伤
- 踝关节背屈时 AROM 和 PROM 减少
- 能够在没有踝足矫形器 (AFO) 的情况下安全行走 20 英尺
- 无法通过初始足跟接触走动
- 6 个月内未接受 BTX 治疗
- R1 为 -10° 或更大(使用 Tardeiu 评估方法)
- 跖屈肌的平均 Ashworth 量表测试≥3
排除标准:
- 贝尔麻痹
- 病毒性脑炎
- 肌营养不良症
- 多发性硬化症
- 足部/踝部先前的跟绳延长或肌腱转移手术
- 踝关节固定挛缩症患者
- 怀孕(尿妊娠试验阳性)、正在哺乳的婴儿或有生育潜力但未采用可靠的节育方法的女性受试者
- 出血性疾病
- 诊断可能干扰神经肌肉功能的疾病(即 重症肌无力、兰伯特-伊顿肌无力综合症、肌萎缩侧索硬化)
- 目前正在使用氨基糖苷类抗生素、箭毒类药物或其他可能干扰神经肌肉功能的药物的受试者
- 注射目标区域肌肉极度虚弱或萎缩的受试者
- 活动性全身感染或注射部位感染
- 对肉毒杆菌毒素 A 过敏或敏感
