ACHTERGROND:
Van verschillende doses aspirine is aangetoond dat ze effectief zijn bij het voorkomen van trombose of vasculaire occlusie bij verschillende klinische aandoeningen. Kortetermijnstudies hebben de werkzaamheid van aspirine gedocumenteerd bij het voorkomen van occlusie van bypasses van de vena saphena, het voorkomen van een myocardinfarct bij patiënten met instabiele angina pectoris, het voorkomen van voorbijgaande ischemische aanvallen en beroerte bij mannen met cerebrale vasculaire aandoeningen, het voorkomen van occlusie van gewonde kransslagaders na transluminale angioplastiek en helpen bij het verminderen van myocardinfarct en totale mortaliteit bij patiënten die fibrinolytische therapie krijgen. Bovendien is aspirine effectief geweest bij de secundaire preventie van een myocardinfarct bij proefpersonen met een bekende coronaire hartziekte. De resultaten van de Physicians' Health Study, een grootschalige primaire preventiestudie van aspirine bij mannelijke artsen, hebben een afname van het aantal hartinfarcten, een niet-significante toename van cerebrale vasculaire gebeurtenissen en geen verschil in algehele mortaliteit aangetoond. Er zijn echter maar weinig studies die de werkzaamheid van aspirine bij vaatziekten bij vrouwen hebben onderzocht, en het is mogelijk dat de risico-batenverhouding bij vrouwen anders is. Er zijn met name geen grote primaire preventieonderzoeken uitgevoerd bij vrouwen die risico lopen op coronaire hartziekten, vooral na de menopauze.
ONTWERP VERHAAL:
De Women's Health Study (WHS) is een gerandomiseerde, dubbelblinde, placebogecontroleerde studie met een 2x2 factorieel ontwerp. De WHS wordt gesponsord door zowel NHBLI (HL080467) als NCI (CA047988). Ongeveer 1,75 miljoen vrouwelijke gezondheidswerkers werden per post gecontacteerd om te bepalen of ze geschikt waren voor deelname aan het onderzoek. Er werd een inloopfase van drie maanden uitgevoerd om degenen met een slechte therapietrouw eruit te filteren. Randomisatie, die begon in februari 1993 en eindigde in januari 1996, was gestratificeerd in leeftijdsgroepen van vijf jaar. In totaal werden 39.876 deelnemers willekeurig toegewezen aan vitamine E (600 IE om de dag) of placebo; en aspirine (100 mg om de andere dag) of placebo. IN het 2x2 factoriële ontwerp werden vrouwen willekeurig toegewezen aan actieve aspirine en placebo vitamine E (n=9.968), placebo aspirine en actieve vitamine E (n=9.971), actieve aspirine en actieve vitamine E (n=9.966) of placebo aspirine en placebo vitamine E (n=9.971). In de hoofdanalyses werden alle vrouwen op actieve aspirine (n=19.934) vergeleken met vrouwen op placebo-aspirine (n=19.942); en alle vrouwen op actieve vitamine E (n=19.937) werden vergeleken met vrouwen op placebo-aspirine (n=19.939).
Als onderdeel van de eerste proef werden prerandomisatiebloedmonsters van 28.345 deelnemers ingevroren en opgeslagen voor genetische analyse die werd ondersteund door niet-federale bronnen.
Het primaire eindpunt is de verlaging van het risico op alle belangrijke vasculaire gebeurtenissen (een gecombineerd eindpunt van niet-fataal myocardinfarct, niet-fatale beroerte en totale cardiovasculaire dood) en een afname van de incidentie van totale maligne neoplasmata van epitheelceloorsprong. Secundaire eindpunten zijn de afzonderlijke componenten van de gecombineerde eindpunten. De naleving wordt gemeten aan de hand van antwoorden op een jaarlijks verzonden vragenlijst. De proef werd voltooid in 2004 en de resultaten werden gepubliceerd in 2005 (N Engl J Med 2005;352:1293-304; JAMA 2005;294:47-55; JAMA 2005;294:56-65).
Momenteel worden vrouwen op observatiebasis gevolgd.
