Percutane leverbiopsie wordt al tientallen jaren gebruikt om de ernst van chronische leverziekte te beoordelen, ongeacht de etiologie. Tijdens deze procedure wordt een kernmonster van de lever verkregen en histologisch onderzocht op de aanwezigheid van ontsteking, fibrose en andere kenmerken die kenmerkend zijn voor specifieke leveraandoeningen. Hoewel leverbiopsie de gouden standaard is om de ernst van de leverziekte te beoordelen, heeft de procedure aanzienlijke beperkingen. Leverbiopsie is een kostbare, invasieve procedure met risico's voor morbiditeit en mortaliteit. Bovendien zijn leverbiopsie en onderzoek van leverhistologie onderhevig aan steekproefvariatie en de manier waarop deze bevindingen worden geëvalueerd en gerapporteerd door individuele pathologen.
Vanwege deze beperkingen hebben verschillende onderzoekers geprobeerd alternatieve methoden te ontwikkelen waarmee de ernst van de leverziekte kan worden beoordeeld. Eén benadering was de ontwikkeling van serummarkers die leverfibrose kunnen schatten. Dergelijke tests zijn ontwikkeld door een reeks serumleverchemieën en het aantal bloedplaatjes te analyseren. Helaas kan de test geen subtielere veranderingen in leverfibrose detecteren en geeft hij geen informatie over de leverfunctie bij patiënten met vastgestelde cirrose. Het concept van een metabole leverfunctietest, die kan worden gebruikt om de leverfunctie te beoordelen, werd enkele decennia geleden voor het eerst onderzocht (20). Dergelijke tests worden uitgevoerd door oraal of intraveneus een verbinding toe te dienen. De verbinding wordt door de lever uit het bloed verwijderd, gemetaboliseerd en een stofwisselingsproduct wordt weer afgegeven aan het bloed en uitgescheiden in de urine, speeksel of uitgeademde lucht; of het metabolische product wordt uitgescheiden in de gal. Het meten van de hoeveelheid van het toegediende product die in de loop van de tijd in het serum achterblijft of de hoeveelheid metabolisch product die wordt geproduceerd en/of de snelheid waarmee dit product wordt uitgescheiden, geeft een nauwkeurige meting van de metabole functie van de lever.
Ademtesten met behulp van 13C-gelabelde substraten bieden een veilige, niet-invasieve manier om het levermetabolisme te meten. 13C is een stabiele, niet-radioactieve isotoop die op een specifieke locatie in een testsubstraat kan worden opgenomen, zodat het vrijkomt wanneer de verbinding door de lever wordt gemetaboliseerd. Idealiter zou de 13C-verbinding oraal worden toegediend, snel worden geabsorbeerd, gemetaboliseerd door de lever en zou 13CO2 binnen 20-30 minuten worden gemeten in uitgeademde lucht. Het levermetabolisme van de verbinding zou worden beoordeeld door de verhouding van 13C/12C in uitgeademde lucht te meten. De mogelijkheid om 13C en 12C in uitgeademde CO2 te detecteren, te differentiëren en te kwantificeren is enorm vergemakkelijkt door de recente ontwikkeling van het Breath ID®-verzamelsysteem en de analyse-eenheid. Dit draagbare apparaat detecteert continu de uitgeademde adem en analyseert CO2 in realtime via een neuscanule die door de patiënt wordt gedragen.

Beschrijving
Inclusiecriteria:
- Volwassen mannen en vrouwen (leeftijd 18+)
- Leverhistologie consistent met NAFLD/NASH uitgevoerd in de afgelopen 24 maanden
- Patiënten met cirrose moeten echografie, computertomografie (CT) scan of magnetische resonantie beeldvorming (MRI) onderzoek van de lever ondergaan in de afgelopen 6 maanden waarbij geen bewijs voor hepatocellulair carcinoom wordt aangetoond
Uitsluitingscriteria:
- Elke leverziekte buiten NAFLD/NASH
- Ernstig congestief hartfalen
- Ernstige pulmonale hypertensie
- Chronische nierinsufficiëntie gedefinieerd door een serumcreatinine boven normaal
- Ongecontroleerde diabetes mellitus
- Elke auto-immuunziekte die momenteel wordt behandeld met immuunonderdrukkende medicatie
- Bewezen of vermoed hepatocellulair carcinoom
- Eerdere chirurgische bypassoperatie voor morbide obesitas
- Uitgebreide dunnedarmresectie
- Patiënten die momenteel totale parenterale voeding krijgen
- Ontvangers van een orgaantransplantatie
- Vrouwen die zwanger zijn
- Patiënten die, naar de mening van de onderzoeker, niet in deze studie zouden moeten worden opgenomen
