Studie om het beheer en de resultaten van gehospitaliseerde patiënten met gecompliceerde urineweginfecties te beoordelen (RESCUING) (RESCUING)
Een retrospectieve observationele studie om de klinische behandeling en resultaten te beoordelen van gehospitaliseerde patiënten met gecompliceerde urineweginfectie in landen met een hoge prevalentie van multidrug-resistente gramnegatieve bacteriën.

Bacteriële resistentie tegen antimicrobiële geneesmiddelen is een groot probleem voor de volksgezondheid. De grootste zorg is de snelle opkomst en verspreiding van resistentie tegen cefalosporines van de derde generatie in Enterobacteriaceae, vooral Escherichia coli en Klebsiella pneumoniae. Dit wordt vaak gezien in combinatie met resistentie tegen andere antibioticaklassen, wat leidt tot een multidrugresistentie (MDR) profiel. Deze MDR-isolaten zijn vaak betrokken bij gecompliceerde urineweginfecties (cUTI) en gaan gepaard met slechte klinische resultaten. Bovendien is er in het afgelopen decennium een opkomst geweest van carbapenemase-producerende Enterobacteriaceae, die ook resistent zijn tegen andere antibiotica dan carbapenems, waardoor er weinig therapeutische opties overblijven.
Het algemene doel van deze studie is om informatie te verschaffen over de epidemiologie, klinische behandeling en uitkomst van patiënten die in het ziekenhuis zijn opgenomen met cUTI, waaronder pyelonefritis. Om dit doel te bereiken, zullen de onderzoekers een multicenter retrospectieve observationele studie uitvoeren bij patiënten met gehospitaliseerde cUTI. De studie zal worden uitgevoerd in verschillende Europese landen plus Israël, die een hoge prevalentie hebben van MDR Gram-negatieve bacteriën (GNB), waaronder Pseudomonas aeruginosa. De studie zal proberen mogelijke beïnvloedbare risicofactoren voor het falen van de behandeling te identificeren, met name die welke verband houden met antibiotische therapie.
Na afronding van deze studie zullen de onderzoekers een beter begrip ontwikkelen van de klinische behandeling van gehospitaliseerde patiënten met cUTI als gevolg van MDR GNB in Europese landen. De onderzoekers zullen de aanpasbare risicofactoren voor falen van de behandeling identificeren. Deze informatie zal worden gebruikt om beter klinisch en antibioticumbeheer van cUTI te informeren om de patiëntresultaten te verbeteren. De studie zal ook worden gebruikt bij de ontwikkeling van hypothesen voor toekomstige gerandomiseerde gecontroleerde studies in cUTI met nieuwe antibiotica. Verspreiding van de resultaten zal plaatsvinden door middel van collegiaal getoetste publicaties en conferentiepresentaties.

Beschrijving
Inclusiecriteria:
Patiënten met een urineweginfectie en ten minste een van de volgende onderliggende aandoeningen:
- Inwonende urinekatheter
- Urineretentie (minstens 100 ml resterende urine na het plassen)
- Neurogene blaas
- Obstructieve uropathie (bijv. nefrolithiase, fibrose)
- Nierfunctiestoornis veroorzaakt door intrinsieke nierziekte: geschatte glomerulaire filtratiesnelheid (eGFR) <60 ml/min
- Niertransplantatie
- Aanpassingen aan de urinewegen, zoals een ileale lus of buidel
- Pyelonefritis en normale anatomie van de urinewegen
en ten minste een van de volgende tekenen of symptomen:
- Rillingen of rillingen geassocieerd met koorts of onderkoeling (temperatuur hoger dan 38ºC of lager dan 36ºC)
- Flankpijn (pyelonefritis) of bekkenpijn (cUTI)
- Dysurie, urinaire frequentie of urinaire urgentie
- Costo-wervelhoekgevoeligheid bij lichamelijk onderzoek
- UTI-gerelateerde veranderde mentale toestand
en ten minste een van de volgende microbiologische resultaten:
- Urinecultuur met ten minste 105 CFU/ml of meer van een uropathogeen (niet meer dan 2 soorten)
- Ten minste één bloedkweek waarin mogelijke uropathogenen groeien (niet meer dan 2 soorten) zonder andere duidelijke infectieplaats
Uitsluitingscriteria:
- Patiënten jonger dan 18 jaar
- Prostatitis
- Polymicrobiële infecties waaronder Candida spp.
- Polymicrobiële infecties die meer dan 2 bacteriesoorten omvatten
- cUTI met Candida spp. als enig uropathogeen
