Beschrijving
Inclusiecriteria:
- Vergevorderde stadium III/IV epitheliale eierstok-, eileider- of primaire peritoneale kanker hebben
- Bereid en in staat zijn om schriftelijke geïnformeerde toestemming/instemming te geven voor het proces.
- 18 jaar of ouder zijn op de dag van ondertekening van geïnformeerde toestemming.
- Suboptimale cytoreductieve chirurgie gedefinieerd als elke residuele ziekte die per operatieverslag wordt opgemerkt en/of meetbare/macroscopische ziekte heeft (gedefinieerd als doel- en/of niet-doellaesies) op basis van RECIST 1.1.
- Wees bereid om weefsel te verstrekken van een nieuw verkregen kern of excisiebiopsie van een tumorlaesie. Nieuw verkregen wordt gedefinieerd als een monster dat tot 6 weken (42 dagen) voorafgaand aan de start van de behandeling op dag 1 is verkregen. ontoegankelijk of onderwerp van veiligheidsoverwegingen) mag een gearchiveerd exemplaar alleen indienen na toestemming van de sponsor.
- Een prestatiestatus van 0, 1 of 2 hebben op de ECOG-prestatieschaal.
- Demonstreer een adequate orgaanfunctie
- Alle screeningslaboratoria moeten binnen 28 dagen na aanvang van de behandeling worden uitgevoerd.
- Vrouwelijke proefpersonen in de vruchtbare leeftijd dienen een negatieve urine- of serumzwangerschap te hebben binnen 72 uur voorafgaand aan het ontvangen van de eerste dosis onderzoeksmedicatie.
- Vrouwelijke proefpersonen in de vruchtbare leeftijd moeten bereid zijn om 2 anticonceptiemethoden te gebruiken of chirurgisch steriel te zijn, of zich te onthouden van heteroseksuele activiteit gedurende het onderzoek tot 120 dagen na de laatste dosis studiemedicatie.
Uitsluitingscriteria:
- Neemt momenteel deel aan en krijgt studietherapie of heeft deelgenomen aan een studie van een onderzoeksgeneesmiddel en heeft studietherapie ontvangen of een onderzoeksapparaat gebruikt binnen 4 weken na de eerste dosis van de behandeling.
- Heeft een diagnose van immunodeficiëntie of krijgt systemische therapie met steroïden of een andere vorm van immunosuppressieve therapie binnen 7 dagen voorafgaand aan de eerste dosis van de proefbehandeling.
- Heeft een bekende geschiedenis van actieve tuberculose (Bacillus Tuberculosis)
- Overgevoeligheid voor pembrolizumab of een van de hulpstoffen.
- Heeft eerder een monoklonaal antilichaam (mAb) tegen kanker gehad binnen 4 weken voorafgaand aan studiedag 1 of die niet is hersteld (d.w.z. ≤ Graad 1 of bij baseline) van bijwerkingen als gevolg van middelen die meer dan 4 weken eerder zijn toegediend.
Eerder chemotherapie, gerichte therapie met kleine moleculen of bestraling heeft gehad binnen 2 weken voorafgaand aan studiedag 1 of die niet is hersteld (d.w.z. ≤ Graad 1 of bij baseline) van bijwerkingen als gevolg van een eerder toegediend middel.
- Opmerking: als de proefpersoon een grote operatie heeft ondergaan, moet hij voldoende zijn hersteld van de toxiciteit en/of complicaties van de ingreep voordat met de therapie wordt begonnen.
- Heeft een voorgeschiedenis van (niet-infectieuze) pneumonitis waarvoor steroïden nodig waren of bestaande pneumonitis.
- Heeft een bekende bijkomende maligniteit in de afgelopen 3 jaar, of die voortschrijdt of actieve behandeling vereist. Uitzonderingen zijn basaalcelcarcinoom van de huid of plaveiselcelcarcinoom van de huid dat een potentieel curatieve therapie heeft ondergaan of in situ baarmoederhalskanker.
- Heeft bekende actieve metastasen van het centrale zenuwstelsel (CZS) en/of carcinomateuze meningitis.
- Heeft een actieve auto-immuunziekte waarvoor in de afgelopen 2 jaar systemische behandeling nodig was (d.w.z. met gebruik van ziektemodificerende middelen, corticosteroïden of immunosuppressiva). Vervangingstherapie (bijv. thyroxine, insuline of fysiologische corticosteroïdvervangingstherapie voor bijnier- of hypofyse-insufficiëntie, enz.) wordt niet beschouwd als een vorm van systemische behandeling.
- Heeft een actieve infectie die systemische therapie vereist.
- Heeft een voorgeschiedenis of actueel bewijs van een aandoening, therapie of laboratoriumafwijking die de resultaten van het onderzoek zou kunnen verstoren, de deelname van de proefpersoon gedurende de volledige duur van het onderzoek zou kunnen belemmeren, of het is niet in het belang van de proefpersoon om deel te nemen, naar het oordeel van de behandelend onderzoeker.
- Patiënten met een medische voorgeschiedenis of aandoeningen die niet eerder op andere wijze zijn gespecificeerd en die naar de mening van de onderzoeker deelname aan dit onderzoek moeten uitsluiten. De onderzoeker dient de Studievoorzitter te raadplegen.
- Heeft psychiatrische stoornissen of stoornissen door middelenmisbruik gekend die de samenwerking met de vereisten van het proces zouden verstoren.
- Is zwanger of geeft borstvoeding, of verwacht kinderen te verwekken binnen de verwachte duur van de proef, te beginnen met de pre-screening of het screeningsbezoek tot en met 120 dagen na de laatste dosis van de proefbehandeling.
- Is eerder behandeld met een anti-PD-1-, anti-PD-L1- of anti-PD-L2-middel.
- Heeft een bekende voorgeschiedenis van het humaan immunodeficiëntievirus (hiv) (hiv 1/2 antilichamen).
- Heeft bekende actieve hepatitis B (bijv. HBsAg-reactief) of hepatitis C (bijv. HCV-RNA [kwalitatief] is gedetecteerd).
- Patiënten met borderline eierstoktumoren, recidiverende epitheliale eierstokkanker/primaire peritoneale kanker/eileiderkanker of niet-epitheliale eierstokkanker komen niet in aanmerking.
- Heeft binnen 30 dagen na de geplande start van de studietherapie een levend vaccin gekregen.

Progressievrije overleving (PFS) van combinatietherapie op basis van platina met anti-geprogrammeerde dood (PD)-1-therapie gevolgd door anti-PD-1-onderhoudstherapie bij patiënten met epitheliaal ovariumcarcinoom (EOC).

Tijd tot progressievrije overleving (PFS) is de periode vanaf het begin van de studie (eerste dosis therapie) tot ziekteprogressie, overlijden of datum van laatste contact. Alle patiënten ondergingen basislijn computertomografische scans voorafgaand aan de start van de therapie. De resterende ziekte-informatie werd verzameld uit chirurgische operatierapporten en postoperatieve CT-scans. De respons op de behandeling werd beoordeeld met CA 125 bij elke therapiecyclus. CT-scans werden postoperatief uitgevoerd voorafgaand aan de start van de systemische therapie, bij het voltooien van de combinatietherapie met platina, taxaan en pembrolizumab en bij het voltooien van de pembrolizumab-onderhoudstherapie. CT-scans werden ook uitgevoerd met toenemende CA 125 of indien klinisch geïndiceerd per behandelend arts en beoordelingen werden gemaakt met behulp van responsevaluatiecriteria in solide tumor (RECIST)-criteria gedefinieerd als een toename van 20% in de som van de langste diameter van doellaesies, of meetbare toename van non-target l |

Elke cyclus is 21 dagen. Alle deelnemers werd gevraagd om FACT-O-enquêtes in te vullen bij baseline/tijd van inschrijving, 3 maanden, 6 maanden en 18 maanden na aanvang van de therapie. FACT-O is een gevalideerde 26-item samenvattende score van 112 punten die de FACT-General (FACT-G) QOL-dimensies van fysiek welzijn (7 items), functioneel welzijn (7 items) en een subschaal voor eierstokkanker vastlegt (12 artikelen). FACT-0 is een enquête met 39 items in eigen beheer. Elk item wordt gescoord op een 5-punts Likert-schaal. Het FACT-0 scorebereik is van 0-44. De subschaalscorebereiken voor FACT-G zijn 0-108.
