Validatie van de Lupus Low Disease Activity State (LLDAS) in de regio Azië-Pacific (APLCLLDAS)
Lupus Low Disease Activity State (LLDAS)-onderzoek is een internationaal, multicenter prospectief onderzoek, ontwikkeld door de Asia Pacific Lupus Collaboration (APLC) om te onderzoeken of het bereiken van LLDAS geassocieerd is met verbeterde resultaten bij patiënten met systemische lupus erythematosus (SLE) .
SLE, of lupus, is de archetypische multisysteem auto-immuunziekte, met een geschatte incidentie van 5-50 gevallen per 100.000 mensen. Patiënten met SLE, meestal jonge vrouwen, lijden aan een duidelijk verlies aan levensverwachting en ernstige morbiditeit als gevolg van een heterogeen scala aan klinische manifestaties veroorzaakt door auto-immuun-gemedieerde ontsteking van meerdere organen. De meest ernstige manifestaties van SLE zijn de toename van onomkeerbare orgaanschade, met name betrokkenheid van de nieren en het centrale zenuwstelsel (CZS). Aangezien er geen effectieve gerichte monotherapie voor SLE bestaat, lijden patiënten ook aan ernstige toxiciteit door het gebruik van glucocorticoïden en breedspectrum immunosuppressieve therapieën. Ondanks combinatietherapie met de huidige medicijnen, tonen veel onderzoeken aan dat de meerderheid van de patiënten onvoldoende ziektecontrole heeft en na verloop van tijd onverbiddelijk permanente orgaanschade opbouwt.
De diversiteit aan klinische kenmerken van actieve SLE heeft kwantificering van ziekteactiviteit problematisch gemaakt. Hoewel er een aantal gepubliceerde systemen in gebruik zijn om de ziekteactiviteit van SLE te meten, zijn er algemeen erkende problemen met deze instrumenten. De gepubliceerde definities van remissie zijn zo streng dat minder dan 5% van de patiënten eraan voldoet. Dit leidde tot het besef dat in plaats van remissie van lupus, een doelwit voor lupus met een lage ziekteactiviteit mogelijk meer haalbaar is, en dat patiënten met een lage ziekteactiviteit homogener zijn dan patiënten met actieve ziekte. De ontwikkeling van een definitie van lupus lage ziekteactiviteit, die haalbaar is en validiteit heeft, ontsnapt dus aan de complexiteit van pogingen om heterogene toestanden van actieve ziekte te kwantificeren.
In deze studie zullen de onderzoekers prospectief longitudinale gegevens verzamelen over opeenvolgende SLE-patiënten in elk centrum om de LLDAS-definitie te evalueren. Bescherming tegen opbouw van orgaanschade als primair eindpunt.

In deze studie zullen patiënten met SLE ongeveer 5 jaar gevolgd worden. Regelmatige opnames van de gegevens die nodig zijn om LLDAS te scoren (ziekteactiviteit en behandelingsdomeinen, zie Franklyn L et al, Ann Rheum Dis 2016) zullen worden verzameld, evenals jaarlijkse registratie van lupusgerelateerde schade met behulp van de SLICC_ACR Damage Index (SDI) en kwaliteit van leven met behulp van de Short Form 36 versie 2 (SF36v2).
Aan het einde van de primaire gegevensverzameling zal de medewerker van LLDAS-bereiking, of aanhoudende verwezenlijking, met bescherming tegen opbouw van orgaanschade worden vastgesteld.

Beschrijving
Inclusiecriteria:
- Alle patiënten moeten voldoen aan ofwel de gewijzigde classificatiecriteria voor SLE van het American College of Rheumatology (ACR) uit 1997, met ten minste vier van de 11 items; of als alternatief voldoen aan de classificatiecriteria van Systemic Lupus International Collaborating Clinics (SLICC) 2012, met ten minste vier van de 17 items (ten minste één klinisch en één immunologisch criterium) of met lupus nefritis in aanwezigheid van ten minste één immunologisch criterium. Patiënten kunnen nieuw gediagnosticeerde of langdurige lupuspatiënten zijn.
Alle patiënten moeten ouder zijn dan 18 jaar en in staat zijn om schriftelijke toestemming te geven.
Uitsluitingscriteria:
- Patiënten jonger dan 18 jaar en patiënten die geen toestemming kunnen geven, worden van het onderzoek uitgesloten.
