Gedetailleerde beschrijving

Het register is bedoeld om real-world gegevens te verstrekken over patiëntbeheer en kwaliteit van zorg voor patiënten met T2DM, hypertensie, hitte-insufficiëntie en chronische nierziekte in de klinische praktijk in veel landen. Om deze kloof te dichten is een vrijwillig register opgezet om real-world gegevens vast te leggen over patiëntkenmerken, ziektebeheer, gebruik van gezondheidszorg en resultaten bij patiënten met diabetes type 2, hypertensie, hartfalen en chronische nierziekte. Multinationaal, observationeel register dat gebruikmaakt van een cloudgebaseerd eCRF, voor prospectieve en retrospectieve gegevensverzameling, toegankelijk voor onderzoekers en het Wetenschappelijk Comité. Dit register staat open voor alle artsen die T2DM, HTN, HF of CKD over de hele wereld behandelen.

Inclusiecriteria:

  1. 18 jaar of ouder zijn
  2. Diabetes type 2, hypertensie, hartfalen en/of chronische nierziekte hebben
  3. Geven van schriftelijke geïnformeerde toestemming om deel te nemen aan het register

Uitsluitingscriteria:

  1. Een levensbedreigende comorbiditeit hebben met een levensverwachting van minder dan 1 jaar
  2. Deelnemen aan een interventioneel onderzoek waarvoor geïnformeerde toestemming vereist is
