Karakterisering van patiënten met POAF in relatie tot hartchirurgie door wavECG en TTE
Karakterisering van patiënten met postoperatieve boezemfibrilleren in relatie tot hartchirurgie door middel van een signaalverwerkt oppervlakte-elektrocardiogram (wavECG) en transthoracale echocardiografie A LAACS-2-subonderzoek
Postoperatieve boezemfibrilleren (POAF) wordt vaak waargenomen na een openhartoperatie. Ondanks talloze pogingen om POAF te voorspellen, blijft het een uitdaging om de risicopatiënten correct te identificeren. Nieuwe technologieën zijn beschikbaar, maar werken nog niet samen in klinische richtlijnen en voorspellingsmodellen. We streven ernaar patiënten te identificeren die risico lopen op het optreden/recidief van AF na een openhartoperatie door gebruik te maken van signaalverwerkt oppervlakte-ECG (wavECG).
De studie is een verkennende, prospectieve cohortstudie van 100 patiënten die voor het eerst een electieve openhartoperatie ondergingen in de Left Atrial Appendage Closure by Surgery-2 (LAACS-2) trial (NCT03724318). Patiënten worden onderzocht door middel van elektrocardiogram (ECG), golfECG, transthoracale echocardiografie en continue hartritmebewaking. Het primaire eindpunt is het optreden/terugkeren van AF. Diastolische linkerventrikeldisfunctie zoals geëvalueerd door waveECG zal worden gebruikt om de incidentie van het primaire eindpunt te voorspellen.

Inleiding: Postoperatief boezemfibrilleren (POAF) wordt vaak waargenomen na openhartchirurgie,1-3 en boezemfibrilleren (AF) wordt vaak gecompliceerd door een beroerte.4-6 Tijdens AF is de elektrische activiteit ongecoördineerd en wordt de atriale contractie verminderd, waardoor het risico op trombivorming toeneemt. Vervolgens kunnen de trombi de algemene bloedsomloop binnendringen, de kleine intracerebrale slagaders bereiken en de bloedstroom blokkeren, wat leidt tot ischemische schade.5,7 Antistollingsmiddelen voorkomen op efficiënte wijze ischemische beroertes8, hoewel veel patiënten asymptomatisch zijn of vage symptomen hebben, waardoor de diagnose moeilijk te stellen kan zijn.5 Bovendien wordt AF dat secundair aan een operatie optreedt momenteel als zelfbeperkend beschouwd en is continue antistolling niet noodzakelijk. aanbevolen.4,9 Gecumuleerd bewijs suggereert echter dat secundair AF bij patiënten met een bekende hartaandoening het langetermijnrisico op beroerte en voorbijgaande ischemische aanval, terugkerend AF en mortaliteit verhoogt.10-15 Bij patiënten die een coronaire bypassoperatie (CABG) ondergaan, wordt nieuw AF in verband gebracht met een verhoogd langetermijnrisico op beroerte en mortaliteit in vergelijking met patiënten zonder AF.14,15 Hoewel er gevestigde risicofactoren zijn voor AF, zoals leeftijd, arteriële hypertensie, hartfalen, hartinfarct, hartklepaandoening en diabetes mellitus, is de pathofysiologie van AF complex en wordt deze niet volledig begrepen.6,16,17 Zowel structurele hermodellering van de boezems en elektrische banen, als oxidatieve stress, calciumoverbelasting, activering van myofibroblasten, microRNA's en ontsteking worden genoemd als mogelijke factoren die betrokken zijn bij het ontstaan en de progressie van AF.6,16-18 Pogingen om risico's te creëren en te verbeteren Stratificatiemodellen voor AF en beroerte zijn eerder gedaan, zowel in gemeenschapsgebaseerde cohorten als bij patiënten die een hartoperatie ondergaan, b.v. CHA2DS2-VASC, FHS AF-risicoscore, ARIC-risicovoorspellingstool, CHARGE-AF-risicomodel, HAVOC-score, SYNTAX-score, POAF-score en POAF-voorspellingsmodel.19-27 Het blijft echter een uitdaging om het optreden van AF nauwkeurig te voorspellen, zowel perioperatief als in de daaropvolgende jaren.28,29
Een recente systematische review en meta-analyse concludeerden dat p-golf terminale kracht in afleiding V1 (PTFV1), p-golfduur en maximaal p-golfoppervlak allemaal gecorreleerd zijn met beroerte,30 en een recent overzicht suggereert eveneens dat korte en langdurige p-wave en PTFV1 is gecorreleerd met AF.31 sp-ECG van Myovista (Southlake, Texas, VS) is een nieuw geavanceerd ECG dat gebruikmaakt van continue wavelet-transformatiesignaalverwerking en kan linkerventrikeldiastolische disfunctie (LVDD) detecteren die geassocieerd is met AF en postoperatief hartfalen in andere studies.17,32-35 LA-stam- en E/LA-stamindices voorspelden nieuwe cardiovasculaire gebeurtenissen, waaronder beroertes, in een beroerte-populatie36 Bij patiënten die katheterablatie ondergaan voor AF, kan LA-stam patiënten identificeren met een risico op AF-recidief37,38 POAF is bovendien voorspeld in een cohort van patiënten met ernstige aortastenose door LA-stamindexen.39
Doel: We streven ernaar patiënten met AF te karakteriseren door middel van nieuwe, gemakkelijke en toegankelijke methoden zoals elektrocardiogram (ECG), signaalverwerkt oppervlakte-ECG (Myovista, wavECG) en transthoracale echocardiografie (TTE).
Methoden: Verkennende, prospectieve cohortstudie van volwassen patiënten die voor het eerst een electieve openhartoperatie ondergaan bij de afdeling Cardiothoracale Chirurgie van het Universitair Ziekenhuis van Kopenhagen, Rigshospitalet. Honderd opeenvolgende patiënten die deelnamen aan de Left Atrial Appendage Closure by Surgery-2 (LAACS-2)-studie (NCT03724318) die aan de opname-/uitsluitingscriteria voldeden, zijn opgenomen in de huidige studie.
De studie is goedgekeurd door de Regionale Commissie voor Gezondheidsonderzoeksethiek en alle gevolgde procedures zijn in overeenstemming met de ethische normen van de verantwoordelijke commissie voor menselijke experimenten en met de Verklaring van Helsinki. Alle patiënten hebben geïnformeerde toestemming ondertekend voorafgaand aan de studie-inschrijving.
Studietijdlijn: één dag voorafgaand aan de operatie; patiëntkaarten worden bekeken en ECG, waveECG en TTE worden verkregen. Anesthesiologen zullen AF melden tijdens intra-operatieve hartbewaking. Postoperatieve hartritmebewaking wordt toegepast gedurende ten minste 48 uur. Bij ontslag worden patiëntendossiers en monitoring gecontroleerd op elk optreden van nieuw AF. Drie maanden na de operatie worden de patiëntendossiers beoordeeld en worden patiënten uitgenodigd voor een eenmalig bezoek aan de afdeling Cardiologie van het Frederiksbergziekenhuis, waar een controle-ECG, wavECG en TTE worden uitgevoerd. Afwijking van het onderzoeksprogramma wordt geaccepteerd, d.w.z. patiënten kunnen delen van de pre- of postoperatieve onderzoeken afwijzen en in de studie blijven voor follow-up.
Statistieken: Nauwkeurige berekeningen van de steekproefomvang zijn niet van toepassing in deze verkennende studie met LVDD in sp-ECG als primair eindpunt. Er is één eerdere studie uitgevoerd waarbij wavECG patiënten met LVDD identificeerde in een populatie (n=188) die was doorverwezen voor computertomografie coronaire angiografie.32 LA-stam gemeten door TTE voorspelde POAF bij 26 van de 60 patiënten die een openhartoperatie ondergingen voor ernstige aortastenose.39 P-golfindices van preoperatieve ECG's hebben POAF voorspeld in een populatie van 105 hartchirurgische patiënten.40 De studie omvat 100 opeenvolgende patiënten die een openhartoperatie ondergaan. Verwachte verdeling van patiënten is n=10 voor pre-operatieve AF (overeenkomend met de incidentie in de achtergrondpopulatie 1%6), POAF n= 35-50 (overeenkomend met de incidentie in eerdere hartchirurgische populaties; 35-50%1- 3), en niet-AF n=40-55 (resterende populatie). De follow-up duurt drie maanden.
Parametrische statistiek wordt gebruikt omdat normale verdeling wordt verwacht (bijv. twee-steekproef ongepaarde Student's t-test) en gegevens worden gepresenteerd met gemiddelde ± standaarddeviatie. Alle tests zijn tweezijdig en een P-waarde < 0,05 wordt als statistisch significant beschouwd. IBM SPSS versie 22 zal worden gebruikt voor statistische analyses.
