Musculoskeletale en bekkenbodemgezondheid bij chronische overlappende bekkenpijn bij vrouwen (de MSK-PELVIC-studie)
Het doel van deze studie is om meer te weten te komen over de zenuwfunctie en de bekkenspierfunctie.
Om dit te doen zullen we de bekkenzenuw- en spierfunctie van vrouwen met chronische bekkenpijn vergelijken met vrouwen die geen chronische bekkenpijn hebben.
Het begrijpen van de pijn kan in de toekomst leiden tot betere behandelingen.

Chronische bekkenpijn (CPP) kan tot een kwart van alle vrouwen treffen.
Van oudsher wordt aangenomen dat CPP wordt aangedreven door viscerale pijnmechanismen zoals interstitiële cystitis/pijnlijke blaassyndroom (IC/PBS), prikkelbaredarmsyndroom (PDS) en endometriose.
Er is ook vastgesteld dat deze viscerale pijnaandoeningen overlappen met vulvodynie, fibromyalgie, depressie en angst.
Meer recentelijk zijn onderliggende bekkenbodemmyofasciale (PFMP) en dyssynergie geïdentificeerd als aanvullende overlappende CPP-aandoeningen.
Eerder werk suggereert dat BBMP een viscero-somatische reactie kan zijn, maar BBP als compenserend gevolg van andere regionale musculoskeletale (MSK) aandoeningen is ook voorgesteld.
Sterker nog, gedurende hun hele leven lopen vrouwen grotere risico's dan mannen voor een overvloed aan MSK-verwondingen en chronische MSK-aandoeningen, waaronder een verhoogde prevalentie van sportgerelateerde verwondingen, hypermobiliteit van gewrichten, fibromyalgie, osteoartritische aandoeningen (postmenopauze) en osteoporose/osteoporose -geassocieerde fracturen.
Aangenomen wordt dat deze verhoogde risico's te wijten zijn aan de unieke anatomische (structurele), biomechanische en hormonale factoren die kunnen worden toegeschreven aan het fysiologische proces van zwangerschap en veroudering.
De huidige toepassing is bedoeld om de aanzienlijke kenniskloof aan te pakken met betrekking tot het beperkte begrip van de neuromusculaire functie van de bekkenbodemspieren bij CPP, evenals de rol van overlappende MSK-aandoeningen en MSK-gezondheid die de bekkenbodemrespons mogelijk beïnvloeden.
Het kortetermijndoel is om de biomechanica van bekkenbodemspieren te onderzoeken door de meest nauwkeurige spiermetingen bij vrouwen met CPP van verschillende overlappende diagnoses te identificeren in vergelijking met asymptomatische controles, samen met beoordeling van de algehele MSK-gezondheid/fysieke activiteit.
Het langetermijndoel is het bepalen van MSK-bekkenpijnmechanismen die klinisch relevante classificatie zullen opleveren, het ontwikkelen van evidence-based niet-farmacologische (fysiotherapie/oefening) behandelingen voor vrouwen met CPP, en geavanceerde onderzoeksinstrumenten op het gebied van bekkenbodemspieren en CPP. aangezien het betrekking heeft op de algehele MSK-gezondheid.
Onze innovatieve strategie combineert neuromusculaire maatregelen met behulp van nieuwe apparaten en gevalideerde maatregelen bij het evalueren van CPP, MSK-gezondheid en fysieke activiteit.
Onze centrale hypothese is dat vrouwen met CPP kwantificeerbare PFM-afwijkingen en klinische MSK-kenmerken zullen vertonen die verschillen van asymptomatische controles.
De resultaten van deze studie zullen een aanzienlijke impact hebben op de volksgezondheid met bijdragen van rigoureuze objectieve en uitgebreide PFM- en MSK-methoden, die geschikt zullen zijn voor toekomstige NIH klinische onderzoeksnetwerken/-onderzoeken, om de MSK-bijdrage en mogelijke behandelingsresultaten bij vrouwen te evalueren en te beoordelen. met CPP.

Beschrijving
Inclusiecriteria:
- Vrouwelijk geslacht
- Leeftijd 21 tot 80 jaar
- Symptomen van CPP zoals gedefinieerd door het American College of Obstetrics & Gynecology (ACOG) gedurende meer dan 6 maanden
- Ten minste twee van de zeven overlappende CPP-diagnoses (d.w.z. IC/PBS, IBS, endometriose, vulvodynie, PN, FM, PFMP)
- Een gemiddelde CPP-pijnscore van ten minste drie op een pijnschaal van 10 punten
Uitsluitingscriteria:
- Huidige of geschiedenis van GI of GU bekkenkanker
- Huidige bekkeninfectie (bijvoorbeeld een urineweginfectie of vaginale infectie)
- Huidige of op handen zijnde geplande zwangerschap of recente bevalling in de afgelopen 6 maanden
- Buik- of bekkenoperatie in de afgelopen 36 maanden.
