Badanie pilotażowe interwencji regulacji emocji i funkcji wykonawczych w przypadku samookaleczających myśli i zachowań (SITB) u dzieci
Badanie Proof-of-Concept dotyczące interwencji regulacji emocji i funkcji wykonawczych dla SITB u dzieci
Celem tego badania jest opracowanie i przetestowanie interwencji ambulatoryjnej dla dzieci w wieku przedpokwitaniowym (7-12 lat) z myślami i zachowaniami autoagresywnymi (SITBs).
Główne pytania, na które ma odpowiedzieć badanie, to:
- Czy nowa interwencja prowadzi do lepszego zaangażowania rodzin w leczenie w porównaniu ze standardowym leczeniem (TAU)?
- Czy nowa interwencja jest wykonalna, akceptowalna i odpowiednia?
- Czy nowa interwencja prowadzi do większej poprawy w zakresie SITBs, objawów zdrowia psychicznego i celów leczenia w porównaniu z TAU?
Uczestnicy w wieku przedpokwitaniowym z SITBs i ich rodziny zostaną losowo przydzieleni do nowej interwencji lub TAU, która będzie obejmować typowe interwencje stosowane przez terapeutę badania dla dzieci w wieku przedpokwitaniowym z SITBs. Uczestnicy będą:
- Wypełniać wstępną ocenę wyjściową w celu określenia kwalifikowalności oraz oceny SITBs, objawów zdrowia psychicznego, funkcji wykonawczych i regulacji emocji
- Uczestniczyć w około cotygodniowej interwencji ambulatoryjnej trwającej około 3-4 miesięcy
- Wypełniać dodatkowe oceny w połowie leczenia, po leczeniu oraz po 3 miesiącach od zakończenia
- Uczestniczyć w wywiadzie, dzieląc się swoimi odczuciami na temat interwencji

W fazie 1 badania (Cel 1) badacze będą iteracyjnie projektować interwencję dla przedadolescentów z SITB poprzez proces krok po kroku, wykorzystując ramy do systematycznego opracowywania i adaptacji interwencji (Wingood i in., 2008; Bartholomew i in., 1998). Proces projektowania będzie obejmować metody projektowania zorientowanego na użytkownika, w ramach których wielosektorowi eksperci i użytkownicy końcowi (tj. terapeuci i rodziny) będą uczestniczyć w procesie projektowania, aby zmaksymalizować potencjał rozpowszechniania i wdrażania interwencji. W fazie 2 badania (Cele 2 i 3) badacze przeprowadzą pilotażowe randomizowane badanie kontrolowane (RCT) interwencji, z porównaniem do standardowego leczenia (TAU), u 52 przedadolescentów w wieku 7-12 lat. Cel 2 będzie oceniał zaangażowanie (wynik pierwotny), wykonalność, akceptowalność i wyniki wdrożenia dla interwencji i procedur badania. Celem 3 będzie zebranie wstępnych dowodów w celu zbadania wyższości interwencji SITB nad TAU w redukcji SITB oraz angażowaniu docelowych mechanizmów EF i regulacji emocji.
W fazie 1 badacze wykorzystają podejścia metod mieszanych i zasady projektowania zorientowanego na użytkownika do iteracyjnego opracowania interwencji SITB dla przedadolescentów. Poprzez ocenę potrzeb badacze zastosują eksploracyjny sekwencyjny projekt metod mieszanych jakościowych-->ilościowych, w ramach którego jakościowe wywiady z terapeutami będą informować o opracowaniu ilościowej ankiety oceniającej potrzeby i doświadczenia terapeutów leczących przedadolescentów z SITB. Wyniki oceny terapeutów zostaną zintegrowane z wynikami jakościowych wywiadów z przedadolescentami i opiekunami, aby dalej informować o wstępnych decyzjach dotyczących podstawowych funkcji i konkretnych komponentów interwencji. Podczas fazy przedtestowej wykorzystującej równoległe podejście metod mieszanych terapeuci i przedadolescenci/rodziny będą uczestniczyć w sesjach projektowania na głos, aby dostarczyć ilościowe i jakościowe informacje zwrotne na temat wstępnych prototypów interwencji. Następnie zostanie opracowany wstępny projekt interwencji i przejrzany przez grupę ekspertów, co zaowocuje drugim projektem interwencji oraz procedurami szkolenia i konsultacji dla terapeutów. Jednoarmowe, pilotażowe badanie wykonalności interwencji z udziałem 9 przedadolescentów (ADAPT-ITT Krok 8, Część 1) będzie informować o adaptacjach interwencji, szkoleniu terapeutów i procedurach badania.
Faza 2 będzie obejmować szkolenie terapeutów w zakresie interwencji, wykorzystując procedury i materiały udoskonalone podczas badania pilotażowego, a następnie RCT z warunkiem kontrolnym TAU z udziałem 52 przedadolescentów (ADAPT-ITT Krok 8, Część 2). Badanie będzie miało wystarczającą moc, aby wykryć różnice w zaangażowaniu między warunkami (wynik pierwotny) i ustalić wykonalność oraz akceptowalność, jednocześnie oceniając wstępne wyniki skuteczności i mechanizmy zmian EF.

Measured using the Columbia Suicide and Self Injury Severity Rating Scale (C-SSRS; Posner et al., 2011), a brief interview that consists of sections that assess: suicidal ideation, ideation intensity, and suicidal behavior.
The C-SSRS has strong convergent validity, sensitivity to change, predictive and incremental validity, and good internal consistency.

Non-Suicidal Self-Injury

Measured using an adapted version of the Self-Injurious Thoughts and Behaviors Interview (SITBI)-Short Form, Thoughts of NSSI and NSSI subscales (Nock et al., 2007), which assesses history and characteristics of NSSI thoughts and behaviors.
First instance of engagement in NSSI after beginning treatment will be the outcome of most interest, but other outcomes assessed by the interview (e.g., frequency of NSSI thoughts) may be examined as well if rates of NSSI are very low.

We will measure anxiety using the Revised Child Anxiety and Depression Scale (RCADS)-Short Version (Ebesutani et al., 2012; 2017), which is a 25-item measure of depression and anxiety symptoms that includes both parent-and self-report measures.
Both versions are rated on a 5-point Likert scale, with higher scores indicating greater frequency of anxiety symptoms.
Anxiety items are summed to yield a raw score, which is then converted to a t-score.
The measure has good psychometric properties and treatment sensitivity (Ebesutani et al., 2012; 2017).

The Brief Rating Inventory of Executive Function 2nd Edition (BRIEF 2) --Parent Form (Gioia et al., 2015) is 63-item questionnaire with eight clinical scales designed to assess everyday behaviors associated with seven domains of EF.
These scales are combined to form two broader indices (Behavioral Regulation and Metacognition) and a Global Executive Composite score.
Items are rated on a 4-point scale, and sum scores for each scale and index are converted to t-scores, with higher t-scores indicating greater difficulties with executive functioning.
The measure has strong psychometric properties, discriminates between typically developing and clinical populations, and is sensitive to change.

Behavioral Assessment of Executive Functioning and Distress Tolerance

Executive Function Challenge Task (EFCT; Kenworthy et al., 2014, 2020) is a "hot" measure of EF that uses a standardized protocol to assess flexibility and planning during several activities with an examiner that change rules or modalities.
The EFCT has precise behavioral markers to guide scoring on a three-point scale and yields flexibility and planning raw scores that range from 0 to 8. The EFCT has good psychometric properties and demonstrated moderate associations with parent-reported EF and treatment-specific sensitivity to change (Kenworth et al., 2014; 2020).

Task-Based Assessment of Executive Functioning

NIH toolbox Dimensional Change Card Sort test (DCCS; Kavanaugh et al., 2020; Zelazo et al., 2013). is a standard, lab-based, computerized task for assessing cognitive flexibility, will be administered as a computerized measure of EF.
The DCCS has been validated in youth, does not demonstrate practice effects, and has excellent convergent validity.
