Farmacocinética do Superboosting de Lopinavir/Ritonavir em Lactentes e Crianças Pequenas Co-infectadas com HIV e TB
Um estudo farmacocinético comparando a exposição plasmática ao lopinavir quando administrado como lopinavir/ritonavir (1:1) na presença de rifampicina e lopinavir/ritonavir (4:1) sem rifampicina em crianças infectadas por HIV e TB na África do Sul.

Este é um estudo multicêntrico, aberto, não randomizado, prospectivo, de não inferioridade para comparar a farmacocinética do lopinavir administrado com superpotencialização (LPV/r 1:1) e tratamento concomitante com RIF ou com potencialização padrão (LPV/r 4:1) sem tratamento concomitante com RIF e para avaliar a segurança, tolerância e efeito virológico do superboosting em lactentes e crianças co-infectadas por HIV-TB com peso >3 kg e ≤15 kg.
O LPV/r será administrado como a formulação líquida de 80/20 mg/mL (taxa de reforço padrão de 4:1). Durante o tratamento anti-TB, uma formulação líquida adicional de RTV será fornecida para fornecer uma proporção superboosting de 1:1 de LPV para RTV. As doses reais de medicamentos anti-retrovirais e anti-TB serão baseadas nas recomendações de dosagem da faixa de peso da África do Sul (SA) e fornecidas de acordo com o padrão de tratamento do local.

Critério de inclusão:
- Documentação de um diagnóstico confirmado de infecção por HIV-1 seguindo as diretrizes clínicas da SA
- Peso >3kg ≤15kg no momento da inscrição
- > 42 semanas de idade gestacional
- Em terapia baseada em LPV/r ou prestes a iniciar uma terapia antirretroviral combinada baseada em LPV/r com 2 NRTIs [ABC+3TC ou AZT+3TC ou d4T+3TC]
- Diagnóstico clínico de TB que requer terapia baseada em RIF
- Pai ou responsável legal capaz e disposto a fornecer consentimento informado por escrito e capaz de comparecer às visitas do estudo.
Critério de exclusão:
- Para recém-nascidos, menos de 42 semanas de gestação e 14 dias de idade
- Tratamento concomitante/crônico com drogas potentes indutoras/inibidoras de enzimas diferentes daquelas dos tratamentos do estudo. Consulte o Apêndice E (indutores/inibidores menores e medicamentos usados como parte do tratamento da condição são permitidos, por exemplo, Esteróides)
- Antecipação no início de que a duração do tratamento anti-TB será superior a 9 meses
- Qualquer outra condição/descoberta que, na opinião do investigador, comprometeria a participação da criança neste estudo, por exemplo. alanina transferase (ALT) mais de 10 vezes o limite superior do normal (ULN), ou doença crônica renal, hepática ou gastrointestinal, como má absorção.
- Crianças com doenças malignas conhecidas e contra-indicações para tomar LPV/r
- Tratamento com drogas experimentais para qualquer indicação dentro de 30 dias antes da entrada no estudo; a participação em outro estudo pode ser aprovada pela equipe do estudo.
