Farmacocinetica di Lopinavir/Ritonavir Superboosting in lattanti e bambini co-infetti da HIV e tubercolosi
Uno studio di farmacocinetica che confronta l'esposizione plasmatica del lopinavir quando somministrato come lopinavir/ritonavir (1:1) in presenza di rifampicina e lopinavir/ritonavir (4:1) senza rifampicina nei bambini co-infetti da HIV e tubercolosi in Sud Africa.

Questo è uno studio multicentrico, in aperto, non randomizzato, prospettico, di non inferiorità per confrontare la farmacocinetica del lopinavir somministrato con superboosting (LPV/r 1:1) e trattamento concomitante con RIF o con boosting standard (LPV/r 4:1) senza trattamento concomitante con RIF e per valutare la sicurezza, la tolleranza e l'effetto virologico del superboosting nei neonati e nei bambini con co-infezione da HIV-TBC di peso >3 kg e ≤15 kg.
LPV/r verrà somministrato come formulazione liquida 80/20 mg/mL (rapporto di potenziamento standard 4:1). Durante il trattamento anti-TBC, verrà fornita una formulazione liquida RTV aggiuntiva per fornire un rapporto di superboosting 1:1 tra LPV e RTV. Le dosi effettive per i farmaci antiretrovirali e anti-TBC saranno basate sulle raccomandazioni di dosaggio della fascia di peso sudafricana (SA) e fornite secondo lo standard di cura del sito.

Descrizione
Criterio di inclusione:
- Documentazione di una diagnosi confermata di infezione da HIV-1 seguendo le linee guida cliniche SA
- Peso >3kg ≤15 kg all'iscrizione
- > 42 settimane di età gestazionale
- In terapia a base di LPV/r o in procinto di iniziare una terapia di associazione antiretrovirale a base di LPV/r con 2 NRTI [ABC+3TC o AZT+3TC o d4T+3TC]
- Diagnosi clinica di tubercolosi che richiede una terapia basata su RIF
- Genitore o tutore legale in grado e disposto a fornire il consenso informato scritto e in grado di partecipare alle visite di studio.
Criteri di esclusione:
- Per i neonati, meno di 42 settimane di gestazione e 14 giorni di età
- Trattamento concomitante/cronico con potenti farmaci induttori/inibitori enzimatici diversi da quelli inclusi nei trattamenti in studio. Vedere l'Appendice E (induttori/inibitori minori e farmaci utilizzati come parte della gestione della condizione sono consentiti, ad es. steroidi)
- Anticipazione all'inizio che la durata del trattamento anti-TBC sarà superiore a 9 mesi
- Qualsiasi altra condizione/reperto che, a giudizio dello sperimentatore, comprometterebbe la partecipazione del bambino a questo studio, ad es. alanina transferasi (ALT) superiore a 10 volte il limite superiore della norma (ULN) o malattie renali, epatiche o gastrointestinali croniche come il malassorbimento.
- Bambini con tumori maligni noti e controindicazioni all'assunzione di LPV/r
- Trattamento con farmaci sperimentali per qualsiasi indicazione entro 30 giorni prima dell'ingresso nello studio; la partecipazione a un altro studio può essere approvata dal gruppo di studio.
