Descrição
Critério de inclusão:
Os pacientes devem atender a TODOS os seguintes critérios para serem incluídos no estudo:
- Pacientes de ambos os sexos.
- Idade entre 40 e 90 anos.
- Arteriosclerose vascular de grau grave (categoria RB 4 e 5 mono ou bilateral).
- Impossibilidade de revascularização cirúrgica ou endovascular ou falha na cirurgia de revascularização realizada, no mínimo 30 dias antes da inclusão no estudo, definindo falha como a não chegada direta de vasos ao arco plantar.
Parâmetros bioquímicos normais definidos por:
- Leucócitos> 3000 / mm3
- Neutrófilos> 1500 / mm3
- Plaquetas > 100.000 / mm3
- AST/ALT <2,5x limite superior do normal
- Em pacientes com úlcera isquêmica, ela deve permanecer estável por pelo menos 1 semana.
- Pacientes em tratamento médico convencional para CLI.
- As mulheres em idade reprodutiva devem obter um resultado negativo em um teste de gravidez de urina realizado no momento da inclusão no estudo e se comprometer a usar um método contraceptivo eficaz durante sua participação no estudo.
- Pacientes que não participaram de nenhum outro ensaio clínico durante os 3 meses anteriores à visita de inclusão.
- Pacientes que assinam o consentimento informado.
Critério de exclusão:
Pacientes que apresentarem ALGUNS dos seguintes critérios não podem ser selecionados para participar deste estudo:
- CLI com perda tecidual no membro alvo (categoria 6 de RB).
- Amputação maior anterior no membro alvo.
- Hipertensão não controlada (definida como PAS> 180 ou PAD> 110 em pelo menos 2 determinações durante o período de seleção).
- Pacientes com insuficiência cardíaca grave ou fração de ejeção inferior a 30%.
- Pacientes com diagnóstico prévio de arritmias ventriculares ou angina instável.
- Pacientes com septicemia.
- Pacientes diagnosticados com trombose venosa profunda nos 3 meses anteriores à sua inclusão no estudo.
- Terapia concomitante que inclui oxigênio hiperbárico, agentes angiogênicos ou inibidores da Cox II.
- Contraindicação à RMN de perfusão.
- Retinopatia proliferativa sem tratamento.
- Nefropatia diabética em hemodiálise.
- Pacientes previamente tratados com terapia celular, terapia gênica ou fatores de crescimento no último ano.
- Doença concomitante que limite a esperança de vida a 1 ano ou que não assegure o período de seguimento.
- Pacientes que sofreram acidente vascular cerebral ou infarto do miocárdio nos 3 meses anteriores à consulta de inclusão.
- Anemia grave (hemoglobina <7,9g/dl) na análise de inclusão.
- Pacientes com diagnóstico prévio de alcoolismo crônico.
- Qualquer anomalia clinicamente significativa detectada no Período de Seleção e que, na opinião do investigador, constitua impedimento à correta participação do paciente no estudo ou ao cumprimento dos procedimentos nele estabelecidos.
