背景
自 2013 年最新版《精神疾病诊断和统计手册》(DSM-5) 以来,暴食症 (BED) 才成为一种独立的诊断。 它是全球最普遍的饮食失调症,影响全球超过 5000 万受试者。 BED 的特点是暴饮暴食:在不连续的时间内吃大量食物,并伴有失控感。 已确定发病的两个主要高峰:一是平均年龄 14 岁,二是 18-20 岁。 BED 是青少年中主要的慢性疾病之一,通常与精神合并症(占一生患病率的 60% 以上)有关,而且还与肥胖及其身体后果有关,对生活功能有很大影响,死亡率很高。 青少年群体中暴食症的患病率在 1-3% 之间,而肥胖青少年群体中的患病率高达 37%。 这些数字的上升与该人群肥胖患病率的大幅上升同时发生。 当考虑到一般人群中的亚临床表现(涉及出现一些症状但不满足 BED 的所有诊断标准)时,患病率为 3%。 亚临床形式用诸如情绪化饮食、无组织饮食、饮食失控等术语来描述。 后者是唯一具有可操作标准的结构。 在某些情况下,但不是全部,患有这些疾病的个体可能被诊断为患有指定或未指定的 DSM-5 饮食失调类别(“其他指定的进食或饮食失调”-OSFED-,或“未另行指定的饮食失调”-EDNOS-) 。

描述
纳入标准:
- 以体重指数 (BMI) z 得分高于 2 个标准差来衡量的肥胖作为主要咨询标准的患者来到内分泌科。
- 年龄在12-16岁之间。
- 受试者父母或法定监护人签署的知情同意书,以及 12 岁或以上青少年签署的同意书。
BED 和 PREC-BED 组的附加纳入标准:
- BED 组中是否存在 BED 的 DSM-5 标准。
- 满足 PREC-BED 组中的 LOC(失控)标准(与原始 Marcus&Kalarchian 相关)。
排除标准:
- 用 K-BIT 测量的智商 < 70。
- 任何共病精神疾病,BED 组中的 BED 或 PREC-BED 组中的 PREC-BED 除外。 所有群体都可以接受吸烟以及存在适应性障碍或任何轻度焦虑症。
- 外伤性脑损伤或任何神经系统疾病。
