Beschreibung
Wichtige Einschlusskriterien:
Krankheitsdiagnose und Vorbehandlung:
Kohorte 1 (die Erstlinien-Brustkrebsbehandlungskohorte):
- Teilnehmerinnen mit histologisch oder zytologisch bestätigtem inoperablem, lokal fortgeschrittenem, rezidivierendem oder metastasiertem Adenokarzinom der Brust und Kandidaten für eine Chemotherapie. Lokal rezidivierende Erkrankungen dürfen einer Resektion mit kurativer Absicht nicht zugänglich sein.
- Humaner epidermaler Wachstumsfaktorrezeptor 2 (HER2) IHC 3+ oder In-situ-Hybridisierung positiv auf dem archivierten Tumorgewebe oder einer frischen Biopsieprobe.
- Keine vorherige systemische Krebstherapie für lokal fortgeschrittene nicht resezierbare oder metastasierte Erkrankungen erhalten haben.
Kohorte 2 (die Erstlinien-Behandlungskohorte für das Adenokarzinom des Magens/gastroösophagealen Überganges):
- Histologisch oder zytologisch bestätigtes inoperables, lokal fortgeschrittenes, rezidivierendes oder metastasiertes Adenokarzinom des Magens oder des gastroösophagealen Übergangs
- HER2 IHC 3+ oder HER2 IHC 2+ zusammen mit positiver In-situ-Hybridisierung auf dem archivierten Tumorgewebe oder einer frischen Biopsieprobe.
- Keine vorherige systemische Krebstherapie für lokal fortgeschrittene inoperable oder metastasierte Erkrankungen erhalten haben, einschließlich zugelassener oder in der Erprobung befindlicher geschätzter glomerulärer Filtrationsraten (EGFR) oder Anti-HER2-Wirkstoffe oder -Impfstoffe, zytotoxische Chemotherapie oder Checkpoint-Inhibitoren
- Mindestens 1 messbare Läsion gemäß RECIST Version 1.1
- Leistungsstatus der Eastern Cooperative Oncology Group (ECOG) ≤ 1
- Ausreichende Organfunktion, wie durch folgende Laborwerte während des Screenings angezeigt:
- Linksventrikuläre Ejektionsfraktion (LVEF) ≥ 50 % zu Studienbeginn, bestimmt entweder durch Echokardiogramm oder Multigated Acquisition Scan (MUGA) (Echokardiogramm ist die bevorzugte Methode) innerhalb von 28 Tagen vor der ersten Dosis des Studienmedikaments
Wichtige Ausschlusskriterien:
- Vorherige Therapie mit einem Anti-PD-1, Anti-PD-L1, Anti-PD-L2 oder einem anderen Antikörper oder Medikament, das spezifisch auf T-Zell-Kostimulation oder Checkpoint-Signalwege abzielt
Geschichte von zugelassenen oder Prüf-Tyrosinkinase / HER-Inhibitoren in jedem Behandlungsumfeld
A. außer Trastuzumab mit oder ohne Pertuzumab, das in neoadjuvanter oder adjuvanter Behandlung für Kohorte 1 verwendet wird
- Aktive leptomeningeale Erkrankung, unbehandelte oder unkontrollierte Hirnmetastasen
- Jede aktive Malignität ≤ 2 Jahre vor der ersten Dosis des Studienmedikaments, mit Ausnahme des in dieser Studie untersuchten spezifischen Krebses und jedes lokalisierten Krebses, der kurativ behandelt wurde (z situ des Gebärmutterhalses oder der Brust)
- Jeder Zustand, der eine systemische Behandlung mit entweder Kortikosteroiden (> 10 mg täglich Prednison oder Äquivalent) oder anderen immunsuppressiven Medikamenten ≤ 14 Tage vor der ersten Dosis des Studienmedikaments erforderte
Hinweis: Teilnehmer, die derzeit oder zuvor eines der folgenden Steroidschemata erhalten haben, sind nicht ausgeschlossen:
- Nebennierenersatzsteroid (Dosis ≤ 10 mg täglich Prednison oder Äquivalent)
- Topisches, okulares, intraartikuläres, intranasales oder inhaliertes Kortikosteroid mit minimaler systemischer Resorption
- Kurze Behandlung (≤ 7 Tage) mit prophylaktisch verschriebenem Kortikosteroid (z. B. bei Kontrastmittelallergie) oder zur Behandlung einer Nicht-Autoimmunerkrankung (z. B. durch Kontaktallergen verursachte Überempfindlichkeitsreaktion vom verzögerten Typ)
HINWEIS: Es können andere im Protokoll definierte Einschluss-/Ausschlusskriterien gelten.

Experimental: Cohort 1A: Breast Cancer: Zanidatamab 30 mg/kg + Docetaxel
Participants with HER2+ breast cancer received zanidatamab 30 mg/kg intravenously (IV) and docetaxel 75 mg/m^2 IV once every 3 weeks (Q3W) for an initial period of up to six cycles.
After cycle 6, continuation of docetaxel treatment was at the discretion of the investigator.
Zanidatamab was administered until disease progression, intolerable toxicity, or another criterion for treatment discontinuation was met.

Participants also received CAPOX chemotherapy (1000 mg/m^2 oral capecitabine twice daily for 14 days in each cycle and 130 mg/m^2 oxaliplatin IV once every 3 weeks) for up to 6 six cycles.
After cycle 6, oxaliplatin was discontinued and continuation of capecitabine as maintenance treatment was at the investigator's discretion.

Participants also received CAPOX chemotherapy (1000 mg/m^2 oral capecitabine twice daily for 14 days in each cycle and 130 mg/m^2 oxaliplatin IV once every 3 weeks) for up to 6 six cycles.

A serious AE (SAE) was any untoward medical occurrence that, at any dose - resulted in death, - was life threatening, - required hospitalization or prolongation of existing hospitalization, - resulted in disability/incapacity, - was a congenital anomaly/birth defect. |

Objective Response Rate (ORR)

Complete Response (CR): Disappearance of all target lesions. Any pathological lymph nodes (whether target or non-target) must have reduction in short axis to < 10 mm. Partial Response (PR): At least a 30% decrease in the sum of diameters of target lesions, taking as reference the baseline sum diameters. |
