Beschreibung
Einschlusskriterien Kliniken:
- VA-Abteilungen und gemeinschaftsbasierte Ambulanzen (CBOCs) oder „Kliniken“ aus regionalen VA-Gesundheitssystemen
- Muss unter dem Gesamt-VA-Qualitätsmedian liegen (wie von Strategic Analytics for Improvement and Learning oder SAIL bewertet), der 3 von 8 SAIL-Maßnahmen umfasst, die mit vier evidenzbasierten Psychotherapien und drei evidenzbasierten Pharmakotherapien für Depression, PTSD und Opioid verbunden sind Störung verwenden.
Ausschlusskriterien Kliniken:
- Kliniken mit weniger als 12 Monaten Daten im Jahr 2018
- Kliniken, die bereits zu Studienbeginn am Qualitätsverbesserungsprogramm des Office of Veterans Access to Care (OVACS) beteiligt waren.
- Kliniken, in denen die Implementierung der elektronischen Patientenakte (EHR) von VA Cerner während der Projektlaufzeit stattfinden wird (Veterans Integrated Services Networks (VISNs) 20, 21, 22 und 7)
- Kliniken, die im Durchschnitt weniger als 122 einzelne Patienten pro Monat behandeln
- Kliniken ohne ein multidisziplinäres Team von Dienstleistern für psychische Gesundheit oder Sucht vor Ort (mindestens erforderlich: 1 Psychiater, 1 Psychologe, 1 Sozialarbeiter vor Ort)

Die partizipative Systemdynamik ist eine erleichterte Strategie zur Verbesserung der Gesundheitsversorgung oder einer evidenzbasierten Praxisimplementierung, die Sucht- und Psychiatriemitarbeiter an vorderster Front umfasst, die Simulationen von klinischen Verbesserungsstrategien durchführen, um die besten Ansätze zur Verbesserung der Reichweite von evidenzbasierter Psychotherapie und evidenzbasierter Pharmakotherapie zu finden .

Audit und Feedback ist eine Strategie zur Verbesserung der Qualität der Gesundheitsversorgung oder einer evidenzbasierten Praxisimplementierungsstrategie, bei der Mitarbeiter aus den Bereichen Sucht und psychische Gesundheit an vorderster Front Daten von klinischen Versorgungsteams überprüfen, um die besten Ansätze zur Verbesserung der Reichweite evidenzbasierter Psychotherapie und evidenzbasierter Pharmakotherapie zu finden.

Die Aufnahme einer evidenzbasierten Praxis ist durch eine evidenzbasierte Psychotherapievorlage oder eine evidenzbasierte Pharmakotherapieverordnung nach Einnahme angezeigt.
Ein angemessener Kurs basiert auf dem Erhalt einer angemessenen Anzahl von evidenzbasierten Psychotherapiesitzungen, um eine „Vervollständigung“ (typischerweise 8 Sitzungen) oder einer ausreichenden Anzahl von Nachfüllungen für eine von den Leitlinien empfohlene angemessene Prüfung jedes Medikaments (variiert je nach Medikament) zu erhalten.

Wir werden 5 evidenzbasierte Psychotherapien untersuchen: 3 für Depressionen (Cognitive Behavior Therapy (CBT-D), Acceptance and Commitment Therapy (ACT) und Interpersonal Psychotherapy (IPT)) und 2 für PTBS (Prolonged Exposure (PE) and Cognitive Processing). Therapie (CPT)).

Wir werden 8 evidenzbasierte Pharmakotherapien untersuchen: 2 für Depressionen (84 und 180 Tage therapeutische Kontinuität bei Beginn eines neuen Antidepressivums), 2 für Opioidkonsumstörungen (OUD) (Methadon und Buprenorphin) und 4 für Alkoholkonsumstörungen (AUD) (Acamprosat , Disulfiram, Naltrexon und Topiramat).
