Analyse des échantillons de sténose de la colonne lombaire pour l'identification de l'amylose cardiaque à transthyrétine
Objectif principal:
Identifier les personnes âgées atteintes d'ATTR-CA au début de l'évolution de la maladie, à un moment où les traitements modificateurs de la maladie sont les plus efficaces.
Les objectifs spécifiques de cette enquête épidémiologique comprennent :
- Identifier les sujets ayant déjà subi une chirurgie LSS qui présentent des signes de dépôts amyloïdes TTR dans des échantillons de la colonne vertébrale et pourraient être à risque d'amylose cardiaque ATTR.
- Évaluer l'ATTR-CA parmi ceux avec un TTR localisé dans le tissu rachidien.
L’étude explorera également les éléments suivants :
- La prévalence de l'amyloïde dans les échantillons de sténose rachidienne lombaire par coloration au rouge Congo.
- La prévalence des dépôts de TTR chez les sujets atteints d'amyloïde, déterminée par spectrométrie de masse.
- Évaluation d'une nouvelle technique d'intelligence artificielle permettant d'identifier histologiquement l'amyloïde avec une coloration H&E standard.
- Différence de prévalence d'ATTR-CA entre les sujets atteints de TTR et de dépôts amyloïdes indéterminés dans la colonne vertébrale du sujet par captation myocardique de Tc99-PYP.

Description détaillée
Il s'agit d'une étude de cohorte prospective multicentrique visant à faciliter l'identification des individus atteints d'ATTR-CA. Les enquêteurs identifieront les sujets ayant déjà subi une chirurgie LSS qui présentent des preuves de dépôts amyloïdes TTR dans les échantillons de la colonne vertébrale. Les sujets présentant un TTR localisé dans le tissu rachidien seront invités à une visite sur place et seront évalués pour la présence de manifestations cliniques de l'amylose cardiaque ATTR (ATTR-CA).
Les sujets subiront une scintigraphie nucléaire, un test génétique TTR, une évaluation clinique, un électrocardiogramme et un échocardiogramme. Des questionnaires sur la qualité de vie seront également administrés. Sujets présentant une rétention myocardique du radiotraceur d'imagerie avec un score de Perugini égal à 2 (absorption égale à l'os) ou 3 (absorption supérieure à l'os) et confirmation par SPECT ou SPECT/CT que l'absorption est de nature myocardique et n'ont aucune preuve de les protéines monoclonales seront diagnostiquées avec ATTR-CA.
Cette étude doit être menée sur cinq sites d'étude : Boston Medical Center, Cedars Sinai Medical Center, Cleveland Clinic, Columbia University Irving Medical Center et Tufts University Medical Center. La clinique Mayo servira de noyau de recherche en pathologie pour analyser des spécimens de colonne vertébrale.
La plupart des sujets doivent remplir toutes les procédures d'étude et tous les questionnaires en une seule journée.
Jusqu'à 1663 patients seront autorisés dans cette étude à faire examiner des échantillons de colonne vertébrale pour la présence d'amyloïde par Congo Red au laboratoire principal de la Mayo Clinic. Parmi le pool de sujets avec de l'amyloïde détecté (qui devrait être de 33 % ou au moins 544), les enquêteurs s'attendent à recruter 163 participants pour subir un phénotypage cardiaque (Tc99m et évaluation cardiaque). Parmi les 163 participants qui subissent un phénotypage cardiaque, 98 auront le TTR comme protéine précurseur identifiée par spectrométrie de masse et 65 auront une protéine précurseur indéterminée par spectrométrie de masse.

Critère d'intégration:
- Chirurgie de décompression de la colonne lombaire cliniquement indiquée qui impliquait l'ablation du ligament jaune 5 à 15 ans avant l'inscription.
- Âge > 60 ans au moment de l'intervention chirurgicale.
- Capable de comprendre et de signer le document de consentement éclairé une fois que la nature de l'étude a été entièrement expliquée.
Critère d'exclusion:
- Amylose primaire (AL) ou secondaire (AA) confirmée.
- Amylose TTR connue.
- Chirurgie lombaire pour des indications autres que la sténose vertébrale.
- Malignité active.
- Déficience due à un accident vasculaire cérébral, une blessure ou un autre trouble médical qui exclut la participation à l'étude.
- Démence invalidante ou autre maladie mentale ou comportementale.
- Inscription à un essai clinique non approuvé pour la co-inscription.
- Poids > 350 livres.
- Incapacité ou refus de se conformer aux exigences de l’étude.
- Résident d'une maison de retraite.
- Autre raison qui rendrait le sujet inapproprié pour entrer dans cette étude.

Les participants qui ont déjà subi une chirurgie de la colonne lombaire et/ou du canal carpien fourniront des données de dossier médical, un échantillon de la colonne vertébrale, des données prospectives d'un bilan cardiaque avec de l'amyloïde ATTR confirmé dans un échantillon de la colonne vertébrale, puis subiront un bilan cardiaque pour le phénotypage/génotypage cardiaque.

Identifier qui peut être à risque d’amylose cardiaque ATTR.
L'enquêteur va collecter des échantillons de ligamentum flavum de la colonne vertébrale et les colorer avec du rouge congo et ceux qui sont positifs pour l'amyloïde ATTR dans leur échantillon de colonne vertébrale seront comptés.

L'investigateur va effectuer un bilan de soins standard (SOC) sur l'amylose cardiaque, y compris une analyse PYP pour confirmer le phénotype cardiaque.
