Coronaire hartziekte blijft de belangrijkste oorzaak van morbiditeit en mortaliteit bij vrouwen, goed voor meer dan 250.000 sterfgevallen per jaar. Ondanks de hoge prevalentie van ischemische hartziekte bij vrouwen, hebben de meeste klinische onderzoeken zich gericht op mannelijke cohorten. De optimale niet-invasieve test voor de evaluatie van ischemische hartziekte bij vrouwen is onbekend. Er zijn een aantal verschillende modaliteiten gebruikt, waaronder inspannings-ECG-stresstests, 2-dimensionale stress-echocardiografie, SPECT-myocardperfusiebeeldvorming en computertomografie met elektronenbundels.
Het cohort van vrouwen voor wie tests moeten worden uitgevoerd, is ook slecht gedefinieerd. Myocardiale perfusiebeeldvorming, in combinatie met farmacologische stresstests, is ook effectief gebleken bij de diagnose van vrouwen met bekende of vermoede coronaire hartziekte en bij risicostratificatie. Onlangs is farmacologische stress gecombineerd met oefeningen op laag niveau, waardoor de testtolerantie en de SPECT-perfusiebeeldkwaliteit worden verbeterd. Bovendien kan het gebruik van een gecombineerd adenosine- en inspanningsprotocol grotere hoeveelheden ischemie detecteren met perfusiebeeldvorming dan met een inspanningstest alleen. Daarom kan dit gecombineerde farmacologische en inspanningsbeeldvormingsprotocol bij vrouwen die mogelijk niet in staat zijn om maximale inspanning uit te voeren, een aanzienlijk voordeel hebben ten opzichte van adenosine-stresstests alleen.
Het doel van deze studie is om de veiligheid en symptomen van deze twee methoden van stresstesten te vergelijken. De huidige studie probeert ook de optimale methode vast te stellen voor de detectie van coronaire hartziekte bij vrouwen met een beperkt vermogen tot lichaamsbeweging (DASI-score ≤5 METS), en onderzoekt ook de prognostische waarde van elke testmethode door de twee te vergelijken: incidentiecijfers per jaar voor vrouwen die adenosine SPECT-beeldvorming ondergaan of SPECT-beeldvorming met behulp van adenosine met aanvullende oefening.

Beschrijving
Inclusiecriteria:
- Vrouwelijk
- Leeftijd groter dan of gelijk aan 60 jaar
- Moet aanwezig zijn met pijn op de borst, vermoeidheid of andere angina-equivalente symptomen
- Moet worden doorverwezen voor stresstests op basis van klinische indicaties
- Moet schriftelijke geïnformeerde toestemming kunnen geven
Uitsluitingscriteria:
- Vrouwen met bekende coronaire hartziekte (>50% laesie OF eerder MI OF eerdere revascularisatie)
- Onvermogen om een oefening op een loopband uit te voeren
- Onderzoek naar nucleaire geneeskunde in de afgelopen 30 dagen
- Contra-indicatie voor adenosine, inclusief matige tot ernstige COPD of astma, tweede- of derdegraads AV-blok, of bekende overgevoeligheid voor adenosine of aminofylline
- Linkerbundeltakblok of elektronische ventriculaire pacemaker
- Aanzienlijke hartklepaandoening
- Hemodynamische instabiliteit (bloeddruk >210/110 ml/Hg of <90/60 mm/Hg)
- 2° of 3° atrioventriculair blok
- Symptomatisch hartfalen
- Inname van theofylline of dipyridamol binnen de voorgaande 48 uur
- Onbeschikbaarheid voor follow-up
