Marginale ulceratie "MU", die zich presenteert als een zweer aan de rand van de gastrojejunostomie aan de jejunale zijde, is een veel voorkomende late complicatie na RYGB. De incidentie na RYGB varieert van slechts 0,6 tot wel 16%. In onze handen met de laparoscopische handgenaaide techniek voor de GJ is de incidentie 1,4%. De aanwezigheid van specifieke technische factoren - dehiscentie van de nietjeslijn of gastro-gastrische fistel, vergroot zakje, vreemd materiaal en lokale ischemie - en omgevingsfactoren - onder andere tabak, NSAID's, alcoholgebruik en H pylori-infectie - zijn in verband gebracht met marginale ulceratie, maar de exacte etiopathogenese is niet volledig opgehelderd.
Net als bij maagzweren (PUD), reageren de meeste marginale ulcera op medische therapie, met name sucralfaat en zuurverlagende medicatie. Gecompliceerde marginale ulcera daarentegen - perforatie, bloeding of chroniciteit (obstructie, penetratie en hardnekkigheid) - rechtvaardigen operatieve interventie.
Een vroege presentatie van een bloeding na RYGB houdt meestal verband met het falen van de nietlijn en kan resulteren in een gastro-intestinale of intra-abdominale bloeding. Indien geïndiceerd, bestaan operatieve ingrepen uit ofwel endoscopische therapie, heroperatie of beide. Daarentegen is de late presentatie van gastro-intestinale bloeding na RYGB meestal secundair aan een bloedende marginale zweer, hoewel gecompliceerde maagzweren ook in de uitgesloten maag en twaalfvingerige darm kunnen voorkomen.
De meeste literatuur die beschikbaar is voor de behandeling van gastro-intestinale bloedingen na RYGB is voor de vroege presentatie van bloedingen secundair aan nietjeslijnfalen. De opties voor endoscopische hemostatische therapie die in dit scenario worden beschreven, zijn dus I) injectietherapie, II) coagulatietherapie, III) endoscopische clipping en IV) een gecombineerde modaliteit (bijvoorbeeld injectie & coagulatie of injectie en clipping).
De haalbaarheid, betrouwbaarheid, reproduceerbaarheid, werkzaamheid, validiteit en veiligheid van de endoscopische hemostatische therapie voor acuut bloedende maagzweren is goed gedocumenteerd. Er zijn ook meerdere risicostratificatietools voor bloedingen in het bovenste deel van het maagdarmkanaal ontwikkeld, zoals de Blatchford, klinische en volledige Rockall-scores en de Forrest-classificatie. Bovendien zijn pre- en post-endoscopische schema's van PPI's therapie bij patiënten met bloedende maagzweren effectief en kostenbesparend. Ze zijn echter niet allemaal gevalideerd in de obese populatiestatus na RYGB gecompliceerd met een bloedende marginale zweer.
Samenvattend, er is weinig informatie over de behandeling van late complicaties na een maagbypass, vooral na de wijdverbreide acceptatie van de laparoscopische benadering en de moderne anatomische constructie van Roux-en-Y Gastric Bypass-chirurgie. We analyseren formeel de effectiviteit van het management van patiënten met actief bloedende marginale ulcera na Roux-en-Y gastric bypass (RYGB) chirurgie.

Inclusiecriteria:
- patiëntenstatus na laparoscopische RYGB-operatie met actieve gastro-intestinale bloeding secundair aan marginale ulcera
Uitsluitingscriteria:
- bloedende marginale ulcera na andere bariatrische procedures
- nietlijnbloeding na RYGB
- bloedarmoede door ijzertekort (chronisch) secundair aan niet-actief bloedende marginale zweer na RYGB
- andere bronnen van gastro-intestinale bloedingen, verschillend van marginale zweren, zoals nietjeslijnen, gecompliceerde PUD en andere chirurgische en medische oorzaken van gastro-intestinale bloedingen
- ontbrekende dossiers en/of onbereikbare patiënten met weinig informatie voor analyse
