Europees protocol voor neuroblastoom met laag en gemiddeld risico
De Europese studie, LINES 2009 (Low and Intermediate Risk Neuroblastoma European Study), groepeert in één enkel protocol de behandeling van alle patiënten met "niet-hoog risico" neuroblastoom (NB), met stratificatie in twee groepen: laag risico en gemiddeld risico. Deze twee afzonderlijke cohorten zijn opgenomen in één enkel protocol, zodat patiëntgegevens van deze twee groepen in een gemeenschappelijke database kunnen worden ingevoerd, aangezien de huidige prognostische classificaties die de behandeling bepalen, verder kunnen evolueren met daaropvolgende meer gedetailleerde moleculaire analyse van de tumoren.
1. ONDERZOEK MET LAAG RISICO
De Low Risk Study wordt voorgesteld om:
- minimaliseer de hoeveelheid behandeling (chemotherapie en chirurgie) voor alle geschikte patiënten met een laag risico, van wie in eerdere onderzoeken is aangetoond dat ze een uitstekend resultaat op de lange termijn hebben (zoals in de SIOPEN 99.1-2-onderzoeken naar neuroblastoom bij zuigelingen waar de totale overleving groter was dan 97%(H. Rubie, JCO).
de EFS verbeteren en de OS (overall survival) behouden bij L2- en MS-patiënten met een SCA (Segmental Cromosomal Aberration) genomisch profieltumor (aanwezigheid van enige segmentale chromosomale verandering (SCA)) door deze patiënten electief te behandelen met chemotherapie ondanks de afwezigheid van symptomen .
2) TUSSENTIJDSE RISICOONDERZOEK
De Intermediate Risk Study wordt voorgesteld om:
- de hoeveelheid chemotherapie verminderen voor het differentiëren van histologie INRG (International Neuroblastoma Risk Group) stadium L2 NB en ganglioneuroblastoom nodulair patiënten waarvan in eerdere SIOPEN-studie is aangetoond dat ze een uitstekend langetermijnresultaat hebben;
- de hoeveelheid behandeling verhogen (radiotherapie en 13-cis-RA (13-cis-retinoïnezuur) voor slecht gedifferentieerde of ongedifferentieerde histologie INRG stadium L2 NB of ganglioneuroblastoom nodulaire patiënten om de EFS geregistreerd in de vorige SIOPEN-studie te verbeteren;
- verbetering van de EFS (Event Free Survival) van MYCN (V-Myc myelocytomatosis viraal gerelateerd oncogeen, NB-afgeleid, aviaire) versterkte INSS (International NB Staging System) stadium 1 NB-patiënten met de introductie van adjuvante behandeling;
- behoud van de zeer goede resultaten verkregen in de vorige SIOPEN-studie voor INRG-stadium M-zuigelingen met een matige behandeling.
NEONATALE SUPRARENALE MASSA'S
De incidentie van suprarenale tumoren/massa's is de afgelopen tien jaar toegenomen als gevolg van het uitgebreide gebruik van prenatale echografie in de routinematige verloskundige zorg en in de neonatale en vroege zuigelingenzorg. De differentiaaldiagnose van deze massa's varieert van goedaardig (bijnierbloeding) tot kwaadaardige processen (neuroblastoom, bijniercarcinoom). Kennis over perinatale suprarenale massa's, hoewel gebaseerd op een relatief grote literatuur, is verspreid over studies over zeer weinig gevallen zonder methodische aanpak en vaak korte follow-up. Daarom is het optimale beheer van deze massa's niet duidelijk gedefinieerd. Neuroblastoom op deze leeftijd is een intrigerende entiteit met in de meeste gevallen een zeer goede prognose. Op basis van hun resultaten in de eerste multicenter Europese studie voor zuigelingen met neuroblastoom (INES) en de wereldwijde ervaring in de literatuur, lanceert de SIOPEN-groep deze Europese surveillancestudie (Multi-centre, non-blinded, one arm prospective proef) voor deze massa's. 1. ONDERZOEK MET LAAG RISICO
De laagrisicogroep van patiënten omvat NB-patiënten zonder MYCN-amplificatie met of zonder levensbedreigende symptomen in de volgende klinische situaties:
- Kinderen van ≤ 18 maanden met gelokaliseerd neuroblastoom geassocieerd met door het beeld gedefinieerde risicofactoren die voorafgaande chirurgie uitsluiten (stadium INRG L2).
Kinderen van ≤ 12 maanden met gedissemineerd neuroblastoom zonder bot-, pleura-, long- of CZS-ziekte (stadium INRG Ms)
2) TUSSENTIJDSE RISICOONDERZOEK
De intermediaire risicogroep van patiënten omvat NB-patiënten in de volgende klinische situaties:
- Kinderen ouder dan 18 maanden met gelokaliseerd neuroblastoom zonder MYCN-amplificatie, geassocieerd met door het beeld gedefinieerde risicofactoren die een voorafgaande operatie uitsluiten (stadium INRG L2).
- Kinderen van ≤12 maanden met gedissemineerd neuroblastoom waarbij bot, borstvlies, long en/of CZS betrokken zijn (stadium INRG M), zonder MYCN-amplificatie.

Inclusiecriteria:
- Leeftijd minder dan of gelijk aan 90 dagen wanneer de suprarenale massa wordt ontdekt.
- Suprarenale massa gedetecteerd door echografie en/of MRI. De suprarenale massa kan cystisch en/of vast zijn, maar KAN DE MIDDENLIJN NIET BEREIKEN EN MOET ≤ 5 CM METEN OP DE GROOTSTE DIAMETER.
- Geen regionale betrokkenheid: MRI-scan toont geen bewijs van positieve ipsi/contralaterale lymfeklieren of andere verspreiding buiten de suprarenale klier.
- Geen metastatische betrokkenheid.
- Bevroren plasma beschikbaar.
- Geïnformeerde toestemming.
- Beschikbaarheid om de adequate follow-up te doen
Uitsluitingscriteria:
- Leeftijd ouder dan 90 dagen.
- Suprarenale massa groter dan 5 cm.
- Regionale betrokkenheid.
- Metastatische betrokkenheid.
- Onvermogen om verplichte diagnostische onderzoeken uit te voeren (biologische markers, US, MRI, MIBG).
