PANS - Een gedetailleerde studie van de patiënten, hun symptomen, biomarkers en behandeling aangeboden in een Scandinavisch cohort
Achtergrond: Obsessief-compulsieve stoornis (OCD) wordt beschouwd als een van de meest invaliderende psychiatrische ziekten, maar er is nog veel onduidelijk over oorzaken en behandelingen. Een diagnostische subgroep met acuut optredende obsessief-compulsieve symptomen (en soms tics of anorexia nervosa), mogelijk als gevolg van een auto-immuunreactie, wordt Pediatric Acute onset Neuropsychiatric Syndrome (PANS) genoemd. PANS wordt soms behandeld met immunomodulerende therapie of antibiotica, met een wisselend resultaat. Een diagnose van PANS wordt ondersteund door verhoogde niveaus van auto-antilichamen en antilichaam-enzymactiviteit gemeten met het Cunningham-panel, maar de relatie tussen deze biomarkers en de symptomen van de patiënt blijft onduidelijk. Er is een door een arts beoordeelde symptoomschaal voor PANS (de PANS-schaal) ontwikkeld, maar deze moet verder worden geëvalueerd wat betreft sensitiviteit en specificiteit.
Doelstellingen:
- Om een Zweeds cohort van patiënten met de diagnose PANS te beoordelen en hun psychiatrische gezondheidsstatus, biomarkers, psychiatrische symptomen, zachte neurologische symptomen en behandelingsresultaten op een systematische manier samen te stellen
- Om de psychiatrische gezondheidsstatus, biomarkers en psychiatrische, neurologische en motorische symptomen in dit PANS-cohort te vergelijken met een controlegroep van psychiatrische patiënten en met gezonde kinderen.
- Het Cunningham-panel evalueren als een diagnostisch hulpmiddel voor PANS.
- Een door een arts beoordeelde symptoomschaal (de PANS-schaal) evalueren als een diagnostisch hulpmiddel voor PANS.
Methode: Observationeel onderzoek Deelnemers: Patiënten (n≈150) die zijn getest met het Cunningham-panel van PANS-biomarkers in Zweden (of Zweedse patiënten getest in Denemarken) zullen worden gevraagd om deel te nemen.
Procedure: Beoordeling van huidige symptomen, psychiatrische gezondheid, neurologische en motorische symptomen en mogelijke biomarkers voor PANS zullen voor alle patiënten worden verzameld. Retrospectieve beoordeling door middel van interviews en medische dossiers, inclusief resultaten van de eerste beoordeling met het Cunningham-panel van PANS-biomarkers, wordt gemaakt met alle patiënten. 50 van het totale PANS-cohort van 150 patiënten zullen opnieuw worden getest met het Cunningham-panel. Een controlegroep bestaande uit psychiatrische patiënten (n=60) en gezonde kinderen (n=25) zal met een vergelijkbare testbatterij worden onderzocht en tekenen en symptomen zullen worden vergeleken met de PANS-groep.
Betekenis:
Eerdere en huidige symptomen van PANS, niveaus van PANS-biomarkers en behandelingsresultaten zullen worden onderzocht, waardoor kennis over langetermijnresultaten en bewijs voor het gebruik van klinische beoordelingsinstrumenten en biomarkers voor het diagnosticeren van PANS zal worden verkregen.

Alle huidige Zweedse gevallen die zijn onderzocht met het Cunningham-panel (ongeveer 150 personen) zullen worden uitgenodigd om deel te nemen aan het onderzoek.
