Evaluatie van microRNA-expressie in bloed- en cytologiemonsters als een nieuwe methode voor het detecteren van Barrett-slokdarm
Het primaire doel van deze studie is om nieuwe methoden te testen om BE op tijd te diagnosticeren voordat het vergevorderde kanker wordt.
Zodra BE is gediagnosticeerd, is de huidige zorgstandaard het monitoren van de ziekte, zodat complicaties zoals kanker vroegtijdig kunnen worden gediagnosticeerd.
De twee nieuwe methoden die de onderzoekers evalueren zijn: a) bloedtest en b) borsteltest van de slokdarm.
De onderzoekers verzamelen bloed, gal en cellen uit de voedselleiding en maag en meten voor een biomarker genaamd microRNA (miRNA).
In de toekomst zouden metingen van microRNA-biomarkers de artsen kunnen helpen erachter te komen welke patiënten een verhoogd risico lopen op slokdarmkanker.

De onderzoekers zullen serum-, gal- en slokdarmcellen verzamelen (met behulp van cytologieapparatuur) van instemmende proefpersonen.
De onderzoekers hebben eerder BE-specifiek miRNA geïdentificeerd dat zal worden getest op de cytologiemonsters.
Dit zijn miRNA's -192-5p, -215-5p en -194-5p.
De onderzoekers hebben ook serum-microRNA's in volledig serum en exosomen geïdentificeerd door sequencing om verder te bestuderen.
De onderzoekers zullen ook voor het eerst de miRNA-expressie in exosomen van de gal evalueren bij patiënten met refluxgerelateerde ziekten.

Studie Bevolking
Patiënten zullen worden aangeworven in het Kansas City VA Hospital en de Mobile Medical-eenheid van de Kansas City VA.
De Mobile Medical Unit (MMU) reist naar meerdere provincies om gezondheidszorg te bieden aan de veteranen voor de deur.
Dit is een programma dat al enkele jaren bestaat en nog steeds groeit.
Momenteel biedt de MMU diensten zoals basisgezondheidsevaluaties en aderlaten aan ~600 veteranen per week.
Beschrijving
Inclusiecriteria:
- Leeftijd patiënt: > 18 jaar
- Mogelijkheid om schriftelijke, geïnformeerde toestemming te geven
Uitsluitingscriteria:
- Zwangerschap of het plannen van een zwangerschap
- Geschiedenis van neus-, slokdarm- en maagchirurgie
- Geschiedenis van terugkerende epistaxis of neustrauma
- Proefpersonen met een voorgeschiedenis van onopgeloste drugs- of alcoholafhankelijkheid
- Onvermogen om biopsieën te verkrijgen als gevolg van een coagulopathie, varices, trombocytopenie, enz.
- Proefpersonen met ontoereikende cytologiemonsters zullen geen herhalingstest ondergaan en zullen van het onderzoek worden uitgesloten
- Onvermogen om schriftelijke geïnformeerde toestemming te geven
- Onvermogen om medicijnen zoals Plavix te stoppen
- Gevorderde chronische leverziekte
- Ernstige ongecontroleerde coagulopathie
- Actieve kanker in elk orgaan in de afgelopen 3 jaar.

Geen-GERD-controles
NERD-controles worden geregistreerd van proefpersonen die zich op de endoscopie-eenheid presenteren en die worden geëvalueerd om andere redenen dan GERD- of BE-surveillance. Dit zijn patiënten die naar de endoscopie-afdeling worden verwezen voor: beoordeling van bloedarmoede, dysfagie, occulte bloedpositiviteit, gastro-intestinaal bloedverlies etc.

• Patiënten die zich presenteren voor evaluatie van refluxsymptomen en die bij endoscopie ten minste 1 cm van de slokdarm met zuilvormige lijnen hebben, met intestinale metaplasie op biopsieën.
