Dit is een gerandomiseerde, dubbelblinde, placebogecontroleerde fase 2-studie met een inleiding van 12 patiënten om de veiligheid te evalueren, voorafgaand aan volledige inschrijving bij nog eens 28 patiënten (voor een totaal van 40 patiënten) om de werkzaamheid van decitabine bij de behandeling van ernstig zieke patiënten te beoordelen. patiënten met COVID-ARDS. De patiënten zullen worden gerandomiseerd in een verhouding van 1:1 om standaardzorg plus decitabine of standaardzorg plus op zoutoplossing gebaseerde placebo te krijgen.
Geschikte patiënten krijgen dagelijks decitabine 10 mg/m2 gedurende 5 dagen, slechts 1 cyclus. Dit is een dosis die de helft is van de door de FDA goedgekeurde dosis voor myelodysplastisch syndroom (MDS) en met een enkele cyclus.
Als minder dan 2 van de eerste 6 (behandelingsarm) patiënten binnen 15 dagen na aanvang van de behandeling een onaanvaardbare toxiciteit ervaren, gedefinieerd als elke aan de behandeling gerelateerde bijwerkingen van graad III of hoger, zoals vermeld in rubriek 5.7, kan het geneesmiddel veilig worden voortgezet. Als de onderzoekers meer dan 33% patiënten met onaanvaardbare toxiciteit observeren, zullen de onderzoekers de opbouw onderbreken in afwachting van de veiligheidsevaluatie. Nadat de veiligheid is gevalideerd, zullen de onderzoekers 28 extra patiënten inschrijven voor het primaire eindpunt voor werkzaamheid. De onderzoekers zullen de veiligheid tijdens het onderzoek bewaken door dagelijks de klinische hematologische, chemie, vitale functies, ademhalingsparameters, medicatie en klinische veranderingen te controleren volgens het schema van procedures.
Biomonsters van perifere mononucleaire bloedcellen (PBMC) en mini-bronchoalveolaire lavage (BAL) zullen worden verzameld en opgeslagen voor secundaire analyse en mini-BAL zal alleen worden verzameld als een optionele substudie voor patiënten die hebben ingestemd met een afzonderlijk onderzoeksprotocol, ofwel op tijd- klinisch geïndiceerde bronchoscopie, of voor proefpersonen die hebben ingestemd met een afzonderlijk interventieonderzoek naar bronchoalveolaire lavage (BAL), onder auspiciën van dat protocol. Voor onderzoeksbiospecimens die nodig zijn na de start van de studiemedicatie, is een periode van +/- 24 uur tijdens klinische opname en +/- 4 dagen voor poliklinische monitoring toegestaan.
Deze doelstellingen zullen de planning van volgende fase 3-onderzoeken mogelijk maken en de implementatie van een multicenter gerandomiseerde studie versterken, mocht deze studie de veiligheid bevestigen en de werkzaamheid van de therapie suggereren.

Inclusiecriteria:
- ≥18 jaar
- Gebruik van intermitterende mechanische ventilatie, niet-invasieve mechanische ventilatie (NIMV) of High Flow-neuscanule.
- Laat ARDS of Acute Lung Injury fysiologie bevestigen door Pao2/Fio2 ratio van < 300
- Severe Acute Respiratory Distress Syndrome (SARS) - Coronavirus (CoV-2) bepaald door laboratoriumpolymerasekettingreactieassay bij bemonstering van de bovenste of onderste luchtwegen (bijv. bronchoalveolaire lavage of nasofaryngeaal uitstrijkje)
- Als u zwanger kunt worden: stem ermee in effectieve anticonceptie toe te passen vanaf de screening tot ten minste 180 dagen na de laatste dosis
Uitsluitingscriteria:
- Hematologische cytopenieën: absoluut aantal neutrofielen (ANC) <1500/mm3, Hgb<7,0 en/of bloedplaatjes <100.000/mm3
- Proefpersonen die een klinische proef krijgen of deelnemen aan een andere onderzoeksbehandeling voor SARS-2-CoV.
- Actieve maligniteit, solide tumoren en huidige of recente chemotherapie
- Gelijktijdig gebruik van niet-biologische immunosuppressiva (bijv. Janus Kinase (JAK)-remmers, Bruton's Tyrosine Kinase (BTK)-remmers)
- Actieve hiv-viremie of een andere ongecontroleerde secundaire infectie.
- Gelijktijdige immunomodulerende biologische geneesmiddelen of gebruik van Palifermin, Dipyrone, Deferiprone
- Proefpersonen met ernstige sepsis met vasopressoren of extrapulmonaal orgaanfalen:
- Aspartaataminotransferase (AST) / alanineaminotransferase (ALT) / alkalische fosfatase (ALK) Phos ≥3x bovengrens van normaal (ULN) en totaal bilirubine (TBILI) ≥2x ULN; of creatinineklaring <30 ml/min
- Zwangere vrouwen of vrouwen die borstvoeding geven
- Elke voorwaarde, naar mening van PI die de veiligheid en/of naleving van het onderwerp zou kunnen beïnvloeden
- Eerdere overgevoeligheid voor decitabine

De duur van het onderzoek is 6 weken na de laatste dosis van het onderzoeksgeneesmiddel. Het aantal studiebezoeken is afhankelijk van de duur van de ziekenhuisopname van de studiedeelnemer. Studiebezoeken zijn gepland op dag 0-7, 11, 15, 29, en kunnen plaatsvinden via telegeneeskunde of klinische beoordeling of poliklinische beoordeling bij COVID-herstelde deelnemers.
