Deelname Criteria

Inclusiecriteria:

Deelnemers komen alleen in aanmerking voor deelname aan het onderzoek als aan alle volgende criteria is voldaan:

  • Heeft nieuw gediagnosticeerde, niet-gereseceerde LA, histologisch bevestigde HNSCC van de mondholte, orofarynx, hypofarynx of strottenhoofd en voltooide cisplatine plus radiotherapie (in dit protocol "CRT" genoemd) met curatieve bedoeling en heeft geen bewijs van metastatische ziekte op afstand.

  • Heeft aanvaardbaar kern- of excisieweefsel opgeleverd waaruit blijkt:

    • PD-L1 positieve tumorstatus
    • Als de primaire tumorplaats een orofarynxcarcinoom is, moet de deelnemer een p16 IHC-test ondergaan.
  • Heeft een Eastern Cooperative Oncology Group (ECOG)-prestatiestatus van 0 of 1
  • Heeft voldoende orgaanfunctie.

Uitsluitingscriteria:

Deelnemers worden uitgesloten van het onderzoek als een van de volgende criteria van toepassing is:

  • Heeft eerder radiotherapie, systemische therapie, gerichte therapie of radicale chirurgie ondergaan voor de behandeling van hoofd-halskanker die niet als onderdeel van CRT wordt beschouwd.
  • Heeft kanker buiten de orofarynx, het strottenhoofd, de hypofarynx of de mondholte, zoals nasofaryngeale, sinus-, andere paranasale of andere onbekende primaire hoofd- en nekkanker.
  • Heeft binnen 28 dagen voorafgaand aan de inschrijving een grote chirurgische ingreep ondergaan of een aanzienlijk traumatisch letsel opgelopen.
  • Heeft een voorgeschiedenis van interstitiële longziekte of pneumonitis (verleden of huidig).
  • Heeft cirrose of een huidige instabiele lever- en galziekte, volgens de beoordeling van de onderzoeker, gedefinieerd door de aanwezigheid van ascites, encefalopathie, coagulopathie, hypoalbuminemie, slokdarm-/maagvarices of aanhoudende geelzucht.
  • Heeft een geschiedenis of actueel bewijs van een medische aandoening, therapie of laboratoriumafwijking die de onderzoeksresultaten zou kunnen verwarren, hun deelname gedurende de volledige duur van de onderzoeksinterventie zou kunnen verstoren, of erop zou kunnen wijzen dat deelname niet in het beste belang van de deelnemer is , naar het oordeel van de onderzoeker.
  • Krijgt een andere antikanker- of experimentele therapie. Er zijn geen andere experimentele therapieën (inclusief maar niet beperkt tot chemotherapie, bestraling, hormonale behandeling, antilichaamtherapie, immuuntherapie, gentherapie, vaccintherapie of andere experimentele medicijnen) van welke aard dan ook toegestaan ​​terwijl de deelnemer studieinterventie ontvangt.
  • Eerdere behandeling met een anti-PD-1-, anti-PD-L1- of anti-PD-L2-middel of een middel dat is gericht op een andere stimulerende of co-remmende T-celreceptor (bijv. Cytotoxisch T-lymfocyt-geassocieerd eiwit 4 (CTLA4), ( OX-40, CD134).
  • Is zwanger, geeft borstvoeding of verwacht kinderen te krijgen binnen de verwachte duur van het onderzoek, beginnend met het screeningsbezoek tot en met 120 dagen na de laatste dosis onderzoeksinterventie.
  • Heeft een voorgeschiedenis van ernstige allergische en/of anafylactische reacties op chimere, menselijke of gehumaniseerde antilichamen, fusie-eiwitten of bekende allergieën voor dostarlimab of de hulpstoffen ervan.
