Подробное описание
Повреждение спинного мозга (SCI) значительно ухудшает способность стоять и ходить, что серьезно затрудняет повседневную деятельность. Хотя этот дефицит частично устраняется локомоторной тренировкой, даже после многократных тренировок аномальная мышечная активность и координация все еще сохраняются. Таким образом, одни только двигательные тренировки не могут полностью оптимизировать пластичность нейронов, необходимую для укрепления синапсов, соединяющих головной и спинной мозг и местные цепи. Таким образом, крайне необходимы лечебные вмешательства, которые эффективно способствуют нейромодуляции локомоторных сетей позвоночника и укрепляют нейронные связи поврежденного спинного мозга человека в сочетании с физической реабилитацией. Предлагается использовать чрескожную (трансспинальную) стимуляцию спинного мозга как метод синергетического «запуска» нервной системы, чтобы она лучше реагировала на двигательную тренировку. Трансспинальная стимуляция изменяет возбудимость мотонейронов во многих сегментах позвоночника, что является предпосылкой для функционирования нисходящих и локальных входов. Важно отметить, что неизвестно, максимизирует ли одновременное лечение трансспинальной стимуляцией и локомоторной тренировкой двигательное восстановление после ТСМ. Целью этого клинического исследования является использование высокочастотной (30 Гц) трансспинальной стимуляции для начального двигательного тренинга и, в конечном счете, улучшения стояния, ходьбы и общей функции у людей с хронической неполной ТСМ (iSCI). Сорок пять человек с iSCI пройдут 40 сеансов степ-тренировки с поддержкой веса тела с высокочастотной трансспинальной стимуляцией. Участники будут рандомизированы для получения трансспинальной стимуляции в положении стоя (настоящее или фиктивное) или в положении лежа на спине (настоящее). Цель 1 оценивает, как предварительная двигательная тренировка с высокочастотной трансспинальной стимуляцией при травме спинного мозга изменяет силу корково-тоннерональной связи, о чем свидетельствуют двигательные вызванные потенциалы, зарегистрированные в ногах.
Цель 2 оценивает, как предварительная двигательная тренировка с высокочастотной трансспинальной стимуляцией при iSCI влияет на реорганизацию и соответствующее вовлечение спинальных нейронных цепей. Наконец, Цель 3 оценивает моторную функцию, способность стоять и ходить, а также качество жизни. Эти результаты подтверждают идею о том, что тоническая высокочастотная трансспинальная стимуляция укрепляет корково-тонерональную связность и улучшает организацию спинномозговой цепи за счет корково-спинномозговой нейропластичности, зависящей от позы. Ожидается, что информация, полученная в результате этого механистического клинического испытания, окажет большое влияние на клиническую практику. Это связано с тем, что в реальных клинических условиях неинвазивная трансспинальная стимуляция может быть реализована проще и шире, чем инвазивная эпидуральная стимуляция.

Описание
Критерии включения:
- Готовность соблюдать все процедуры исследования и доступность на время исследования.
- Умение понимать форму согласия и подписывать форму согласия.
- Мужчина или женщина, возраст 18-70 лет.
- Общее состояние хорошее, что подтверждается анамнезом.
- Диагноз: двигательная неполная травма спинного мозга (AIS C-D).
- Минеральная плотность кости тазобедренного сустава (проксимальный отдел бедренной кости) Т-критерий
- Поражение выше грудного отдела (T) 10, чтобы гарантировать отсутствие поражения нижнего мотонейрона.
- Наличие сухожильных рефлексов, позволяющих вызвать Н-рефлекс камбаловидной мышцы.
- Отсутствие постоянных контрактур голеностопного сустава, препятствующих пассивным или активным движениям в голеностопном суставе, поскольку корково-спинномозговая и спинальная возбудимость зависит от угла голеностопного сустава. Ремни на лодыжках Lokomat также требуют гибких голеностопных суставов.
- Диагноз ТСМ впервые вследствие травмы, сосудистой или ортопедической патологии.
- Срок после ТСМ более 6 мес.
- Стабильное медицинское состояние без сердечно-легочных заболеваний или когнитивных нарушений.
Критерий исключения:
- Супраспинальные поражения.
- Значительные невропатии периферической нервной системы.
- Значительные дегенеративные неврологические расстройства позвоночника или спинного мозга.
- АИС А или Б.
- Наличие пролежней.
- Запущенная инфекция мочевыводящих путей.
- Неопластические или сосудистые заболевания позвоночника или спинного мозга.
- Участие в продолжающемся научном исследовании или новой программе реабилитации.
- Беременные женщины или женщины, которые подозревают, что они могут быть или могут забеременеть, будут исключены из участия, поскольку риски грудопоясничной стимуляции для плода неизвестны.
- Люди с кохлеарными имплантами, кардиостимуляторами, имплантированными инфузионными устройствами и/или имплантированными стимуляторами любого типа и назначения будут исключены во избежание их неисправности из-за стимуляции.
- Люди с историей судорог.
- Медицинские состояния, повышающие вероятность судорог.

Трансспинальная тоническая стимуляция грудопоясничной области будет проводиться с частотой 30 Гц в положении стоя с необходимой поддержкой веса тела (BWS) в вертикальной раме или в Lokomat для обеспечения безопасности.

Пятнадцать человек с травмой спинного мозга получат 40 ежедневных 30-минутных сеансов неинвазивной высокочастотной (например, 30 Гц) чрескожной трансспинальной стимуляции в положении стоя, после чего следует 30-минутная тренировка шагающей роботизированной ходьбы.
До и после тренировки будут использоваться стандартизированные клинические и нейрофизиологические тесты для оценки восстановления сенсомоторной функции.
