描述
纳入标准:
- 出现 ACS* 并接受侵入性冠状动脉造影并可识别罪魁祸首病变的患者
- 在急性事件发生前接受冠状动脉 CT 血管造影的患者,无论出于何种原因(例如,常规健康检查,或稳定型心绞痛或非典型胸痛的评估)。
CCTA时限:活动前1个月~3年。
- ACS 的定义:
A. 急性心肌梗死患者应有心肌酶升高,并经有创冠状动脉造影、IVUS 或 OCT 确定罪魁祸首病变。
B.不稳定型心绞痛患者应有斑块破裂的证据,至少包括以下一项:(1)有创冠状动脉造影存在斑块破裂或混浊包括血栓,(2)血管造影狭窄≥90%,或(3)IVUS或OCT证实的破裂证据。
患者登记的排除标准
- 没有明确证据表明罪犯病变的 ACS 患者
- 在 CCTA 之前在两个或多个血管区域放置支架的患者
- CCTA 质量差,不适合牙菌斑和 CFD 分析
- 支架植入部分发生 ACS 罪魁祸首的患者
- 既往有冠状动脉旁路移植手术史的患者
- 在 CCTA 之后和 ACS 事件之前进行血运重建的患者(*可以入组 CCTA 后 3 个月内对 1 条血管进行选择性 PCI 的患者。
- 继发性 ACS 由于其他一般医疗条件,如败血症、心律失常、出血等。
- 没有斑块破裂证据的不稳定型心绞痛患者 计算流体动力学的附加排除标准
- 质量差的 CCTA 图像不适合 CFD 和斑块分析
- 没有未处理的 CCTA 数据
