Kryoterapi for retinopati hos præmaturiteter (CRYO-ROP) - Resultatundersøgelse af kryoterapi til præmaturitetsretinopati
At bestemme sikkerheden og effektiviteten af transskleral kryoterapi af den perifere nethinde hos visse spædbørn med lav fødselsvægt med retinopati af præmaturitet (ROP) for at reducere blindhed fra ROP.

ROP er en sygdom i øjnene hos for tidligt fødte spædbørn, hvor nethindens blodkar øges i antal og forgrener sig overdrevent, hvilket nogle gange fører til blødning eller ardannelse. Før etableringen af denne undersøgelse i 1985 blev mere end 500 spædbørn årligt blindet af ROP alene i USA.
For mere end 30 år siden sponsorerede National Institutes of Health et klinisk forsøg, der viste, at hvis for tidligt fødte børn kun får ilt efter behov, falder antallet af spædbørn, der udvikler ROP, dramatisk. Efterfølgende skærer hospitalerne ned på rutinemæssigt at give for meget ilt til for tidligt fødte børn. Men med forbedringer i neonatalpleje i løbet af de sidste to årtier stiger antallet af risikobørn, efterhånden som overlevelsesraterne for mindre for tidligt fødte børn forbedres. Jo lavere fødselsvægt, jo højere forekomst og sværhedsgrad af ROP.
I en nyere NEI-støttet undersøgelse ved University of Miami blev iltniveauer i blodet hos spædbørn med meget lav fødselsvægt overvåget kontinuerligt ved brug af transkutane målinger, så længe der var behov for iltbehandling. Undersøgelsen viste, at der ikke er nogen statistisk signifikant forskel mellem frekvensen af ROP hos spædbørn overvåget på kontinuerlig iltbehandling og hos dem, der kun overvåges, når de fik ilt på over 40 procent.
Studiet med Supplemental Therapeutic Oxygen for Prethreshold ROP (STOP-ROP), også finansieret af NEI, undersøgte, om en lille stigning i iltbehandling ville forhindre progression af moderat ROP til ROP, der er alvorlig nok til at kræve kirurgisk behandling. Denne intervention gjorde ringe eller ingen forskel i resultater.
Ligeledes viste et andet NEI-sponsoreret klinisk forsøg (LIGHT-ROP) fravær af beskyttende effekt på ROP ved at begrænse lyseksponering for nyfødte for tidligt fødte spædbørn. Disse undersøgelser har ført til den konklusion, at andre faktorer end ilt- eller lyseksponering skal være involveret i at forårsage ROP.
Hos de fleste spædbørn, der udvikler ROP, aftager sygdommen spontant, hvilket tillader udvikling af normalt syn. Men andre spædbørn, der udvikler sig til en alvorlig form for ROP, er i fare for at blive permanent blinde. Selvom årsagen til ROP ikke er fuldt ud forklaret, søger forskere måder at behandle ROP med succes og finde det rigtige tidspunkt i sygdommens udvikling til at bruge behandling. Kryoterapi, som ødelægger kanten af nethinden gennem frysning, er den eneste behandling hidtil, der har vist sig at give væsentlig fordel for disse øjne.
Multicenterforsøget med kryoterapi til ROP indskrev mere end 4.000 for tidligt fødte børn, som ikke vejede mere end 1.250 gram ved fødslen. Denne kategori af spædbørn har størst risiko for at udvikle ROP. Øjnene på de spædbørn, der var indskrevet i undersøgelsen, blev undersøgt med forudbestemte intervaller, mens forsøgspersonerne stadig var i intensiv-vuggestuen. Efter at pupillerne var blevet udvidet med øjendråber, blev øjnene undersøgt af en øjenlæge ved hjælp af et kikkert indirekte oftalmoskop for at visualisere den udviklende nethinde. Den naturlige historie af tilstanden af hvert spædbarns nethinde blev registreret. Da undersøgelsen afslørede den alvorlige form for ROP (tærskel ROP) i begge øjne, og forældrene gav informeret samtykke, blev et af spædbarnets øjne tilfældigt udvalgt til at modtage kryoterapi. I denne teknik blev en kryoprobe brugt til at fryse og dermed ødelægge den perifere udstrækning af nethinden og derved standse udviklingen af de blodkar, der voksede vildt mod den.
Resultatet af terapien blev vurderet 3 måneder og 12 måneder efter randomisering ved en omfattende undersøgelse, der omfattede fotografering af det indre af både de behandlede og kontroløjne. Den 12-måneders eksamen målte også visuel funktion med teknikker med præference-look. Sådanne målinger muliggjorde korrelationer mellem fundusfotografier og visuel funktion og en sammenligning af synsfunktion for behandlede kontra kontroløjne. Hverken de uddannede fotolæsere, der vurderede billederne fra begge øjne, eller de specialtrænede synstestere vidste, hvilke øjne der havde fået kryoterapi. Yderligere vurderinger af synsstyrke og nethindestatus er blevet foretaget cirka hvert år indtil nu. I øjeblikket (2001) forberedes en 15-årig resultatundersøgelse, der afsluttes i 2003.
