Le système Calypso® (Varian Medical Systems, Inc., Palo Alto, CA) est une technologie récente utilisant des transpondeurs électromagnétiques implantés dans la prostate. Il s'agit d'un système de suivi de cible en temps réel qui prend en compte à la fois le mouvement de cible inter et intrafractionnel. Ainsi, la position et le mouvement exacts de la prostate peuvent être déterminés pendant le traitement de radiothérapie.
Le but de cette étude est d'évaluer le taux de toxicité tardive pelvienne après radiothérapie avec modulation d'intensité (IMRT) lors de l'utilisation du système Calypso® avec une réduction des marges de traitement. Dans cette étude randomisée, les patients recevront un traitement IGRT en utilisant le système Calypso ou un traitement IGRT conventionnel.

Description détaillée
Le cancer de la prostate est le cancer le plus fréquent chez l'homme. La radiothérapie est l'un des traitements de référence du cancer localisé de la prostate. La prostate était initialement considérée comme une cible immobile, mais les derniers rapports ont soulevé le contraire comme preuve. La radiothérapie guidée par l'image (IGRT) est un traitement standard actuel du cancer localisé de la prostate qui permet de prendre en compte les mouvements interfractionnels de la prostate avant le traitement.
Le système Calypso® (Varian Medical Systems, Inc., Palo Alto, CA) est une technologie récente utilisant des transpondeurs électromagnétiques implantés dans la prostate. Il s'agit d'un système de suivi de cible en temps réel qui prend en compte à la fois le mouvement de cible inter et intrafractionnel. Ainsi, la position et le mouvement exacts de la prostate peuvent être déterminés pendant le traitement de radiothérapie.
Comme les marges de volume cible planifié (PTV) intègrent les marges de configuration et la gestion du mouvement des organes, une réduction de la marge peut raisonnablement être envisagée en cas d'amélioration de la gestion du mouvement cible.
À ce jour, aucune donnée clinique prospective randomisée n'est disponible sur l'utilisation de cette technique pour le traitement des patients atteints d'un cancer de la prostate à risque faible ou intermédiaire avec une radiothérapie par fractionnement standard moderne et un guidage par l'image.
Le but de cette étude est d'évaluer le taux de toxicité tardive pelvienne après radiothérapie avec modulation d'intensité (IMRT) lors de l'utilisation du système Calypso® avec une réduction des marges de traitement. Dans cette étude randomisée, les patients recevront un traitement IGRT en utilisant le système Calypso ou un traitement IGRT conventionnel.

La description
Critère d'intégration:
- Cancer localisé de la prostate, prouvé histologiquement.
- Aucun signe de métastases (M0). Aucun signe d'atteinte ganglionnaire (N0) (le curage ganglionnaire bilatéral n'est pas obligatoire si le risque d'atteinte ganglionnaire est faible selon les tables de Partin).
- Stade clinique à faible risque ou intermédiaire selon la classification D'Amico (T1-T2 et Gleason < 8 et PSA < 20 ng/ml) (annexe 3).
- Aucun signe ou symptôme clinique urinaire ou rectal de grade > 2 selon l'échelle CTCAE V4.03.
- Statut de performance ECOG ≤ 1.
- Pas de prothèse de hanche ou de greffon vasculaire métallique à proximité de la prostate.
- Pas de stent endopénien.
- Pas de stimulateur cardiaque, de défibrillateur implanté ou de neurostimulateur.
- Pas d'allergie aux anesthésiques locaux.
- Pas d'anticoagulation irréversible ni de traitement antiplaquettaire pendant la période d'implantation.
- Anatomie pelvienne et abdominale compatible avec l'utilisation du système Calypso® (distance détecteur prédictif aux repères inférieure à 19 cm, évaluée sur scanner de planification) (distance de la surface de la peau au centre de la prostate inférieure à 17 cm).
- Patient âgé de ≥ 18 ans et de moins de 80 ans.
- Consentement éclairé écrit daté et signé disponible.
- Les patients doivent être affiliés à un système français de sécurité sociale.
Critère d'exclusion:
- Indication d'irradiation des ganglions pelviens.
- Irradiation pelvienne préalable.
- Invasion des vésicules séminales prouvée par biopsie.
- Orchidectomie bilatérale préalable.
- Prostatectomie radicale préalable.
- Autre tumeur maligne à l'exception d'un carcinome basocellulaire de la peau traité de manière adéquate ou d'une autre tumeur maligne dont le patient est exempt de maladie depuis au moins 5 ans.
- Condition psychologique, familiale, sociologique ou géographique pouvant entraver le respect du protocole d'étude et du calendrier de suivi ; ces conditions doivent être discutées avec le patient avant son inscription à l'essai.
- Autre maladie systémique non contrôlée (cardiovasculaire, rénale, hépatique, embolie pulmonaire, etc.).
- Patients VIH positifs connus (pas de test spécifique nécessaire).
- Mutation ATM homozygote connue (Ataxie télengiectasie).
