Effets du régime cétogène sur la fréquence des crises non épileptiques chez les patients présentant un trouble des symptômes neurologiques dissociatifs : une étude pilote

Description détaillée
Cette étude consiste en un essai clinique contrôlé randomisé ouvert composé de 20 patients ayant reçu un diagnostic de trouble des symptômes neurologiques fonctionnels avec des crises non épileptiques répartis en 2 bras de 10 patients chacun.
Les patients seront assignés au hasard à l'un des groupes. Les deux groupes recevront un traitement comme d'habitude (ISRS et/ou psychothérapie). Un groupe se verra attribuer un régime cétogène tandis que l'autre se verra attribuer un régime alimentaire sain, tous deux supervisés par une équipe de nutritionnistes. Les patients seront évalués toutes les 2 semaines jusqu'à atteindre 6 semaines.

Critère d'intégration:
- Les patients qui ont un dossier à l'institut et qui ont un diagnostic de trouble des symptômes neurologiques dissociatifs avec des crises non épileptiques avec une certitude au moins cliniquement établie qui ont une fréquence mensuelle des crises supérieure à 3.
- Patients présentant les caractéristiques décrites ci-dessus avec une épilepsie comorbide mais qui n'ont pas présenté de crises d'épilepsie au cours du dernier mois.
- Patients présentant les caractéristiques susmentionnées et qui donnent leur consentement éclairé par écrit pour participer à l'étude.
- Les patients qui, une fois le régime leur expliqué, estiment possible de le mettre en œuvre au moins pendant la durée du suivi.
Critère d'exclusion:
- Patients présentant une instabilité métabolique ou hémodynamique, une insuffisance hépatique, une incapacité à tolérer la voie orale, une pancréatite aiguë ou une grossesse en raison d'un éventuel effet tératogène non encore élucidé.
- Patients présentant des troubles métaboliques tels qu'un déficit primaire en carnitine, un déficit en palmitoyltransférase carnitine I ou II, un déficit en carnitine translocase, des défauts d'oxydation bêta, un déficit en pyruvate carboxylase, une porphyrie et d'autres troubles du transport et de l'oxydation des acides gras.

Expérimental: régime cétogène + traitement conventionnel
L'équipe de nutrition aidera les patients à mettre en place un régime cétogène de type Atkins.
Le régime doit être poursuivi pendant au moins 6 semaines.
Ils pourront continuer le traitement à base pharmacologique mais sans qu'aucune modification n'ait été apportée au cours des 6 dernières semaines ou pendant la durée de leur participation à l'étude.
Pour assurer le respect du régime, les cétones urinaires seront déterminées deux fois par semaine.

Régime modifié Atkins : 1,2-1,5 g/kg/jour de protéines avec un rapport 1:1 (lipides : glucides + protéines)

Comparateur actif: Régime alimentaire sain + traitement conventionnel
En plus de leur traitement de base (pharmacologique), l'équipe de nutrition les aidera à mettre en place un régime hypocalorique non cétogène.
La présence de cétones sera également déterminée dans l'urine pour éviter les biais
|
Régime normocalorique établi par un nutritionniste.
1,2-1,5 g/kg/jour de protéines avec une contribution maximale de 50 % de glucides de l'apport calorique total en éliminant les aliments industriels.
