L'acne è una condizione infiammatoria cronica che coinvolge l'unità pilosebacea.
Una delle complicazioni più comuni dell'acne è la cicatrizzazione, che si verifica a seguito di un danno dermico durante il processo di guarigione dell'acne attiva.
Gli studi hanno dimostrato che le cicatrici da acne possono avere un impatto negativo sulla qualità della vita di un individuo ed è stato descritto come un fattore di rischio per molteplici condizioni psichiatriche tra cui depressione, ansia, suicidio e scarsa autostima.
Sfortunatamente, il trattamento delle cicatrici da acne, in particolare quelle del sottotipo atrofico, si rivela difficile da trattare.
Una revisione Cochrane del 2016 non è riuscita a fornire prove sufficienti per supportare l'uso di prima linea di qualsiasi intervento.
La tecnologia laser sta emergendo come trattamento efficace e non invasivo delle cicatrici da acne.
I laser ablativi funzionano fornendo energia allo strato dermico della pelle, provocando lesioni termiche e stimolando le cellule dermiche a produrre collagene, risolvendo così la cicatrice.
Più recentemente, sono stati introdotti i laser frazionali, che creano più minuscole colonne di lesioni termiche, con il vantaggio di ridurre i tempi di inattività e gli effetti negativi rispetto ai laser ablativi.
Il laser a CO2 frazionato è ampiamente utilizzato per il trattamento delle cicatrici da acne, tuttavia il suo uso è limitato nei pazienti con pelle Fitzpatrick IV e V a causa del rischio di iperpigmentazione tra gli altri eventi avversi.
Il laser frazionato Erbio-YAG funge da alternativa e ha dimostrato di produrre meno lesioni termiche e quindi di essere più ben tollerato con un minor rischio di effetti avversi nei pazienti.
Il laser frazionale ibrido combina una lunghezza d'onda di 2940 nm con una lunghezza d'onda di 1470 e ha una componente ablativa mirata all'epidermide e una componente non ablativa mirata al derma nello stesso punto.
I ricercatori cercano di indagare sullo Sciton Halo, laser frazionato ibrido per la sua efficacia nel trattamento delle cicatrici da acne e della tollerabilità nei pazienti con un'enfasi specifica su quelli con pelle Fitzpatrick IV e V.

Criterio di inclusione:
- - Maschi e femmine sani, ≥ 18 anni di età al momento del consenso informato, in cerca di trattamento per cicatrici da acne
- Il soggetto deve essere di tipo cutaneo Fitzpatrick III-VI
- Il soggetto deve volontariamente firmare e datare un modulo di consenso informato approvato dall'IRB
- Soggetti con diagnosi di cicatrici da acne registrate negli ultimi 6 mesi
- In grado di leggere, comprendere e fornire volontariamente il consenso informato scritto.
- Il soggetto è determinato ad essere sano, non fumatore
- Soggetti in grado e disposti a rispettare il protocollo di trattamento e il programma e i requisiti di follow-up.
- Comprende e accetta l'obbligo di non sottoporsi ad altre procedure nelle aree da trattare durante il periodo di follow-up.
Criteri di esclusione:
- I soggetti non hanno la capacità di acconsentire allo studio
- il soggetto è stato sottoposto a trattamenti per cicatrici da acne negli ultimi 6 mesi prima dell'arruolamento nello studio
- Il soggetto ha un'acne papulo-pustolosa o cistica attiva negli ultimi 6 mesi.
- Qualsiasi storia di cicatrici cheloidee.
- Qualsiasi precedente intervento chirurgico nell'area di trattamento negli ultimi 12 mesi o intervento chirurgico importante negli ultimi 6 mesi.
- Storia di disturbi da immunosoppressione/deficienza immunitaria (inclusi AIDS e infezione da HIV) e/o qualsiasi storia di chemioterapia sistemica nei 12 mesi precedenti.
- Storia o uso attuale dei seguenti farmaci su prescrizione:
Farmaci immunosoppressivi/farmaci biologici, 6 mesi prima e durante lo studio
- Accutane o altri retinoidi sistemici negli ultimi dodici mesi
- Fumare o svapare negli ultimi 12 mesi.
- Storia di fotosensibilità e/o malattia del tessuto connettivo
- Storia di iperlipidemia, diabete mellito, epatite o disturbi della coagulazione.
- Storia di disturbi depressivi maggiori o disturbi endocrini inclusi ma non limitati a; ipotiroidismo, tiroidite di Hashimoto o ipertiroidismo.
- Storia di gravidanza in corso, allattamento al seno attivo, cancro ed epilessia

Sperimentale: Laser Frazionale Ibrido - SOC
23 partecipanti con tipo di pelle fitzpatrick IV-V saranno in questo braccio.
Alla prima visita, i partecipanti saranno sottoposti a consenso informato, istruzione sulla procedura, educazione sulla corretta cura del sole e verrà fornita la protezione solare.
Ai partecipanti verrà prescritta una prescrizione di idrochinone o retinolo per prevenire l'iperpigmentazione, secondo lo standard di cura.
Se i partecipanti scelgono di prenderlo, aspetteranno un mese dall'assunzione della prescrizione e quindi inizieranno le sessioni di trattamento.
In caso contrario, i partecipanti possono iniziare subito le sessioni di trattamento.
Il trattamento consiste in 3 sessioni laser con il laser frazionato ibrido Sciton Halo.
L'importo della sessione dipende dal grado di miglioramento.
Ad ogni sessione verranno raccolte le immagini prima e dopo.
Ad ogni sessione i partecipanti compileranno il sondaggio sulla qualità della vita in dermatologia e un sondaggio sulla soddisfazione del paziente.

Sperimentale: Laser frazionale ibrido - Non SOC
23 partecipanti con pelle Fitzpatrick di tipo I-III saranno in questo braccio.
Alla prima visita, i partecipanti saranno sottoposti a consenso informato, istruzione sulla procedura, educazione sulla corretta cura del sole e verrà fornita la protezione solare.
Ai partecipanti verrà prescritta una prescrizione di idrochinone o retinolo per prevenire l'iperpigmentazione, secondo lo standard di cura.

Change in Acne Scar Severity

|
Change in acne scarring severity from baseline to nine months will be assessed using the Global Scale for Acne Scar Severity (SCAR-S) scale.
The SCAR-S is a validated scale used to assess the severity of acne scarring using a 6-point scale ranging from 0 ("Clear, no visible scars from acne") to 5 ("Very Severe; entire area with prominent atrophic or hypertrophic scars") such that higher scores are indicative of more severe scarring.
Two independent blinded dermatologists will rate before and after treatment images using the SCAR-S scale.
Results will be summarized by study arm using descriptive statistics.

Change in Clinician Graded Global Assessment of Improvement Scale (GAIS) score

Change in aesthetic success from baseline will be evaluated using the Clinician's Global Assessment of Improvement Scale (CGAIS).
The CGAIS evaluates overall efficacy and cosmetic outcomes of the treatment.
The CGAIS is based upon a 7-point scale ranging from -3 ("Greatly worsened") to +3 ("Greatly improved"), with 0 ("No change") as the midpoint.
Two independent unbiased blinded dermatologists will conduct live/photographic assessments to evaluate patients at baseline, Month 2, Month 3, Month 4, Month 6, and Month 9. Results will be summarized by study arm using descriptive statistics.

This question asks patients to select the option that better represents the change in how their skin looks using a 7-point scale ranging from -3 ("Greatly decreased") to +3 ("Greatly increased"), with 0 ("No change") as the midpoint.
Scores will be summarized by study arm using descriptive statistics.

This question asks patients "beyond texture, pigmentation, and overall acne scarring, did you notice any other changes in the area that was treated."
Patients will select either "Yes" or "No."

Patients who confirmed that they did notice changes in the "Patient Assessment of General Skin Changes - Binary" query on the PSQ will be asked to specify what those changes were from a menu of options.
These options are "Pores are smaller," "Skin is less oily," "Skin is smoother," "Skin feels better," "Skin looks more radiant," "Face feels more youthful," and "Other."
Patients will have the option of selecting more than one response.
Responses will be categorically summarized by study arm.

Patients will be asked whether they are experienced any of the following side effects following laser treatment: "Erythema," "Prolonged erythema," "Acne flare up," "Hypopigmentation," and "Hyperpigmentation." on the Patient Satisfaction Questionnaire (PSQ) completed during each visit.
Patients can select any/all treatment-related side effects which they experienced.
Responses will be categorically summarized by study arm.

Pain Discomfort

One of the safety variables will be pain discomfort.
Patients will be asked to quantify the degree of pain discomfort experienced following treatment using a pain scale item provided on the Patient Satisfaction Questionnaire (PSQ) completed during each visit.
Patients will rate their pain discomfort on an 11-point Likert scale ranging from 0 ("No Pain") to 10 ("Worst Possible Pain"), such that higher scores are indicative of increased pain discomfort.
Results will be summarized by study arm using descriptive statistics.

Dermatology Quality of Life

|
Dermatology quality of life will be assessed using the Dermatology Life Quality Index (DLQI).
The DLQI is a standardized 10-item dermatology scale which determines how much impact a skin condition has had on life quality.
Patients are asked to consider how much their skin problem has affected their life over the prior week when responding to each question.
Questions are subjectively answered by patients using the following choices: "Very much" (= 3 points), "A lot" (= 2 points), "A little" (= 1 point), and "Not at all" or "Not relevant" (= 0 points).
Scores are summarized for an overall possible range of 0-30, such that higher scores are indicative of greater dermatology-related life quality impairment.
Results will be summarized by study arm using descriptive statistics.
