2 型糖尿病の高齢者における転倒予防の結果を評価する (FallT2DMOLD)
このランダム化比較臨床試験は、2 型糖尿病の高齢者における転倒予防の効果を評価することを目的としています。 回答することを目的とした主な質問は次のとおりです。
- 高齢の 2 型糖尿病患者における転倒の現状、転倒のリスク、およびいくつかの関連要因について説明する
- 研究対象集団における転倒予防の結果と関連要因を評価します。
参加者はランダムに対照群と介入群に分けられます。 介入グループは、主な転倒予防法としてオタゴトレーニングプログラムに従うように訓練されます。 一方、対照群は、2型糖尿病および他の併存疾患の治療ガイドラインに従って標準治療を受けることになります。
介入の終了時に、研究では主に介入6か月後の転倒事故率と他の身体能力テストを比較します。

包含基準:
最初の目的 (横断的研究) については、次のとおりです。
- 米国糖尿病協会の 2022 年の基準を使用して診断された 2 型糖尿病患者
- HbA1c ≧ 7.0 かつ ≤ 9%
- 年齢 60 歳以上 80 歳以下
2 番目の目的 (縦断的研究) については、次のとおりです。
- 最初の目的で募集されたすべての患者は、英国老年学会による高齢者の転倒予防と管理に関する世界ガイドラインの基準に従って、中等度または高転倒リスクを抱えています。
除外基準:
- 急性糖尿病合併症
- 患者は筋骨格系疾患の急性期にあります。急性痛風、進行性の軽度の関節炎、関節変性による急性関節痛、坐骨神経痛、感染性関節炎などです。
- 患者は、脳卒中の後遺症(脱力感、四肢麻痺を伴う)、筋力低下、四肢障害、重度の心不全、重度の認知機能低下、精神障害など、認知力と運動能力に大きな影響を与える症状に苦しんでいます。
- 患者は募集時点までの過去 3 か月以内に病気により 1 か月以上寝たきりとなっている。
- 心血管疾患の患者:胸痛、血圧が160/100mmHg以上のコントロール不良、未治療の不整脈、うっ血性心不全の既往歴、重度の心臓弁膜症、心筋炎または心膜炎、肥大型心筋症。

実験的:介入グループ
介入グループは、監督下のオタゴ運動プログラムと 2 型糖尿病のベースライン治療を受けます。 オタゴ運動プログラムは、筋力を強化し、患者のバランスを改善することを目的としています。 トレーニング期間は 24 週間、頻度は週 3 回で、強度はパーソナル セラピスト (PT) の評価に基づいて徐々に増加します。 糖尿病のベースライン治療: 米国糖尿病協会のガイドラインに従った推奨事項。これには、高齢の 2 型糖尿病患者に推奨される食事と運動に従うための指示が含まれます。 |
OEP は、高齢者が自宅で週に 3 回実行する 17 の筋力とバランスの訓練とウォーキング プログラムで構成されています。 プログラムの処方 このプログラムは 6 か月にわたって提供されるように設計されており、最低 5 回の来院が含まれます。 最初の訪問はオタゴの 1 日目で、機能的対策のパフォーマンスが評価され、国立老人病院のリハビリテーション センターで運動が処方されます。 その後、セラピストは 8 週間にわたって 4 回 (通常は隔週) 患者を診察します。 これはオタゴの初回ボーラスとみなされます。 その後、セラピストは毎週電話で患者を追跡し、3 か月、4 か月、5 か月、6 か月ごとに訪問して、患者のトレーニング能力とコンプライアンスを再評価します。 監視: カレンダーを毎月作成して、転倒を記録できます。 カレンダーや日記を使用して、運動プログラムの遵守状況を監視できます。
