Radiosensibilisatie met een COX-2-remmer (Celecoxib), met chemoradiatie voor hoofd-halskanker
Dit is een open-label, niet-gerandomiseerde fase IB/II-studie in één instelling waarin celecoxib gelijktijdig wordt toegediend met carboplatine, paclitaxel en radiotherapie bij patiënten met lokaal gevorderd of recidiverend plaveiselcelcarcinoom van het hoofd-halsgebied.

Gedetailleerde beschrijving
De behandeling voor dit protocol bestaat uit radiotherapie, 70,2 Gy, 1,8 Gy qd, van maandag tot en met vrijdag. Celecoxib 400 mg tweemaal daags wordt tijdens de radiotherapie ingenomen, te beginnen 1 week voor de radiotherapie.
Carboplatine IV, AUC 2,0, wekelijks gedurende week 1 tot en met 7, paclitaxel 45 mg/m2, wekelijks gedurende week 1 tot en met 7, en celecoxib 400 mg tweemaal daags, voortgezet na therapie gedurende twee jaar of tot ziekteprogressie.

Beschrijving
Inclusiecriteria:
- Histologisch bewezen primair plaveiselcelcarcinoom ontstaan in de orofarynx, mondholte, hypofarynx of larynx. Patiënten met recidieven na primaire chirurgie (zonder voorgeschiedenis van radiotherapie of chemotherapie) komen ook in aanmerking.
- De patiënt heeft stadium III of IV ziekte, T3 of hoger, of N2 of hoger, niet-gemetastaseerd. Terugkerend hoeft niet te voldoen aan deze stadiëringsvereisten bij herbestemming, maar patiënten moeten niet-gemetastaseerd zijn en ofwel inoperabel zijn, medisch niet te opereren, of verdere chirurgie weigeren.
- Prestatiestatus < 2 (ECOG-schaal) bij een levensverwachting van > 12 maanden.
- Leeftijd > 19 jaar.
- De patiënt is medisch geschikt om een definitieve bestralingstherapie te ondergaan.
De patiënt heeft:
- adequate leverfunctie met bilirubine < 1,5 x bovengrens van normaal (ULN),
- transaminasen (SGOT en SGPT) kunnen tot 2,5 x ULN zijn als alkalische fosfatase < ULN is, of alkalische fosfatase kan tot 4 x ULN zijn als transaminasen < ULN zijn,
- adequate nierfunctie met serumcreatinine < 1,5 mg/dl (of geschatte creatinineklaring > 50 ml/min),
- normaal serumcalcium,
- adequate hematologische functie als: gedefinieerd door een absoluut aantal neutrofielen > 1500/ml, hematocriet > 24 % en aantal bloedplaatjes > 100.000/ml. Patiënten met een hematocrietwaarde tussen 24% en 30% dienen een transfusie of behandeling met epoëtine te ondergaan en kunnen worden ingeschreven.
- De patiënt kan een eerdere maligniteit hebben gehad, maar moet 5 jaar ziektevrij zijn voordat hij aan de studie begint. Een voorgeschiedenis van oppervlakkige niet-melanome huidkanker of in situ carcinoom van de baarmoederhals van minder dan drie jaar wordt toegestaan.
- De patiënt moet ermee instemmen om effectieve anticonceptie te gebruiken als er sprake is van voortplantingspotentieel, en de anticonceptie voortzetten gedurende ten minste 3 maanden na voltooiing van het onderzoek.
- De patiënt moet op de hoogte worden gebracht van het onderzoekskarakter van het onderzoek en moet een formulier voor geïnformeerde toestemming ondertekenen.
Uitsluitingscriteria:
- De patiënt is eerder bestraald op het hoofd en de nek. Eerdere radiotherapie voor huidkanker van het hoofd-halsgebied is toegestaan als de velden elkaar niet overlappen.
- De patiënt heeft eerder chemotherapie gekregen voor hoofd-halskanker.
- De patiënt is zwanger of geeft borstvoeding.
- Plaveiselcelcarcinoom dat ontstaat in de nasopharynx, sinussen, speekselklieren of de primaire is onbekend.
- Niet-squameuze histologieën (zoals adenoïde cystic of mucoepidermoid)
- Perifere neuropathie > Graad 2.
- Ernstige niet-kwaadaardige ziekte (bijv. congestief hartfalen, ongecontroleerd atriumfibrilleren, actieve hepatitis, nierfalen of niertransplantatie).
- Sclerodermie of actieve bindweefselaandoening (Lupus)
- Allergie voor celecoxib, sulfonamiden of andere NSAID's
- Elke onderliggende psychologische aandoening die het begrijpen en geven van geïnformeerde toestemming zou verhinderen.
- Grote operatie < 3 weken voor aanvang van de studie

De respons wordt bepaald door alleen lokale controle, lokale en metastasen op afstand, alleen metastasen op afstand, tweede primaire en chirurgische berging.
