Voor arm A zullen 15 patiënten met resectabele MPM worden ingeschreven en preoperatief nivolumab, 240 mg IV, krijgen op dag -42, -28 en dag -14 (+/- twee dagen voor elk tijdstip) voorafgaand aan de geplande operatie op dag 0 (om voor het plannen van een operatie kan plaatsvinden tussen dag -3 en dag +10).
Na volledige opbouw naar arm A zullen 15 patiënten met resectabele MPM worden ingeschreven en preoperatief nivolumab, 3 mg/kg IV, krijgen op dag -42, -28 en dag -14 (+/- twee dagen voor elk tijdstip) + ipilimumab 1 mg /kg IV op dag -42 voorafgaand aan de geplande operatie op dag 0 (om planning mogelijk te maken kan de operatie plaatsvinden tussen dag -3 en dag +10).

Beschrijving
Inclusiecriteria:
- Mannen en vrouwen ≥ 18 jaar
- Primaire tumor vatbaar voor veilige onderzoeksbiopsie. Een tumorbiopsie is vereist voor deelname aan de studie.
Histologisch bewezen epitheliale of bifasische MPM
- Diagnostische kernbiopsiespecimens moeten worden beoordeeld door faculteitspatholoog bij SKCC, MDACC of UMGCCC.
- Ofwel een met formaline gefixeerd paraffineblok waarvan door een patholoog is bevestigd dat het een tumor bevat, ofwel minimaal twintig 5-micron weefselcoupes (glaasjes) van een tumorbiopsiemonster moeten beschikbaar zijn voor biomarkerevaluatie (onderzoekspatholoog moet beoordelen of de bemonstering adequaat is). Dit kan worden verkregen uit gearchiveerde weefsels, indien nodig, of uit een nieuwe biopsie indien nodig.
- Stadium I-III en wordt geacht mogelijk chirurgisch reseceerbaar te zijn, zoals beoordeeld door de facultaire chirurg van SKCC, MDACC of UMGCCC
- ECOG-prestatiestatus 0-1
Adequate orgaanfunctie als volgt:
- Leukocyten ≥ 2.000/mm3
- Absoluut aantal neutrofielen (ANC) ≥ 1000/mm3
- Aantal bloedplaatjes ≥ 100.000/mm3
- Hemoglobine ≥ 9 g/Dl
- Creatinine ≤ 1,5 x ULN of creatinineklaring (CrCl) ≥ 40 ml/min (bij gebruik van onderstaande Cockcroft-Gault-formule):
Vrouwelijke CrCl = (140 - leeftijd in jaren) x gewicht in kg x 0,85 72 x serumcreatinine in mg/dL

- Totaal bilirubine ≤ 1,5 x institutionele ULN (behalve proefpersonen met het syndroom van Gilbert, die totaal bilirubine < 3,0 mg/dl kunnen hebben)
- AST(SGOT), ALT(SGPT) en alkalische fosfatase ≤ 3 keer de institutionele bovengrens van normaal
Proefpersonen moeten voldoende longfunctie hebben om chirurgische resectie mogelijk te maken, bepaald door pre-inschrijving longfunctietesten om DLCO op te nemen
- De effecten van nivolumab op de zich ontwikkelende menselijke foetus zijn niet bekend. Om deze reden moeten vrouwen in de vruchtbare leeftijd (WOCBP) en mannen overeenkomen om adequate anticonceptie te gebruiken (hormonale of barrièremethode voor anticonceptie; onthouding) voorafgaand aan deelname aan het onderzoek en voor de duur van deelname aan het onderzoek en tot 23 weken daarna. de laatste dosis nivolumab. Als een vrouw zwanger wordt of vermoedt dat ze zwanger is terwijl zij of haar partner aan dit onderzoek deelneemt, dient zij haar behandelend arts hiervan onmiddellijk op de hoogte te stellen. Seksueel actieve, vruchtbare mannen moeten tot 31 weken na de laatste dosis nivolumab effectieve barrière-anticonceptie gebruiken als hun partners WOCBP zijn. WOCBP moet binnen twee weken na registratie een negatieve zwangerschapstest in serum of urine hebben (minimale gevoeligheid 25 IU/L of equivalente HCG-eenheden). Vrouwen mogen geen borstvoeding geven.
- De patiënt begrijpt het onderzoeksregime, de vereisten, risico's en ongemakken en kan en wil het formulier voor geïnformeerde toestemming ondertekenen. Vrijwillig ondertekend en gedateerd IRB/IEC-goedgekeurd schriftelijk geïnformeerd toestemmingsformulier in overeenstemming met regelgevende en institutionele richtlijnen moet worden verkregen vóór de uitvoering van protocolgerelateerde procedures die geen deel uitmaken van de normale patiëntenzorg. Proefpersonen moeten bekwaam zijn om AE's te melden, het doseringsschema van medicijnen te begrijpen en medicijnen te gebruiken om AE's onder controle te houden.
Uitsluitingscriteria:
- Stadium I-III ziekte, maar beschouwd als inoperabel, een slechte chirurgische kandidaat of ongeschikt voor onderzoekstherapie zoals beoordeeld door onderzoeksonderzoekers
- Zuivere sarcomatoïde histologie
- Proefpersonen worden uitgesloten als ze een actieve, bekende of vermoede auto-immuunziekte hebben. Proefpersonen mogen zich inschrijven als ze vitiligo, diabetes mellitus type I, resterende hypothyreoïdie als gevolg van een auto-immuunziekte hebben die alleen hormoonvervanging vereist, psoriasis die geen systemische behandeling vereist, of aandoeningen die naar verwachting niet zullen terugkeren bij afwezigheid van een externe trigger.
- Proefpersonen worden uitgesloten als ze een aandoening hebben die een systemische behandeling met ofwel corticosteroïden (> 10 mg dagelijkse prednison-equivalenten) of andere immunosuppressieve medicatie vereist binnen 14 dagen na toediening van het onderzoeksgeneesmiddel. Geïnhaleerde of topische steroïden en bijniervervangende doses > 10 mg dagelijkse prednison-equivalenten zijn toegestaan bij afwezigheid van een actieve auto-immuunziekte. Aangezien er potentieel is voor levertoxiciteit met nivolumab of nivolumab/ipilimumab-combinaties, moeten geneesmiddelen met aanleg voor hepatotoxiciteit met voorzichtigheid worden gebruikt bij patiënten die worden behandeld met een nivolumab-bevattend regime.
- Toediening van chemotherapie of een andere kankertherapie in de preoperatieve periode.
- Proefpersonen met actieve gelijktijdige maligniteiten zijn uitgesloten, d.w.z. kankers anders dan MPM (behalve niet-melanome huidkankers, cervicale dysplasie en in situ kankers van blaas, maag, borst, karteldarm en baarmoederhals).
- Proefpersonen met een voorgeschiedenis van symptomatische interstitiële longziekte.
- Actieve systemische infectie die therapie vereist, evenals positieve tests voor hepatitis B-oppervlakteantigeen of hepatitis C-antilichaam.
- Bekende positieve geschiedenis of positieve test voor humaan immunodeficiëntievirus of verworven immunodeficiëntiesyndroom (aids).
- Geschiedenis van allergie om medicijncomponenten te bestuderen.
- Vrouwen die zwanger zijn of borstvoeding geven.
- Mannen met vrouwelijke partners (WOCBP) die geen anticonceptie willen gebruiken
- Voorafgaande therapie met een anti-PD1-, anti-PD-L1-, anti-PD-L2- of anti-CTLA-4-antilichaam (of een ander antilichaam gericht op T-cel co-regulerende routes).
- Voorgeschiedenis van een andere aandoening die de start van therapieën tegen tumornecrosefactor-alfa (TNFα) of andere immunosuppressieve medicatie tijdens het onderzoek kan vereisen
- Onderliggende medische aandoeningen die, naar de mening van de onderzoeker, de toediening van het onderzoeksgeneesmiddel gevaarlijk maken of de interpretatie van toxiciteit of bijwerkingen vertroebelen.
- Gevangenen of proefpersonen die onvrijwillig worden opgesloten of gedwongen worden vastgehouden voor behandeling van een psychiatrische of lichamelijke (bijv. besmettelijke ziekte) ziekte.
