De primaire preventie van cardiovasculaire (CV) gebeurtenissen is vaak intenser bij personen met een lager risico en vice versa (de zogenaamde "risico-behandelingsparadox"), deels omdat ze zich niet bewust zijn van het CV-risico van elke persoon, van hun voorkeuren voor preventieve interventies, en van hun haalbaarheid in het dagelijks leven van elke persoon. Richtlijnen voor klinische praktijk bevelen aan dat clinici en patiënten samenwerken om tot een effectief en haalbaar preventieplan te komen dat congruent is met ieders CV-risico en geïnformeerde voorkeuren, een proces dat gedeelde besluitvorming (SDM) wordt genoemd. Ondanks de beschikbaarheid van een innovatieve en effectieve tool die het CV-risico inschat, de impact en kenmerken van beschikbare leefstijl- en farmacologische preventieve interventies laat zien, en zo CV-behandelingsdiscussies tussen clinici en patiënten kan vergemakkelijken, komt dit type SDM in de praktijk niet routinematig voor.

De uitdaging is daarom om strategieën te identificeren om de acceptatie van dit type SDM in klinische praktijken in de echte wereld te vergroten. Deze 4-jarige studie - voorgesteld door een multidisciplinair team met expertise in preventieve cardiologie, SDM en implementatiewetenschap - heeft tot doel een SDM-tool (de CV Prevention Choice-tool) te integreren in de eerstelijnszorgpraktijken van drie verschillende gezondheidszorgsystemen in de VS. en bestudeer zowel de tool als de op maat gemaakte strategieën die de acceptatie en het routinematige gebruik ervan bevorderen. De studie zal gebruik maken van een gemengde methode, hybride implementatie-effectiviteit (Type III) step-wedge geclusterd gerandomiseerd proefontwerp om te bepalen:

De onderzoekers veronderstellen dat inspanningen om lokale behoeften te beoordelen en deze te gebruiken om op maat gemaakte implementatiebenaderingen te ontwikkelen, een grotere acceptatie van SDM in de praktijk zullen bevorderen. Ze veronderstellen verder dat individuele preventieve zorgplannen in overeenstemming zullen zijn met het geschatte risico wanneer clinici de SDM-tool gebruiken. Inclusiecriteria:

  • Clinicus-deelnemers: Alle clinici die zijn aangesloten bij een deelnemende eerstelijnszorgpraktijk en zorg voor volwassen patiënten die in aanmerking komen voor CV-preventie, worden uitgenodigd om deel te nemen.
  • Patiënt-deelnemers: Volwassen patiënten (leeftijd 40-75 jaar) met of zonder diabetes die geen atherotrombotisch klinisch voorval hebben meegemaakt en die preventieve zorg krijgen bij een deelnemende eerstelijnszorgpraktijk, komen in aanmerking voor deelname.

Uitsluitingscriteria:

- Personen die geen Engels spreken of een cognitieve stoornis hebben die van invloed zou kunnen zijn op hun vermogen om toestemming te geven voor deelname aan het onderzoek, worden niet uitgenodigd om deel te nemen.

Effectiveness: The Predicted Marginal Probability of Statin Use is Concordant With Estimated Cardiovascular Risk

Patient risk-concordance was assessed using all of the patient's encounter level data. Statin prescription status was captured in the EHR up to 30 days after the encounter (i.e., the patient-clinician clinical visit) and their estimated 10-year risk of developing a first atherosclerotic cardiovascular disease (ASCVD) event, calculated using the Pooled Cohort Equation and data from the EHR including: age, race, total cholesterol, HDL cholesterol, systolic blood pressure, smoking status, diabetes status, and whether the individual is receiving treatment for high blood pressure (if systolic blood pressure is greater than 120 mmHg). Risk concordance was defined as having a statin prescription in groups where ASCVD risk was > or equal to 7.5% and not having a statin prescription if it was < 7.5%. The least square means, using the patient's encounter level data, provides a marginal probability of the proportion of patients that would have a statin prescription within the risk*arm/group.
