Achtergrond:
- Last: COVID -19, voor het eerst gemeld in Wuhan, China, treft de wereld hard als een pandemie door zich te verspreiden naar 213 landen. Bangladesh wordt ook geconfronteerd met zijn ravage met de dood van 3000 van de 229.185 gevallen op de 141e dag. De meeste zijn milde gevallen die zich presenteren met een griepachtige ziekte met verwacht herstel. Volgens de Wereldgezondheidsorganisatie wordt het totale wereldwijde sterftecijfer momenteel geschat op 2,9%. Patiënten met een ernstige of kritieke ziekte die zich presenteren met ernstige ademnood, ARDS of shock lijden echter het meest met een mortaliteit van 49,0-61,5%. Studies suggereren dat er milde of ernstige cytokinestormen zijn bij ernstige patiënten, wat een belangrijke doodsoorzaak is. Een overdreven en ongecontroleerde afgifte van pro-inflammatoire mediatoren door een overdreven geactiveerd immuunsysteem wordt cytokinestorm (CS) genoemd. Studies die aantonen dat er mogelijk een belangrijke relatie bestaat tussen SARS-CoV-2 en darmmicrobiota, als een marker om ARDS te voorspellen, kunnen de ernst van de ziekte en de uitkomsten van COVID-19 bevestigen.
- Kennishiaat: Cytokine storm (CS) speelt een belangrijke rol bij de hemodynamische beledigingen die worden waargenomen bij zeer zieke COVID-19-patiënten. In Bangladesh is er geen onderzoek uitgevoerd om het niveau van cytokines en de darmmicrobiota te evalueren bij COVID-19-patiënten met verschillende ernstniveaus en hun mogelijke correlatie met COVID-19.
- Relevantie: het onderzoekt de rol van CS en darmmicrobiota bij de ernst van de ziekte van COVID-19. De resultaten van het onderzoek zullen dus onze kennis en begrip van het probleem verbeteren, en daardoor hun oplossingen vinden.
Doelstellingen:
Om de last van 'Cytokine Storm' en de correlatie ervan met de omvang van de ernst van de COVID-19-ziekte bij volwassenen in Bangladesh te schatten.
methoden:
Dit wordt een prospectieve observationele studie bij volwassenen van ≥18 jaar oud met COVID-19 met milde/matige/ernstige/kritieke symptomen. De onderzoekers zullen hun cytokineniveaus (IL 6, TNF-Alpha en IL1 Beta) meten en de darmmicrobiota onderzoeken naast reguliere laboratoriumtests op verschillende tijdstippen van ziekte.

Inclusiecriteria:
- Volwassenen van ≥18 jaar,
- Geïnformeerde toestemming verkregen voor deelname,
- Een milde, matige of ernstige (±kritieke gevallen) RT-PCR bevestigde COVID-19-gevallen,
- RT-PCR-negatieve gezonde vrijwilligers.

COVID-19 positief door RT-PCR
gespecificeerd aantal COVID-19-positieve patiënten zal worden opgevolgd van opname tot uitkomst (ontslag/overlijden/verwijzing).
Bloedmonsters worden op verschillende tijdstippen getest; cytokines en ontlastingsmicrobiota zullen aan het einde van de studie worden getest en we zullen de onderzoeksresultaten analyseren.
