Neoadjuvante chemotherapie (NACT) wordt vaak gebruikt voor de behandeling van patiënten met lokaal gevorderde borstkanker, grote tumor maar operabele borstkanker of bewezen lymfekliermetastasen. Het voordeel van NACT is dat het de tumorgrootte kan verkleinen en het aantal borstsparende operaties kan verhogen, en dat het informatie verschaft over de respons op het geneesmiddel door de veranderingen in de tumorgrootte te beoordelen. De pathologische volledige respons wordt consequent in verband gebracht met goede langetermijnresultaten, maar wordt slechts in 10%-20% van de gevallen bereikt. Patiënten die een pathologische complete respons bereiken, hebben een betere prognose dan degenen die dat niet doen. Hogere percentages pathologische complete respons kunnen worden bereikt wanneer wordt gekozen voor bepaalde subtypes van borstkanker en behandelingsregimes.
Borstkanker is een heterogene ziekte die kan worden onderverdeeld in verschillende subtypen door middel van immunohistochemische markerexpressie of genexpressie-arraygegevens. Tijdens de bijeenkomst in St. Gallen werd een nieuw surrogaat intrinsiek subtype voorgesteld om luminale A, luminale B (HER2-/HER2+), HER2-verrijkte en triple-negatieve ziekte te scheiden. Evenzo kunnen tumoren verschillende prognoses hebben op basis van hun moleculaire subtypes. Het luminale A-subtype heeft een betere prognose dan de andere subtypes en de triple-negatieve subgroep heeft de slechtste prognose . Aan de andere kant is het triple-negatieve subtype gevoeliger voor chemotherapie dan luminale A- en B-borstkanker.
De huidige technieken die beschikbaar zijn voor het monitoren van de respons op NACT zijn positronemissietomografie (PET), echografie, mammografie, magnetische resonantiebeeldvorming (MRI) en shear wave elastography (SWE). Conventionele echografie en mammografie hebben een slechte betrouwbaarheid bij het evalueren van de grootte van de resterende tumor na chemotherapie. SWE is een recent ontwikkelde goedkope beeldvormingstechniek voor het meten van weefselstijfheid op een niet-invasieve en kwantitatieve manier met hoge reproduceerbaarheid. Er is aangetoond dat weefselstijfheid significant gecorreleerd is met tumorgroei, aangezien de ontwikkeling en progressie van kanker een uitgebreide reorganisatie van de extracellulaire matrix (ECM) vereisen. Verhoogde afzetting van collageen en andere ECM-moleculen verhoogt de stijfheid van tumorale stroma. Veranderingen in tumorstijfheid waren significant groter bij patiënten die goed op NACT reageerden dan bij patiënten die resistent waren tegen NACT. Borstkankerstijfheid vóór en na de behandeling verkregen uit SWE was significant gecorreleerd met de aanwezigheid van resterende kanker. Een studie toonde aan dat de SWE-stijfheid gemeten na 3 cycli van NACT en veranderingen in stijfheid ten opzichte van de uitgangswaarde sterk geassocieerd waren met pCR na 6 cycli. De combinatie van de SWE na de behandeling en grijswaarden-echografie is ook veelbelovend gebleken voor de identificatie van residuele ziekte aan het einde van de behandeling en dus de respons op NACT, met vergelijkbare nauwkeurigheid gevonden bij beoordeling door MRI .
Er zijn verschillende histopathologische classificaties beschikbaar om de tumorrespons op NACT te categoriseren. De Miller-Payne-grading (MPG) en Residual Disease in Breast and Nodes (RDBN) zijn systemen om de pathologische respons van NACT te beoordelen. MPG biedt een vijfstapsschaal op basis van de cellulariteit van de tumor in het excisie-/mastectomiespecimen in vergelijking met de kernbiopsie vóór de behandeling, en wel als volgt: graad 1, geen vermindering van de algehele cellulariteit; graad 2, klein (<30%) verlies van cellulariteit; graad 3, geschatte 30%-90% vermindering van tumorcellen; graad 4, >90% verlies van tumorcellen; en graad 5, geen invasief carcinoom (IC); ductaal carcinoom in situ kan aanwezig zijn. RDBN gebruikt de volgende formule: niveau 1, pathologische complete respons in borst en klieren, zonder of met carcinoma in situ, en niveaus 2-4, restziekte in drie verschillende hoeveelheden, berekend als 0,2 (restgrootte borsttumor in cm) + de index voor de betrokken knooppunten (0 voor geen positieve knooppunten, 1 voor 1-4 knooppunten, 2 voor 5-7 knooppunten, 3 voor ≥8 knooppunten) + de Scarff-Bloom-Richardson-graad (1, 2 of 3), die houdt rekening met de tumorgrootte, het lymfeklierstadium en de histologische graad om de respons te bepalen.

Inclusiecriteria:
• Histologisch bevestigde infiltrerende borstkanker
- Stadium II of stadium III ziekte. De klierstatus moet worden onderzocht door middel van echografie, fijne naaldaspiratie, schildwachtklierbiopsie of FDG-PET-scan.
- Leeftijd ≥18
- Prestatiestatus Eastern Cooperative Oncology Group ≤1
- Adequate beenmergfunctie (ANC >1,5 x 109/l, bloedplaatjes >100 x 109/l)
- Adequate leverfunctie (ALAT, ASAT en bilirubine <2,5 keer bovengrens van normaal)
- Adequate nierfunctie (creatinineklaring >50 ml/min)
- LVEF ≥50% gemeten door echocardiografie of MUGA
- Afwezigheid van enige psychologische, familiale, sociologische of geografische aandoening die mogelijk de naleving van het onderzoeksprotocol en het follow-upschema belemmert
- Afwezigheid van een medische aandoening waardoor de patiënt een ongewoon risico loopt.
- Ondertekende schriftelijke geïnformeerde toestemming
Uitsluitingscriteria:
• eerdere radiotherapie of chemotherapie
- Andere maligniteit behalve carcinoma in situ, tenzij de andere maligniteit ≥5 jaar geleden curatief is behandeld zonder gebruik van chemotherapie of bestraling.
- Huidige zwangerschap of borstvoeding. Vrouwen in de vruchtbare leeftijd moeten adequate anticonceptie gebruiken.
- Bewijs van metastasen op afstand. Evaluatie van de aanwezigheid van metastasen op afstand kan bestaan uit röntgenfoto's van de borst, echografie van de lever, isotoop-botscan, CT-scan van borst en buik en/of FDG-PET-scan, volgens lokale procedures.
- Bewijs van bilaterale infiltrerende borstkanker. Evaluatie van de aanwezigheid van bilaterale infiltrerende borstkanker kan bestaan uit mammografie, echografie van de borst en/of MRI-borst.
- Gelijktijdige behandeling tegen kanker of een ander onderzoeksgeneesmiddel.
