Szczegółowy opis
Guzy komórek warstwy ziarnistej jajnika (OvGCT) stanowią 5-7% wszystkich raków jajnika (~1,5 do 2 tys. nowo zdiagnozowanych pacjentów rocznie w Stanach Zjednoczonych) i są najczęstszym podtypem guzów sznurów płciowych jajnika (70%). Oczekuje się, że leczenie guzów z komórek warstwy ziarnistej za pomocą nirogacestatu hamuje indukowaną przez Notch proliferację komórek warstwy ziarnistej.
Jest to wieloośrodkowe, jednoramienne, otwarte badanie fazy 2 dotyczące leczenia, mające na celu określenie skuteczności, bezpieczeństwa, tolerancji i farmakokinetyki nirogacestatu u dorosłych uczestników z nawracającym/opornym na leczenie OvGCT. Uczestnicy będą kontynuować leczenie do czasu wystąpienia progresji choroby określonej na podstawie kryteriów oceny odpowiedzi w guzach litych (RECIST) v1.1 lub wystąpienia nieakceptowalnej toksyczności.

Kluczowe kryteria włączenia:
- Ma potwierdzony histologicznie nawrót guza jajnika z komórek warstwy ziarnistej typu dorosłego przed pierwszą dawką badanego leku
- Mają udokumentowane radiologiczne dowody nawrotu choroby po co najmniej jednej terapii ogólnoustrojowej, która nie jest podatna na operację lub radioterapię i mają mierzalną chorobę według kryteriów RECIST v1.1
- Mieć odpowiednią czynność szpiku kostnego, nerek i wątroby, zgodnie z wynikami badań laboratoryjnych
Kluczowe kryteria wykluczenia:
- Ma oznaki niedrożności jelit wymagającej żywienia pozajelitowego, zespół złego wchłaniania lub wcześniej istniejące schorzenia żołądkowo-jelitowe, które mogą upośledzać wchłanianie nirogacestatu
- Miał poważny incydent sercowo-naczyniowy lub zakrzepowo-zatorowy w ciągu 6 miesięcy od podpisania świadomej zgody
- Ma nieprawidłowy odstęp QT podczas badania przesiewowego lub ma wrodzony lub nabyty zespół długiego QT lub dodatkowe czynniki ryzyka Torsades de Pointes w wywiadzie
- Ma obecną lub przewlekłą historię chorób wątroby lub znanych nieprawidłowości wątroby lub dróg żółciowych
