Este estudo randomizado, duplo-cego, controlado por placebo pretende recrutar 94 sarcopenias qualificadas com síndrome de baço-astenia. O estudo será realizado no Segundo Hospital Popular Afiliado à Universidade de Medicina Tradicional Chinesa de Fujian, na Província de Fujian, China. Todos os indivíduos serão solicitados a preencher informações de linha de base antes da inscrição para determinar a situação geral dos pacientes antes da randomização. Posteriormente, os indivíduos foram divididos aleatoriamente em dois grupos em uma proporção de 1:1: o grupo da pílula de inhame (n = 47) e o grupo do placebo (n = 47), as sequências aleatórias foram fornecidas por especialistas em estatística que não estiveram envolvidos durante o estudo . Entre eles, a pílula de inhame é uma pílula de fórmula de medicina tradicional chinesa composta de inhame, arte branca e ginseng, e o placebo é composto principalmente de amido de milho, que é totalmente consistente com a aparência, forma, tamanho, cor, especificação, embalagem e rótulo da pílula de inhame. As formulações de medicina chinesa dos dois grupos foram fornecidas na forma de comprimido e entrega de sopa de arroz (uma vez ao dia, 7 dias/semana, um total de 24 semanas). Além disso, para melhorar a adesão do paciente durante o período de intervenção, os pacientes de ambos os grupos foram obrigados a assistir a uma palestra de educação em saúde uma vez por mês, e ambos os grupos foram acompanhados por 24 semanas após a intervenção.
Os quatro momentos foram comparados antes da intervenção (0 semanas), durante a intervenção (após o final da 12ª semana), ao final da intervenção (após o final da 24ª semana) e durante o seguimento (após o final da 48ª semana). O índice principal foram as alterações na massa muscular (24 semanas menos a linha de base) e o índice secundário foram as alterações na força muscular (força de preensão; teste de levantar da cadeira 5 vezes); Função física (por ex. teste de caminhada de 6 min, velocidade da marcha, time-up-go-test); a função cognitiva, a qualidade de vida e a eficácia clínica da medicina tradicional chinesa foram investigadas com os formulários de questionários para explorar a eficácia clínica das pílulas de inhame em pacientes com síndrome de astenia do baço. Em segundo lugar, imagens espectrais de infravermelho próximo (fNIRS) foram usadas para explorar se a melhora da eficácia clínica causada pelas pílulas de inhame após 24 semanas de intervenção estava relacionada às mudanças hemodinâmicas no córtex pré-frontal e na região cerebral do córtex motor com base na tarefa dupla de cognição motora. Posteriormente, o sequenciamento de 16S-rRNA e a metabolômica não direcionada foram usados para explorar se a melhora da eficácia clínica causada pelas pílulas de Yam após 24 semanas de intervenção estava relacionada à regulação da flora intestinal e seus metabólitos, para finalmente esclarecer o crosstalk músculo-cérebro durante o processo de intervenção e explorar o seu potencial mecanismo de ação.

Critério de inclusão:
- Idade ≥60 anos, de acordo com os critérios diagnósticos da AWGS para sarcopenia e de acordo com o padrão dialético da síndrome de deficiência do baço da MTC;
- Ser capaz de compreender e cooperar com o teste e assinar voluntariamente o consentimento informado.
Critério de exclusão:
- Disfunção motora causada por doenças graves do sistema nervoso, doenças do sistema músculo-esquelético, osteoporose, osteoartrite grave;
- doença cardíaca, pulmonar ou mental grave, doença endócrina ou metabólica descontrolada, ou função hepática ou renal grave (como cirrose, histórico de cálculos renais, insuficiência renal ou diálise);
- Incapacidade de se comunicar adequadamente com o pesquisador devido à fala ou deficiência auditiva não corrigida;
- Comprometimento cognitivo grave (exame breve do estado mental: pontuação no MEEM < 24 pontos), incapaz de compreender o conteúdo incluído no questionário e na escala;
- Uso de suplementos de hormônio do crescimento, estrogênio, progesterona ou testosterona por quase 3 meses ou deficiências nutricionais graves;
- Participar de outros ensaios ou ensaios de suplementos de medicina chinesa nos últimos três meses.

Change From Baseline in Appendicular Skeletal Muscle Mass Index (ASMI)

Appendicular skeletal muscle mass (ASM) is assessed using dual-energy X-ray absorptiometry (DXA).
ASM is calculated as the sum of lean soft tissue mass of the arms and legs.
Appendicular skeletal muscle mass index (ASMI) is calculated as ASM divided by height squared (kg/m²).
Changes in ASMI from baseline are assessed at week 12, week 24, and 2 years after completion of the 24-week intervention.The primary efficacy endpoint is the change in ASMI from baseline to week 24.
