Používání elektronických systémů dodávání nikotinu (ENDS nebo e-cigarety) dosáhlo epidemické úrovně mezi mladými lidmi ve Spojených státech (USA). ENDS uvolňují toxické látky včetně nikotinu, který nevratně ovlivňuje vyvíjející se mozek a vede k závislosti a zvýšenému riziku zahájení kouření cigaret. Důkazy naznačují, že mladí lidé užívající ENDS pravděpodobně urychlí užívání a stanou se závislými na nikotinu. Některé z nejoblíbenějších KONCŮ mezi mládeží (např. JUUL s 5% nikotinovou tekutinou) dodávají v jedné kazetě dávku nikotinu rovnající se celému balení cigaret a mnoho uživatelů si není vědomo této značné expozice a souvisejícího rizika závislosti. Na druhé straně byla redukce nikotinu (NR) považována americkým Úřadem pro kontrolu potravin a léčiv (FDA) za důležitou strategii pro omezení návykovosti a užívání tabákových výrobků.
Nyní, když FDA reguluje ENDS podle „pravidla uvažování“, lze o takové strategii u těchto nově vznikajících tabákových výrobků uvažovat. Zkoušejícím však stále chybí důkazy o očekávaných účincích řady úrovní NR na uživatele ENDS v různých fázích trajektorie užívání. Pomocí klinických a analytických laboratorních metod plánují výzkumníci porovnat mezi 5% uživateli ENDS nikotinu účinek částečné (3%) nebo celkové (bez nikotinu, 0%) NR na závislost uživatelů, spokojenost, klinický (např. BP, oxymetrie, funkce plic, symptomy), nadýmání a expozice toxickým látkám. Vyšetřovatelé naberou současné uživatele ENDS (n=120; 21-35 let) pro 2X2 v rámci křížové laboratorní studie. Stavy nikotinu (5 %, 3 %) nebo (0 %, 5 %) x 2krát (pre-post) jsou faktory v rámci subjektu.
Proto je studie rozdělena na 2 části, přičemž první část bude 2x2 porovnávat NJOY 5% a 3% pro testování účinků částečné redukce nikotinu a druhá část bude používat NIDA Standardized Research Electronic Cigarette (SREC) pro Klinický výzkum 5 % a 0 % (placebo), k testování účinku celkové redukce s použitím stejných lusků a šťáv od stejného výrobce.
Vyšetřovatelé předpokládají, že NR bude spojena s menší spokojeností, potlačením abstinenčních příznaků a záměrem užívat a že takový účinek bude celkově výraznější ve srovnání s částečnou NR a mezi uživateli s vysokou závislostí ve srovnání s uživateli s nízkou závislostí. Navrhované studie mají za cíl odpovědět na 3 hlavní regulační otázky: 1) je NR slibnou regulační možností ke snížení návykovosti a užívání ENDS; 2) jaký je vliv částečného vs. celkového NR na kompenzační nadýmání a expozici nikotinu a respiračním toxickým látkám a 3) jaký je účinek NR na uživatele ENDS v různých fázích jejich užívání. Společně tyto studie poskytnou přímé a standardizované důkazy o potenciálu předpisů NR omezit návykovost a používání ENDS a pomohou předpovědět jakékoli potenciální vedlejší účinky NR na vystavení uživatelů ENDS škodlivým toxickým látkám.

Popis
Kritéria pro zařazení:
- Obecně zdraví jedinci (zjištěno fyzikálním vyšetřením).
- Věk 21-35 let.
- Je ochoten poskytnout informovaný souhlas.
- Je ochoten zúčastnit se laboratoře, jak to vyžaduje protokol studie.
- Uživatelé elektronických cigaret (definováno jako používání elektronických cigaret denně nebo příležitostně v posledních 30 dnech)
- Před každým sezením se 12 hodin zdržujte používání elektronických cigaret
Kritéria vyloučení:
- Uveďte pravidelné kouření cigaret (> 5 cigaret měsíčně za poslední rok).
- Nahlaste pravidelné užívání jakéhokoli jiného tabákového/nikotinového výrobku (např. vodní dýmky, dýmky, doutníky) v posledním roce.
- Ženy, které kojí nebo mají pozitivní těhotenský test (rozborem moči při screeningu).
- Jednotlivci s chronickým onemocněním nebo současným psychiatrickým onemocněním v anamnéze, kterou sami uvedli.
- Jedinci s anamnézou nebo aktivním kardiovaskulárním onemocněním, nízkým nebo vysokým krevním tlakem, záchvaty a pravidelným užíváním léků na předpis (jiných než vitamínů nebo antikoncepce).
- Jedinci, kteří hlásí současné kouření/vaping THC (marihuany).
- Jednotlivci, kteří nahlásí používání nekomerčních (tj. pouličních) tekutin nebo produktů pro elektronické cigarety
- Jednotlivci, kteří hlásí aktuální příznaky související s EVALI nebo COVID-19 (tj. kašel, dušnost, bolest na hrudi, nevolnost, zvracení, bolest břicha, průjem, horečka, zimnice nebo ztráta hmotnosti)
- Jednotlivci, kteří byli nebo byli v posledních 14 dnech vystaveni COVID-19.

This aim elucidates how compensatory puffing behavior in response to NR affects exposure to pulmonary toxicants.
The analytical assessments were conducted at the AUB Aerosol Research Laboratory (ARL) using the Aerosol Lab Vaping Instrument (ALVIN).
The ALVIN is programmed to playback participants' puffing behavior recorded during their two sessions (for both partial and total nicotine reduction studies) to generate and sample the e-cigarette aerosol for analysis.
The ARL is equipped with gas chromatographs with a flame ionization detector and a mass spectrometer (GC-FID, GC-MS), as well as and High-performance liquid chromatography Mass Spectrometry (HPLC-MS) for aldehydes analysis.
ARL optimized analytical methods have been used in several NIH-funded studies of ENDS nicotine and aldehydes emissions.

Nicotine withdrawal symptoms were measured using the Minnesota Nicotine Withdrawal Scale (MNWS) before and after each vaping session.
MNWS evaluates 11 withdrawal-related symptoms, including depressed mood, irritability, restlessness, hunger, and difficulty concentrating, using scale from "0" (not at all) to "100" (extremely).
For each session and condition, change scores were calculated by subtracting pre-session from post-session scores for each individual item

The Tiffany-Drobes Questionnaire of Smoking Urges-Brief (QSU-Brief) is a 10-item self-report measure of cigarette craving and urge to smoke. It consists of two subscales representing distinct dimensions of craving: Factor 1 (Desire and Intention to Smoke): assesses desire, intention, and anticipation of pleasure associated with smoking. Factor 2 (Anticipation of Relief from Negative Affect and Withdrawal): assesses expectations that smoking will relieve withdrawal symptoms or negative affect. Each item is rated on a 7-point Likert scale ranging from 1 = strongly disagree to 7 = strongly agree. Subscale scores were calculated by averaging the items within each factor. Higher scores on either factor indicate greater smoking urge or craving, representing a worse outcome, whereas lower scores indicate lower craving or urge to smoke, representing a better outcome. Pre- to post-session change scores were calculated as pre-session score minus post-session score for each factor. |

Modified Cigarette Evaluation Questionnaire (mCEQ) is used to assess subjective effects associated with ENDS use. The 11-item questionnaire evaluates satisfaction, psychological reward, enjoyment of respiratory tract sensations, craving reduction and aversion. Individual item scores were reported (not subscales). Each item was rated using 7-point Likert scale; 1 = not at all to 7 = extremely. For all items, higher scores indicate greater endorsement of the measured subjective experience, while lower scores indicate less endorsement of that experience. The interpretation depends on the construct being measured: For items assessing satisfaction, enjoyment, psychological reward, sensory experiences, and craving reduction, higher scores indicate greater product reinforcement or abuse liability. For items assessing aversion, higher scores indicate greater unpleasant effects or product aversiveness. |
