En klinisk undersøgelse med brug af skadesassocieret molekylært mønster og tilsyneladende diffusionskoefficient til at evaluere effektiviteten af biologiske lægemidler til behandling af spondyloarthritis
Spondyloarthritis (SpA) er en af de potentielt invaliderende inflammatoriske sygdomme, der påvirker hele kroppen, primært belaster sacroiliacaleddene og rygsøjlen.
Det rammer for det meste unge og midaldrende voksne.
SpA kan klassificeres til ikke-radiografisk aksial SpA (nr-axSpA) og radiografisk axSpA (r-axSpA).
Sidstnævnte er ankyloserende spondylitis (AS).
Nøglen til dets tidlige behandling er radiologisk påvisning og behandling af sacroiliitis.
Til dato er biologiske lægemidler det mest kraftfulde antiinflammatoriske lægemiddel.
Nyere forskning har vist, at diffusionsvægtet billeddannelse (DWI) overgår den sekvens, der anbefales af retningslinjerne, når det gælder diagnosticering af inflammation og vurdering af sygdomsaktivitet.
Foreløbig forskning udført af vores team har også vist, at tilsyneladende diffusionskoefficient (ADC) er en værdifuld billeddannende biomarkør.
Til dato er der dog ikke identificeret nogen serumfremstiller med sammenlignelig effektivitet.
Damage-Associated Molecular Pattern (DAMP), inklusive S100A8 og S100A9, højmobilitetsgruppeprotein B1 (HMGB1) og Tenascin-C (TNC), kan spille en rolle i inflammation ved at regulere TLR4/MyD88/NF-KB-signalvejene.
Nærværende undersøgelse vil inkludere 20 patienter med nr-axSpA og 20 patienter med AS.
Det vil bruge serum DAMP og ADC til at vurdere sygdomsaktivitet før og efter behandling samt ændringen i og korrelationerne af behandlingsresultater for at identificere objektive og kvantificerbare serum- og billeddannelsesmarkører, der er gavnlige i kliniske applikationer.
ADC er det primære resultat.
Hovedhypotesen er, at sygdomsaktivitet målt ved ADC vil være reduceret efter 1 års behandling fra baseline sammenlignet med før behandling ved baseline.
Undersøgelsesresultater vil indikere anvendeligheden af ADC som en objektiv indikator for sygdomsaktivitet til at vejlede terapeutiske tilgange og forbedre dosisjustering i kliniske applikationer.

Studiebefolkning
Patienter med axial SpA, som går på reumatologisk klinik på University of Hong Kong-Shenzhen Hospital, vil blive screenet fortløbende af PI, hendes uddannede sygeplejersker og/eller forskningsassistenter for at være berettiget.
Kvalificerede patienter vil blive rekrutteret til at deltage i undersøgelsen.
