Neurobehaviorale Auswirkungen vorherrschender Neurotoxine bei Kindern: Eine Kohortenstudie des Cincinnati Center for Children's Environmental Health

Diese Studie zielt darauf ab, die Auswirkungen einer geringen Exposition gegenüber vorherrschenden Neurotoxinen auf Gesundheit, Wachstum und neurologisches Verhalten bei einer repräsentativen Stichprobe von Kindern zu untersuchen. Schwangere Frauen wurden ungefähr in der 16. Schwangerschaftswoche in das Projekt aufgenommen. In der ersten Phase der Studie haben wir Kinder, die aus der Schwangerschaft hervorgegangen sind, bis zum Alter von 36 Monaten begleitet. Die zweite Phase verlängerte das Follow-up über 72 Monate. Phase 3 erweitertes Follow-up auf 8 Jahre (Bereich 7,5-10) mit umfassenden neurobehavioralen Bewertungen. Phase 4 ermöglicht eine Nachuntersuchung nach 12 Jahren (Bereich 11-13) und umfasst Maßnahmen zu Gesundheit, Wachstum und Körperzusammensetzung, Verhalten und psychischer Gesundheit sowie Neuroimaging. Um die möglichen nachteiligen Gesundheitsrisiken von Umweltchemikalien, einschließlich persistenter Schadstoffe wie PBDEs und PFCs und anderer nicht persistenter Chemikalien, auf das Neuroverhalten von Föten, Säuglingen und Kindern anzugehen, untersuchen die Forscher systematisch ihre Assoziationen mit endokriner Funktion, Kognition, Lernen und Gedächtnis, motorische Fähigkeiten, Aufmerksamkeit und Exekutivfunktion und Verhalten im Alter von 1 bis 7,5-10 Jahren. Die Forscher untersuchen auch Expositionen in verschiedenen Entwicklungsstadien (in utero in der 16. Schwangerschaftswoche, in der frühen Kindheit, im Schulalter, vor der Pubertät) unter Verwendung gespeicherter biologischer Proben und messen das neurologische Verhalten von Kindern bei 1, 2, 3, 4, 5, 8 und 12 Jahre. Diese Längsschnittstudie wird es den Forschern ermöglichen, die Dosiswirkung, Empfindlichkeitsfenster und die Persistenz der Assoziation zu bestimmen. Die Forscher untersuchen auch den Beitrag der PBDE-Exposition durch Hausstaub bei einer Untergruppe von Kindern, die über vollständige Sätze von Proben von mütterlichem Serum und Kinderserum verfügen, die bei jährlichen Besuchen zusammen mit umfangreichen Messungen des Mundverhaltens gesammelt wurden.
Hypothesen aus den vier Phasen der Studie lauten wie folgt:
- In-utero-Expositionen, die durch Umfragen (Alkohol und ETS), mütterliches und Nabelschnurblut (Blei und Quecksilber), mütterliches und Nabelschnurserum (ETS) und Urin (Pestizide) gemessen wurden, sind weniger prädiktiv für die in-utero-Effekte vorherrschender Giftstoffe, einschließlich Wahrnehmungs- und Verhaltensprobleme und Wachstum im Vergleich zu den gleichen Giftstoffen in Mekonium.
- Pränatale und postnatale Expositionen gegenüber vorherrschenden Pestiziden und ETS sind mit nachteiligen Auswirkungen auf das neurologische Verhalten und Wachstumsverzögerungen bei Kindern verbunden.
- Eine höhere Bleiexposition, gemessen während der Schwangerschaft und frühen Kindheit anhand von mütterlichem Blut, Nabelschnurblut, Mekonium und Kinderblut, wird mit niedrigeren IQ-Werten und mehr Verhaltensproblemen bei Kindern mit einem maximalen Blutbleispiegel < 5 mg/dL in Verbindung gebracht.
- Kinder im Hauptbehandlungsarm haben: ein um 2,7 mg/dL niedrigeres Blutblei, höhere IQ-Werte, eine höhere Wachstumsgeschwindigkeit und weniger Verhaltensprobleme als Kinder in der Kontrollgruppe.
- Die Bleikonzentrationen in Staub, Boden und Wasser sind bei den Wohneinheiten im Bleibehandlungsarm im Vergleich zum Verletzungskontrollarm bei Hausbesuchen nach 36 und 48 Monaten deutlich niedriger.
- Eine multifaktorielle Wohnintervention wird häusliche Verletzungen bei Kindern in der Verletzungsbehandlungsgruppe im Vergleich zu denen in der Hauptbehandlungsgruppe um 30 Prozent reduzieren.
- Pränatale und postnatale Expositionen gegenüber PBDEs und PFCs sind mit veränderten Schilddrüsenhormonspiegeln und Defiziten im neurologischen Verhalten von Säuglingen und Kindern verbunden
- Mit zunehmendem Alter des Kindes wird die PBDE-Exposition durch Haushaltsstaub zu einem stärkeren Prädiktor für die Serum-PBDE-Konzentration von Kindern als die Exposition durch Plazenta oder Muttermilch.
- Entwicklungsbedingte PBDE- und PFC-Expositionen sind mit internalisierenden Symptomen verbunden.
- Entwicklungsbedingte PBDE- und PFC-Expositionen sind mit nachteiligen Veränderungen der anatomischen Struktur, der Neurochemie, der Organisation der Bahnen der weißen Substanz und der Konnektivität neuronaler Netzwerke verbunden.
- PFAS beeinflussen die Genexpression und Funktion mehrerer biologischer Signalwege, die den Fötus/Kind auf einen „sparsamen Phänotyp“ programmieren. Dies führt zu beschleunigtem frühkindlichem Wachstum, erhöhter Fettmasse und Merkmalen des metabolischen Syndroms.

zufällige Zuweisung, um Interventionen zur Gefahrenkontrolle durch Blei zu erhalten.
Bewertung der Bleigefährdung im Haushalt.
Verringerung der Gefahren durch Blei durch Reinigen, Streichen, Abdecken und/oder Ersetzen/Reparieren von Innen- und Außenkomponenten des Hauses.

Vor der Geburt ihres Kindes erhielten die Teilnehmer, die randomisiert der Lead Reduction Group zugeteilt wurden, Kontrollen zur Reduzierung der Bleigefährdung, um die Bleibelastung in Wohngebieten zu reduzieren.

ANDERE: Verletzungsreduktionsarm
zufällige Zuweisung, um Interventionen zur Kontrolle der Verletzungsgefahr zu erhalten.
Bewertung des Hauses auf mögliche Verletzungsgefahren.

Im Alter zwischen 3 und 6 Monaten erhielten Teilnehmer, die randomisiert dem Arm zur Reduzierung von Verletzungen zugeteilt wurden, Kontrollen der Verletzungsgefahr, um die Anzahl der häuslichen Verletzungen zu reduzieren.
